Attention-deficit hyperactivity disorder involves differential cortical processing in a visual spatial attention paradigm.

OBJECTIVE: Inattention is undoubtedly one of the main characteristics of Attention-deficit hyperactivity disorder (ADHD). Nevertheless, a growing corpus of evidence shows that not all attentional processes are affected in this condition. This study aimed to explore the distribution of attentional resources in children with ADHD via a spatially shifted double-oddball visual task. METHODS: We recorded event-related potentials (ERPs) for all visual stimuli. Subjects were instructed to allocate attention in a specific area of visual space while ignoring all stimuli presented outside. Ten male chi

Multiple components of lateral posterior parietal activation associated with cognitive set shifting.

Posterior parietal activation has commonly been observed in previous neuroimaging studies in association with flexible shifting of cognitive set. However, it is not clear whether the parietal activation reflects cognitive processes intrinsic to the shifting itself or other confounding factors such as spatial attention. To address this issue, the Wisconsin Card Sorting Task (WCST) was modified such that spatial components were eliminated from the sensory and motor aspects of the task. Moreover, a visual instruction of a next dimension was introduced to eliminate cognitive processes related to t

Functional anatomy of impaired selective attention and compensatory processing in autism.

In autism, physiological indices of selective attention have been shown to be abnormal even in situations where behaviour is intact. This divergence between behaviour and physiology suggests the action of some compensatory process of attention, one which may hold clues to the aetiology of autism's characteristic cognitive phenotype. Six subjects with autism spectrum disorders and six normal control subjects were studied with functional magnetic resonance imaging while performing a bilateral visual spatial attention task. In normal subjects, the task evoked activation in a network of cortical r

Voluntary orienting is dissociated from target detection in human posterior parietal cortex.

Human ability to attend to visual stimuli based on their spatial locations requires the parietal cortex. One hypothesis maintains that parietal cortex controls the voluntary orienting of attention toward a location of interest. Another hypothesis emphasizes its role in reorienting attention toward visual targets appearing at unattended locations. Here, using event-related functional magnetic resonance (ER-fMRI), we show that distinct parietal regions mediated these different attentional processes. Cortical activation occurred primarily in the intraparietal sulcus when a location was attended b

Visual defects in children after cerebral hypoxia.

Visual functions were examined in 18 survivors of perinatal hypoxia/ischemia with mild to severe neurological sequelae, aged between 3 months and 17 years, and in two patients, aged 8 and 13 years, who had suffered postnatal hypoxic events. All but two patients showed clear visual deficits ranging from mild defects in visual acuity, visual field size, and/or optokinetic nystagmus to blindness. In 5 patients, the visual field was restricted to tunnel vision, a finding which appeared to be specifically related to the hypoxic/ischemic nature of the brain damage. The severity of the visual defects

A cross-cultural study showing deficits in gaze-language coordination during rapid automatized naming among individuals with ASD.

Individuals with autism spectrum disorder (ASD) and their first-degree relatives demonstrate automaticity deficits reflected in reduced eye-voice coordination during rapid automatized naming (RAN), suggesting that RAN deficits may be a genetically meaningful marker of ASD language-related impairments. This study investigated whether RAN deficits in ASD extend to a language typologically distinct from English. Participants included 23 Cantonese-speaking individuals with ASD and 39 controls from Hong Kong (HK), and age- and IQ-comparable groups of previously-studied English-speaking individuals

Cervical Spine Deformity: Indications, Considerations, and Surgical Outcomes.

Cervical spinal deformity (CSD) in adult patients is a relatively uncommon yet debilitating condition with diverse etiologies and clinical manifestations. Similar to thoracolumbar deformity, CSD can be broadly divided into scoliosis and kyphosis. Severe forms of CSD can lead to pain; neurologic deterioration, including myelopathy; and cervical spine-specific symptoms such as difficulty with horizontal gaze, dysphagia, and dyspnea. Recently, an increased interest is shown in systematically studying CSD with introduction of classification schemes and treatment algorithms. Both major and minor co

Population response to natural images in the primary visual cortex encodes local stimulus attributes and perceptual processing.

The primary visual cortex (V1) is extensively studied with a large repertoire of stimuli, yet little is known about its encoding of natural images. Using voltage-sensitive dye imaging in behaving monkeys, we measured neural population response evoked in V1 by natural images presented during a face/scramble discrimination task. The population response showed two distinct phases of activity: an early phase that was spread over most of the imaged area, and a late phase that was spatially confined. To study the detailed relation between the stimulus and the population response, we used a simple en

Eye fixation-related potentials (EFRPs) during object identification.

Eye fixation-related potential (EFRP) measures electrical brain activity in response to eye fixations. The aim of the current study was to investigate whether the EFRPs vary during consecutive eye fixations while subjects were performing an object identification task. Eye fixations evoked P1 and N1 components at the occipital and parietal recording sites. The latency of P1 component increased during consecutive fixations. The amplitude of P1 increased and the amplitude of N1 decreased during consecutive fixations. The results indicate that EFRPs are modulated during consecutive fixations, sugg

Surgical management of orbital trapdoor fracture in a pediatric population.

