Patellofemoral osteoarthritis in patients with operative treatment for patellar dislocation: a magnetic resonance-based analysis.

PURPOSE: the clinical role of patellofemoral (PF) osteoarthrosis (OA) in the outcome after PF stabilizing surgery is poorly understood. The study hypothesis was that PF cartilage lesions and OA are associated with a poor long-term outcome after PF stabilizing surgery. METHODS: the study cohort included thirty-seven patients who underwent PF stabilizing surgery by traditional nonanatomic procedures and were evaluated a minimum of 10 years (range 10-21) after surgery. PF OA was assessed by magnetic resonance (MR) images and plain radiographs obtained at follow-up. Median patient age at follow-u

[What's up in patella dislocation? The medial femoro-patellaire ligament (MFPL)].

Patella dislocation is an injury affecting teenagers. This pathology is associated in the majority of the cases with a dysplasia of the femoro-patellar joint. MFPL is a passive restraint to patellar lateral displacement. Rupture of the MFPL would predispose the patients to redislocate their patella. Therefore, a reconstruction of this ligament seems to be a logical proposal for the treatment of recurrent patellar instability. This surgery, carried out with three incisions of small size, would decrease the recurrent dislocation. However, a long-term analysis of the results of this surgery is re

[Clinical examination for patellofemoral problems].

Patellofemoral pain and instability account for 25% of all sports-related knee problems. However, definitive analysis seems to be difficult. Descriptive findings like "anterior knee pain", "patellofemoral pain syndrome", or "chondromalacia" are used instead of diagnosis. One reason may be the complex pathomechanics of the patellofemoral joint which is suspended between the pelvis, the femur, and the tibia and subsequently is influenced by the whole lower extremity from the hip down to the foot. On the other hand, different signs and functional tests are in use with at times lack of quantificat

Reliability of classifications derived from Cyriax's resisted testing in subjects with painful shoulders and knees.

STUDY DESIGN: Intrarater and interrater reliability. OBJECTIVES: Examine intrarater and interrater reliability of the resisted-testing component of Cyriax's selective tension testing for patients with painful shoulders and knees. BACKGROUND: Clinicians make diagnostic and intervention decisions about lesions in contractile tissues based on resisted testing. Diagnostic and intervention decisions require reliable data gathering, especially when more than 1 physical therapist manages a patient. No studies have examined agreement of the results of the resisted tests used in selective tension tes

Arthrogryposis and amyoplasia.

Arthrogryposis (multiple congenital joint contractures) is an uncommon problem. Because there are many causes, correct diagnosis is important to predict the natural history and determine appropriate treatment. Inconsistent terminology has caused confusion about both diagnosis and treatment. Amyoplasia, the most common type of arthrogryposis, is characterized by quadrimelic involvement and replacement of skeletal muscle by dense fibrous tissue and fat. Early physical therapy and splinting may improve contractures, but surgical intervention is often necessary. Aggressive soft-tissue releases in

Surgical correction of medial subluxation of the patella.

We evaluated the results of a surgical procedure to correct medial subluxation of the patella in 63 patients (65 knees), most of whom had undergone a lateral retinacular release. We performed a direct repair or a reconstruction of the lateral patellotibial ligament using locally available tissue such as strips of iliotibial band or patellar tendon. Followup averaged 53.7 months (range, 24 to 99). Outcome was based on the examiner's inability to clinically reproduce the patient's painful medial subluxation and on the patient's general impression of his or her improved functional status. Forty-f

Simplification of the Elmslie-Trillat procedure for patellofemoral malalignment. Is medial capsulorraphy necessary?

We describe a modification of the Elmslie-Trillat operation which includes a lateral retinacular release and medial realignment of the tibial tuberosity, but omits medial capsulorraphy. Seventy-five patients were reviewed with a follow up longer than 2 years and the results compared with the conventional Elmslie-Trillat procedure, which consists of lateral release, medial realignment of the tuberosity and medial capsulorraphy. The simpler operation can work as well and the period of rehabilitation is shorter. The incidence of injury to the saphenous nerve is reduced and the cosmetic appearance

[A case of dislocation of a double patella].

Natural history registries can address knowledge gaps and support the development of best practice guidelines to improve outcomes in rare conditions. The European Registries for Rare Endocrine and Bone conditions (EuRREB) Core Registry is a standardised, multi-national platform designed to collect harmonised clinical data across rare endocrine disorders and facilitate longitudinal real-world analyses. The aim of this study was to describe data collected in the Core Registry across eight endocrine main thematic groups (MTGs) during the first six years following its launch in 2019, with particul

[Restorative-reconstructive surgery of the sequelae of osteomyelitis of the lower extremities].

