CONCLUSIONS: Transcrusal craniotomy is a safe and effective approach to the petroclival region. Excellent hearing results can be expected with this technique.

A 25-year-old female patient with a history of progressive dysphagia of one year duration was detected to have intraluminal polypoidal mass in the oesophagus on barium contrast study and computed tomography. Direct laryngoscopy and flexible endoscopy confirmed the origin from pharyngo-oesophageal junction. Lateral cervical oesophagotomy and excision of a polypoidal mass of 15x6.5x3 cm size was done. The long-term clinical course of six patients with congenital central hypoventilation syndrome is described. During the neonatal period, the patients had prolonged apneas and hypoventilation, in the absence of cardiac, pulmonary, or neuromuscular disease. Hospital records from 1974 through 1983 contained the cases of 20 patients over 18 years old admitted with supraglottitis (epiglottitis). Most patients presented with pharyngitis and odynophagia, and were diagnosed by laryngoscopy and neck radiographs. Four patients required tracheostomies, and one was nasotracheally intubated because of impending airway obstruction. Surgical repair of a cleft palate was carried out in three horses. Mandibular symphisotomy allowed adequate exposure of the defect.