PURPOSE: Orbital trapdoor fractures are pure orbital floor fractures with herniation and entrapment of the orbital contents, leading to restricted eye movement and diplopia. Trapdoor fractures in children have been discussed widely in published reports; however, the treatment policy and outcome remain controversial, although early treatment has been advocated. Our retrospective study analyzed the long-term results of pediatric patients undergoing surgery for trapdoor fractures to determine the outcome in relation to the type of fracture and the timing and technique of intervention. PATIENTS A

Mapping multiple visual areas in the human brain with a short fMRI sequence.

It is a fundamental insight of neuroscience that the cerebral cortex is divided into spatially separated and functionally distinct areas. In this study, we tried to map a large number of visual areas in individual subjects passively viewing a simple stimulus sequence during functional magnetic resonance imaging (fMRI) at 1.5 T. The blocked stimulus sequence contrasted static object photographs with video takes of movement through natural indoor and outdoor scenes, alternated with a control fixation task. Two runs of the 5-min sequence sufficed to invoke 29 distinguishable activations, 16 (13 b

Surgically induced incomitance following unilateral versus bilateral medial rectus recessions for esotropia.

BACKGROUND: For small-angle strabismus, unilateral medial rectus recession (UMR) offers many advantages over bilateral procedures (BMR), including a shorter operative time, faster recovery and fewer complications, while preserving the contralateral medial rectus muscle. However, the asymmetric nature of the procedure poses a theoretical risk of induced incomitance in the direction of action of the recessed muscle. This study aims to compare the incidence and nature of induced incomitance following unilateral and bilateral medial rectus recession in the management of non-accommodative esotropia

Differentiating social preference and social anxiety phenotypes in fragile X syndrome using an eye gaze analysis: a pilot study.

BACKGROUND: Fragile X syndrome (FXS) is the leading inherited cause of autism spectrum disorder, but there remains debate regarding the clinical presentation of social deficits in FXS. The aim of this study was to compare individuals with FXS to typically developing controls (TDC) and individuals with idiopathic autism spectrum disorder (ASD) across two social eye tracking paradigms. METHODS: Individuals with FXS and age- and gender-matched TDC and individuals with idiopathic ASD completed emotional face and social preference eye tracking tasks to evaluate gaze aversion and social interest, r

Dynamic Causal Modelling of Active Vision.

In this paper, we draw from recent theoretical work on active perception, which suggests that the brain makes use of an internal (i.e., generative) model to make inferences about the causes of sensations. This view treats visual sensations as consequent on action (i.e., saccades) and implies that visual percepts must be actively constructed via a sequence of eye movements. Oculomotor control calls on a distributed set of brain sources that includes the dorsal and ventral frontoparietal (attention) networks. We argue that connections from the frontal eye fields to ventral parietal sources repre

Evaluation and validation of sustained upgaze combined with the ice-pack test for ocular myasthenia gravis in Asians.

The accuracy of the ice-pack test for diagnosing myasthenia gravis (MG) has been reported to be excellent, however, the sensitivity is low in patients with mild ptosis which is common in Asians who have absent or low double eyelid folds. Herein, we performed the ice-pack test after sustained upgaze and tested its validity for diagnosing MG. The study included 30 newly diagnosed MG patients with ptosis (median age 47 years, range 2-87 years) and 30 controls with ptosis other than MG (median age 58 years, range 5-83 years). All MG patients initially presented with ptosis and/or diplopia; 26 pati

Extra-ocular movement restriction and diplopia following orbital fracture repair.

PURPOSE: To report a series of patients with extra-ocular movement restriction and diplopia after orbital fracture repair, and determine the effect of timing of repair and the type of implant used. METHODS: A chart review was conducted identifying all patients >18years of age at our institution between June 2005 and June 2008 who underwent orbital fracture repair, and presented with clinically significant diplopia and extra-ocular movement restriction persisting longer than one month after repair. Data collected included timing of repair, implant used within the orbit, and need for revision.

Intentional gaze shift to neglected space: a compensatory strategy during recovery after unilateral spatial neglect.

Unilateral spatial neglect is a common neurological syndrome following predominantly right hemispheric stroke. While most patients lack insight into their neglect behaviour and do not initiate compensatory behaviours in the early recovery phase, some patients recognize it and start to pay attention towards the neglected space. We aimed to characterize visual attention capacity in patients with unilateral spatial neglect with specific focus on cortical processes underlying compensatory gaze shift towards the neglected space during the recovery process. Based on the Behavioural Inattention Test

Non-holistic coding of objects in lateral occipital complex with and without attention.

A fundamental issue in visual cognition is whether high-level visual areas code objects in a part-based or a view-based (holistic) format. Previous behavioral and neuroimaging studies that examined the viewpoint invariance of object recognition have yielded ambiguous results, providing evidence for either type of representational format. A critical factor distinguishing the two formats could be the availability of attentional resources, as a number of priming studies have found greater viewpoint invariance for attended compared to unattended objects. It has therefore been suggested that the ac
