Ankle ligament reconstruction after wide resection of the osteosarcoma of the distal fibula: a case report.

BACKGROUND: Restoration of the lateral ankle after distal fibulectomy is a difficult reconstructive procedure. Many surgical techniques have … ent tumor in the biopsy site. After distal fibulectomy and wide resection, split tibialis posterior tendon transfer to the remaining peroneus brevis restored the stability of the ankle. The pain resolved within 3 months. The ankle was stable and no recurrence of the cancer was found at a 7 year follow-up. CONCLUSION: Reconstruction following distal fibulectomy a …

New concept of and approach to clubfoot treatment: section II--correction of the clubfoot.

The second section of three on the new concept of and approach to treatment of clubfoot presents a radically new method of correction of the abnormal subtalar rotation presented in Section I. The nonoperative and operative approach is based on applying pressure in the opposite direction. Thus, the calcaneus and cuboid are pushed laterally anterior to the ankle joint and the calcaneus is pushed medially toward the medial malleolus posterior to the ankle joint. A detailed description of the surgical technique, including a method of maintaining flexor tendon function and protection of ankle joint

Progress in the management of the paralytic hip in myelomeningocele.

1. The primary aim of management at the hip joint in the patient with myelomeningocele is the prevention and correction of deformity, usually a flexion deformity. 2. It is necessary to treat both flexion deformity and the causative muscle imbalance; correction of this imbalance will also prevent subsequent dislocation of the lip. 3. The psoas tendon is generally excised; transfer is reserved for a few children with excellent walking potential and a good prognosis from all viewpoints. 4. Flexion deformity is treated by a soft tissue release procedure (for children under the age of eight years a

Radiation-induced brachial plexus paralysis.

Fifteen patients with radiation-induced brachial plexus paralysis were studied. Thirteen women had been treated for breast cancer. Two men d … had been performed were not relieved. Two of these were later considered excellent candidates for a tendon transfer in the hand. One did not desire surgery. The other underwent operation and showed marked improvement of her grasp and general hand function.

Gait analysis after tibialis anterior tendon rupture repair using Z-plasty.

Other than limited reports regarding surgical outcomes, little information is available regarding whether Z-plasty of the tibialis anterior results in normal gait kinematics. We included 5 patients with spontaneous ruptures in the present retrospective study. The mean interval between rupture and operative treatment was 2.6 ± 2.6 months (standard deviation). The mean age of the patients was 63 (range 40 to 80) years. All patients were treated operatively with Z-plasty. Gait analysis was used to study the outcome, comparing the operated and nonoperated limbs. The patients were tested after a me

Augmented Hummelsheim procedure to treat complete abducens nerve palsy.

BACKGROUND: Horizontal strabismus due to a weak rectus muscle can be treated with an augmented Hummelsheim procedure, in which both vertical rectus muscle tendons are split, resected by 4 mm, and reattached to the sclera adjacent to the weak rectus muscle. Compared with vertical rectus transposition, the procedure spares two ciliary vessels and does not require placement of augmentation sutures. In this study, we evaluated binocular alignment and ocular motility in patients with abducens nerve palsy treated with an augmented Hummelsheim procedure. METHODS: The medical records of consecutive p

Surgical management of chronic complete sixth nerve palsy.

BACKGROUND AND OBJECTIVE: to evaluate the results of strabismus surgery in patients with chronic complete sixth nerve palsy. PATIENTS AND METHODS: the patients with negative traction test underwent full tendon vertical rectus muscle transposition, whereas the patients with positive traction test had medial rectus recession and transposition surgery. Traction sutures were used in patients with deviations greater than 65 prism diopters (PD) in addition to medial rectus recession. RESULTS: the mean age of 6 female (35.3%) and 11 male (64.7%) patients was 45.1 ± 20.5 years (range: 13 to 80 years

Reconstruction of the digital flexor pulley system: a retrospective comparison of two methods of treatment.

The rare injury of closed rupture of the A2 and A3 flexor pulleys was treated using two non-encircling techniques of pulley reconstruction. Thirteen patients were treated with an extensor retinaculum graft (Group A). At a mean follow-up time of 48 months, the average PIP flexion was 97%, the power grip strength 96%, the pinch grip strength 100% and the thickening 94% of the uninjured side. Ten patients were treated with a free palmaris longus tendon grafts (Group B). At a mean follow-up time of 57 months, the average PIP flexion was 94%, the power grip strength 98%, the pinch grip strength 100

The effects of graft width and graft laxity on the outcome of caprine anterior cruciate ligament reconstruction.

We studied how initial graft size and initial graft laxity affected the biomechanics of anterior cruciate ligament (ACL) reconstruction at six months. Sixteen goats had bilateral reconstructions staged eight weeks apart. Autografts 4 and 7 mm wide were taken from the central patellar tendon (PT). Lax grafts were created by adding 4 mm slack to the graft before fixing. We reconstructed each joint using a combination of width and laxity treatments. Both factors were changed for the contralateral joint and all combinations appeared with equal frequency. At six months we measured the joint extensi

Regeneration of full thickness common extensor tendon tear after percutaneous microfragmented adipose graft.

Tennis elbow, also commonly known as lateral epicondylitis or common extensor tendinosis, is a common musculoskeletal injury in the adult population. Currently, the standard treatment regimen prescribed for this injury involves a combination of rest, physical therapy, bracing and anti-inflammatory medications. If refractory to these conservative measures, platelet-rich plasma has been shown effective. However, in the case of full thickness tears, surgery has remained the only treatment option until now. We present a case report of a 56-year-old man with a diagnosis of a left large full thickne

Traumatic Divergent Elbow Dislocation with Associated Forearm Amputation in an Adult: A Case Report.

CASE: A 49-year-old right-hand-dominant man sustained an auger-related injury that resulted in open dislocation at the left wrist and palm, with complete amputation of the distal aspect of the forearm and the hand. The injury at the elbow included instability with an ulnar coronoid fracture, posterior dislocation of the ulna, and posterolateral dislocation of the radius. To restore stability of the forearm stump and elbow, we performed a complete resection of the radius, open reduction and internal fixation of the coronoid tip, a repair of the lateral collateral ligament, and transfer of the d

Intramedullary screw fixation for balancing of the dysvascular foot following transmetatarsal amputation.

Transmetatarsal amputation is a viable alternative to more proximal pedal amputations or leaving a deformed partial forefoot amputation. Balancing the transmetatarsal amputation in order to correct soft-tissue deformities is an accepted approach and consists of various tendon transfers, However, in the recently re-vascularized limb or in those patients who can not undergo re-vascularization, additional incisions to balance the transmetatarsal amputation are not performed. The authors present a simple and effective technique utilizing a large diameter, cannulated screw extending from the residu…