Results of reconstructive-restorative operations on 40 patients with sequellae of acute hematogenic osteomyelitis of lower extremities are described. Most frequent of them are pathologic dislocations, axial extremity, ankylosis and contractures of large vessels in a faulty position. Extrafocal compression-distraction osteosynthesis after Ilizarov was used. Little-traumatic character of the method, stable fixation and early functional loading gave smooth course of the postoperative period and good anatomo-functional results in most of the patients.

Patella alta and the adolescent growth spurt.

The presence of patella alta has been linked to recurrent dislocation of the patella and the patello-femoral stress syndrome. It is not known whether patella alta is an inherited or acquired trait. To investigate the relationship of patella during the adolescent growth spurt, serial orthoroentgenograms (growth study films) were retrospectively analyzed in 19 patients. Two were found in whom proximal patella migration could be correlated (r = 0.85) with femoral growth rate. This was significant at the p = 0.01 level. Girls had a higher correlation of patella height to growth rate than boys. In

Recurrent dislocation of patella following lateral retinacular release. A case report.

A case is presented with several total dislocations of patella after an operative lateral retinacular release for chondromalacia patellae. The patient, a 27-year-old woman, had no history of preoperative luxations or subluxations. She had, however, a Q-angle of 20 degrees--a fact that may have been significant in the pathogenesis.

Dynamic arthroscopy in retropatellar cartilage defect: identifying trochlear ridge impingement and guiding towards mini-trochleoplasty.

Retropatellar (RP) chondral defects are commonly associated with trauma, sports-related injuries, or patellar dislocations. Effective management typically requires both cartilage repair and correction of underlying predisposing biomechanical factors. However, conventional imaging modalities may fail to detect subtle abnormalities. We describe a young female with persistent anterior knee pain in the absence of trauma or instability. Imaging studies, including radiographs and magnetic resonance imaging (MRI), confirmed an RP chondral defect, with no evidence of patella alta, malalignment or signi

Caudal Regression Syndrome with Pressure Ulcers of the Foot: A Case Report.

Caudal Regression Syndrome (CRS) is a rare disorder which consists of abnormalities in the lumbosacral spine, rectum, urinary system and lower limbs. These abnormalities also include orthopaedic deformities, such as hip dislocation, knee-flexion contracture with popliteal webbing and talipes equinovarus. Because of the rarity of this condition, few medical personnel are aware of it. Here, we present a case of CRS in a 15-year-old girl with pressure ulcers on the lateral malleolus and plantar surface of the foot, which had come into contact with the wheelchair footrest. Although the foot ulcers

Improved limb salvage in popliteal artery injuries.

This study reviews the recent experience with popliteal artery injuries at the Massachusetts General Hospital. Twenty-two patients suffered 24 injuries. The overall limb salvage was 83%. Blunt trauma accounted for 19 of the cases and was associated with femur fractures, knee dislocations, and tibia-fibular and plateau fractures: four amputations (21%) resulted. There were five penetrating injuries from three gunshot wounds, one stab wound, and one laceration: no amputations occurred. The major factor in the amputated limbs was delay in diagnosis and therapy of the arterial injury associated wi…

[Stretching the triceps surae muscle after 40 degrees C warming in patients with cerebral palsy].

PURPOSE OF THE STUDY: Equinus in patients with cerebral palsy results from at least two factors: excessive contracture of the triceps surae and muscle retraction. Tendon surgery and progressive lengthening techniques using plaster walking boots can provide variable improvement in retraction. We compared the effect of this technique when applied with or without prior 40 degrees C warming in the same patients. We also assessed the efficacy of this treatment method in terms or degree of retraction, patient age, puberty maturity, and sex. MATERIALS AND METHODS: This series included 70 muscles in

Original surgical technique for the treatment of patellofemoral instability after failure of conservative treatment.

INTRODUCTION: Acute patellar dislocation is a common but serious injury that can significantly impact a patient's functional prognosis. The objective of this retrospective study is to evaluate the clinical outcomes of arthroscopic medial patellofemoral ligament (MPFL) reconstruction and plication of the medial patellar retinaculum using suture anchors in adolescent patients with first-time acute patellar dislocation (APD) and MPFL insertion injury. HYPOTHESIS: Tightening repair of the medial retinaculum complex can result in favorable clinical and functional outcomes in this patient populatio
