Sphincter-sparing local excision and hypofractionated radiation therapy for anorectal melanoma: a 20-year experience.

BACKGROUND: Anorectal melanoma is a rare disease with a poor prognosis. Because survival is determined by distant failure, many centers have adopted sphincter-sparing excision for primary tumor control. However, this approach is associated with high rates of local failure (∼50%). In this study, the authors report their 20-year experience with sphincter-sparing excision combined with radiation therapy (RT) for the treatment of localized anorectal melanoma. METHODS: The authors reviewed the records of 54 patients with localized anorectal melanoma who were treated at the University of Texas MD A

Unraveling the rarity: p16-positive and p53-positive locally advanced anal cancer in a person living with HIV.

INTRODUCTION: Anal squamous cell carcinoma (ASCC) is rare in the general population but is associated with significant morbidity and mortality, particularly among people living with HIV (PLWH). Patient outcomes are influenced by human papillomavirus (HPV) status, immune function, and tumor biology. CASE REPORT: We report a case of a 56-year-old man with long-standing HIV infection on stable antiretroviral therapy (ART), diagnosed with locally advanced ASCC (T4N1cM0). He received standard concurrent chemoradiotherapy (CRT) with mitomycin C and 5-fluorouracil (5-FU). Treatment was complicated b

Screening for anal high-grade squamous intraepithelial lesions with flexible endoscopy in high-risk individuals.

BACKGROUND: Anal squamous cell carcinoma is increasingly prevalent among men who have sex with men (MSM) living with HIV, related to anal dysplasia associated with persistent Human Papillomavirus (HPV) infection. While high-resolution-anoscopy (HRA), using colposcopy and a rigid anoscope, is the diagnostic standard, it is resource-intensive and not widely available. This study evaluates flexible endoscopy compared to cytology and anal HPV for detecting high-grade squamous intraepithelial lesions (HSIL) in a structured screening protocol. PATIENTS AND METHODS: HIV-positive MSM (n = 304) were c

Health care provider characteristics associated with anal cancer screening in an urban academic medical system in the United States.

BackgroundNew guidelines recommend anal cancer screening for priority populations with increased likelihoods of anal cancer including people with a history of HIV, gynecologic cancers, condyloma acuminata, and solid organ transplantation. However, little is known about U.S.-based screening practices prior to these guidelines.MethodsWe examined outpatient clinical encounters between 01/01/2015 and 08/01/2022 and analyzed provider characteristics and screening behaviors in an urban medical system in Boston, U.S. We used chi-squared tests and logistic regression to identify provider factors assoc

Lower urinary tract sub-structures as predictors of late urinary toxicity in concurrent chemo-radiotherapy for anal cancer.

BACKGROUND AND PURPOSE: Late toxicity is substantial after chemotherapy for anal cancer. This study aimed to investigate the relationship between radiation dose to lower urinary tract sub-structures and the risk of late urinary toxicities, in patients with anal cancer treated with chemoradiotherapy or radiotherapy. MATERIALS AND METHODS: From 2015 to 2021, 314 patients with localized anal cancer were included in a national prospective registration study. Urinary toxicity (CTCAE) was scored during treatment (acute toxicity) and at one- and three-years follow-up (late toxicity). Lower urinary t

ADAR1 mRNA quantification for predicting HSIL in persons with HIV and abnormal anal cytology.

Anal squamous cell carcinoma is a significant concern among people with HIV (PWH), with high-grade squamous intraepithelial lesions (HSIL) as its precursor. Current screening, primarily based on abnormal anal cytology, often results in unnecessary high-resolution anoscopies (HRA) due to limited specificity. This study evaluates whether quantifying Adenosine Deaminase Acting on RNA-1 (ADAR1) mRNA in anal swabs improves HSIL prediction and optimizes HRA selection. In this prospective cohort, HIV-positive adults with abnormal anal cytology underwent HRA. ADAR1 mRNA levels were quantified from ana

ROAR-A: re-optimization based Online Adaptive Radiotherapy of anal cancer, a prospective phase II trial protocol.

BACKGROUND: Chemo-radiotherapy with curative intent for anal cancer has high complete remission rates, but acute treatment-related gastrointestinal (GI) toxicity is significant. Toxicity occurs due to irradiation of surrounding normal tissue. Current radiotherapy requires the addition of large planning margins to the radiation field to ensure target coverage regardless of the considerable organ motion in the pelvic region. This increases the irradiated volume and radiation dose to the surrounding normal tissue and thereby toxicity. Online adaptive radiotherapy uses artificial intelligence to a

Inflammatory Bowel Disease and the Risk of Anal Squamous Intraepithelial Lesions and Anal Cancer in Men Who Have Sex With Men.

