A total of 32 anxiety patients (agoraphobia, panic syndrome, generalized anxiety syndrome) selected according to the DSM-III criteria were compared with 16 controls to determine distinguishing features of autonomic response patterns to a sequence of verbal and acoustic stimuli. In addition sensorimotor tasks were performed. The anxiety patients were classified into electrodermally stabile and labile subgroups on the basis of two reliably measurable psychophysiological parameters: the rate of electrodermal habituation to neutral stimuli and non-specific electrodermal activity. The electrodermal…

Objective: Individual patient characteristics are important in trying to understand why people respond differently to group CBT. Only a few … as important for the outcome of a 14-week group CBT intervention for social phobia, panic disorder, agoraphobia and major depressive disorder. The four themes were Complexity & severity, External circumstances, Attitudes & coping and Cognitive ability & reflection level. The therapists explained how they perceived higher complexity and severity in the patient …

BACKGROUND: Although prior genetic studies of interview-assessed fears and phobias have shown that genetic factors predispose individuals to … DSM-5 constructs of animal and blood-injection-injury phobias but do not support the separation of agoraphobia from social phobia. The results also do not show a coherent genetic factor for the DSM-IV and DSM-5 situational phobia. Finally, the patterns of co-morbidity across individual fears and phobias produced by genetic and environmental influences differ …

Sensory-processing sensitivity is assumed to be a heritable vulnerability factor for shyness. The present study is the first to examine sensory-processing sensitivity among individuals with social anxiety disorder. The results showed that the construct is separate from social anxiety, but it is highly correlated with harm avoidance and agoraphobic avoidance. Individuals with a generalized subtype of social anxiety disorder reported higher levels of sensory-processing sensitivity than individuals with a non-generalized subtype. These preliminary findings suggest that sensory-processing sensitiv

This study evaluated the role of perceived health in predicting anxiety symptoms, bodily vigilance and agoraphobic cognitions among 71 individuals (30 females; mean age 19.9 years, SD=3.1) without a psychiatric history, including non-clinical panic attacks. Results indicated that, relative to anxiety sensitivity, perceived health was a distinct construct that incrementally predicted bodily-oriented catastrophic thinking (8% of unique variance) and heart-focused anxiety (13% of unique variance). Moreover, perceived health significantly incrementally predicted anxious arousal symptoms (9% of uni

Chronic and treatment refractory cases of panic disorder are not uncommon. We report on a man completely remitted from panic disorder after olanzapine and paroxetine combined treatment, and the possible modulating anti-anxiety effect of olanzapine is discussed. Olanzapine augmentation in panic and other anxiety disorders is deserving of more clinical attention.

The aim of the present study was to examine the relationship between anxiety sensitivity, as measured by the Anxiety Sensitivity Index (ASI), and four dimensions of behavioural reactivity to a single inhalation of 35% carbon dioxide (CO(2)) in 31 patients with panic disorder. ASI scores correlated positively with baseline State-Trait Anxiety Inventory scores but did not correlate with post-CO(2) scores. Correlational analyses revealed a significant, albeit modest, correlation between anxiety sensitivity and cognitive symptoms induced with CO(2). However, no significant association was found be

The syndrome of phobic postural vertigo is characterized by a combination of non-rotational dizziness, subjective disturbance in the upright stance and gait, despite normal clinical balance tests. Phobic postural vertigo most frequently occurs in form of attacks. It may be accompanied by anxiety and panic symptoms. It is almost invariably associated with particular constellations of perceptual stimuli or social situations. There is a tendency for rapid conditioning, generalization, and avoidance behavior to develop. Frequently, onset of condition follows periods of particular psychosocial stre

The question is addressed as to whether cognitive-behavior treatment delivered as a routine service in a specialist clinic is effective in the long term. Of 124 consecutive patients completing treatment for panic disorder or social phobia, 93 were assessed an average of 2 years following treatment. The treatment produced significant improvement in measures of symptoms, avoidance, and disablement during treatment and further significant improvement during the follow-up. A quarter of participants no longer met diagnostic criteria, had not sought further treatment, and their anxiety had not troub

OBJECTIVE: With recent advances in molecular genetics, the rate-limiting step in identifying susceptibility genes for psychiatric disorders has become phenotype definition. The success of psychiatric genetics may require the development of a "genetic nosology" that can classify individuals in terms of the heritable aspects of psychopathology. The authors' aim is to begin to apply this analysis to the anxiety disorders, focusing on panic and phobic disorders.
METHOD: Two parallel traditions of defining anxiety phenotypes are reviewed: the first, more closely identified with clinical psychiatry

Controlled studies indicate that cognitive-behavioral therapy eliminates panic attacks in greater than 80% of patients who suffer from panic disorder. However, because most of the screening procedures used in those studies called for excluding patients who were depressed, a question arises as to the extent to which these results apply to patients who are clinically depressed in addition to having panic attacks. Accordingly, an attempt was made in the present study to determine whether or not panic patients who are clinically depressed could be treated as successfully as those who are not clini

Recently, heart rate (HR) variability has received considerable attention, and a decreased HR variability has been linked to a significant risk of cardiovascular illness. We have previously reported such a decreased variability in panic disorder patients. In this study, we report on HR variability in 12 depressed and 6 panic disorder patients at baseline and 1 and 3 weeks of treatment with imipramine as measured by the standard deviation, mean consecutive difference and the standard deviation of the mean consecutive difference of the R-R intervals in supine, supine deep breathing and standing

The anxiogenic effects of yohimbine, a specific alpha-2-receptor antagonist were examined by administering 20 mg yohimbine orally to 8 panic patients on placebo treatment, 7 panic patients on alprazolam treatment and 12 controls using a double-blind randomized design, instructions that minimized the expectancy of experiencing a panic attack and two additional structured situations. Yohimbine induced more pronounced increases in anxiety and panicky ratings, norepinephrine secretion, maximum heart rate and high heart rate variability and decreases in skin temperature in panic patients compared w

Symptomatic agoraphobics, recovered agoraphobics, and normal control subjects completed a series of sentence stems that had either ambiguous or unambiguous meanings, and had either a potentially threatening or a nonthreatening connotation. The written completions made by subjects to these stems were classified as indicating either a biased (i.e. threat-related) or unbiased interpretation of the meaning of the stem, and if a biased interpretation was made, whether the subject indicated efforts at adaptive coping with the perceived threat. Results indicated that symptomatic agoraphobics exhibite

Phobic avoidance--avoiding entering specific fear-provoking situations--can severely restrict patients' lives. To determine the frequency with which patients having some degree of phobic avoidance will discuss these fears with their physician and factors in their decision to discuss, a mailed survey was conducted in Franklin County, Ohio. Data from 32 respondents to a mental health questionnaire who met criteria for phobic avoidance were analyzed. Only 28% indicated that they had seen a physician about their fears. The presence of panic attacks and duration of fears and avoidance were key fact