BACKGROUND: The risk of anal cancer is increased in patients with human immunodeficiency virus (HIV), inflammatory bowel disease (IBD), and among men who have sex with men (MSM). High grade squamous intraepithelial lesions (HSILs) are precursor lesions to anal squamous cell carcinoma (SCC), and treatment of these lesions can decrease progression to anal SCC. This study aims to determine the prevalence of HSIL and anal cancer among MSM patients with and without IBD referred for anal cancer screening. METHODS: This is a retrospective study of all MSM patients seen at an anal squamous intraepith

Facilitators of and barriers to high-resolution anoscopy adherence among men who have sex with men: a qualitative study.

UNLABELLED: Background Anal cancer is a rare malignancy that disproportionately affects men who have sex with men (MSM) and HIV-infected people. Anal cancer is associated with human papillomavirus (HPV) in upward of 90% of cases and is preceded by pre-cancerous changes in cells of the anal canal. High-resolution anoscopy (HRA) is used for the detection, treatment and continued monitoring of anal dysplasia. Practice guidelines regarding anal cancer prevention vary by jurisdiction and institution, and patient engagement is low for high-risk populations such as MSM. The purpose of this study is t

Human Papillomavirus Genotyping of Incidental Malignant and Premalignant Lesions on Hemorrhoidectomy Specimens.

Routine histopathologic examination of hemorrhoidectomy specimens is controversial having been described as not useful and expensive with few of these common cases demonstrating incidental lesions. However, unexpected premalignant and malignant lesions have been detected on excised hemorrhoids. The high-risk human papillomavirus (HR-HPV) types associated with these incidentally identified high-grade lesions are presently unknown. We aimed to identify cases of incidental high-grade anal intraepithelial neoplasia (HG-AIN) and anal squamous cell carcinoma incidentally discovered on hemorrhoidecto

Prolonged antiretroviral therapy is associated with fewer anal high-grade squamous intraepithelial lesions in HIV-positive MSM in a cross-sectional study.

OBJECTIVE: HIV-positive men who have sex with men (MSM) are at increased risk of anal cancer. We evaluate the risk factors for anal high-grade squamous intraepithelial lesion (HSIL) (the precursor of anal cancer) in HIV-positive MSM. METHODS: In this cross-sectional study within a cohort, 320 HIV-positive MSM were screened by anal cytology followed by high-resolution anoscopy (HRA) in case of abnormal cytology. Risk factors for anal HSIL were analysed. RESULTS: Men were mostly middle-aged Caucasians with median CD4+ T lymphocytes of 638 cells/µL, 87% on combined antiretroviral therapy (cART)

Cetuximab Plus Chemoradiotherapy for HIV-Associated Anal Carcinoma: A Phase II AIDS Malignancy Consortium Trial.

Purpose Squamous cell carcinoma of the anal canal (SCCAC) is characterized by high locoregional failure (LRF) rates after definitive chemoradiation (CRT), associated with anogenital human papilloma virus, and often appears in HIV infection. Because cetuximab enhances the effect of radiation therapy in human papilloma virus-associated oropharyngeal SCC, we hypothesized that adding cetuximab to CRT would reduce LRF in SCCAC. Methods Forty-five patients with stage I to III SCCAC and HIV infection received CRT: 45 to 54 Gy radiation therapy to the primary tumor and regional lymph nodes plus eight

Fistula-Associated Anorectal Cancer in the Setting of Crohn's Disease.

BACKGROUND: Cancer arising from perianal fistulas in patients with Crohn's disease is rare. There are only a small series of articles that describe sporadic cases of perianal cancer in Crohn's disease fistulas. Therefore, there are no clear guidelines on how to appropriately screen patients at risk and choose proper management. OBJECTIVE: The purpose of this study was to describe patients diagnosed with cancer in perianal fistulas in the setting of Crohn's disease. DESIGN: The study involved an institutional review board-approved retrospective review of medical charts of patients with perian

High-grade Dysplasia in Anogenital Warts of HIV-Positive Men.

IMPORTANCE: Human papillomavirus (HPV)-induced anogenital lesions are very frequent in men who have sex with men (MSM) who are HIV-positive (HIV+). Anogenital warts (AGWs) are considered benign lesions caused by low-risk HPV-types, whereas anogenital dysplasias are potential cancer precursors associated with high-risk HPV-types. Both types of lesions can usually be distinguished clinically. OBJECTIVE: To describe a case series of HIV+ MSM with typical AGW that harbored different grades of dysplasia. DESIGN, SETTING, AND PARTICIPANTS: For this retrospective virological analysis, we recruited

Laser capture microdissection as a tool to evaluate human papillomavirus genotyping and methylation as biomarkers of persistence and progression of anal lesions.

INTRODUCTION: Anal squamous cell carcinoma is preceded by persistent infection with high-risk human papillomavirus (HPV) and the cancer precursor, high-grade squamous intraepithelial lesion (HSIL). Detection of specific HPV genotypes and HPV-related biomarkers may be an option for primary anal screening. However, more data on the natural history of HPV-related anal lesions are required. The outcomes from this study will enhance our understanding of the clinical and biological behaviour of HPV-related anal lesions and inform the development of future HPV genotype and/or biomarker screening test
