Obesity in pregnancy: addressing risks to improve outcomes.

The rapidly increasing rates of obesity among women of childbearing age, not only in the United States but also across the globe, contribute to increased risks during pregnancy and childbirth. Overweight and obesity are quantified by body mass index (BMI) for clinical purposes. In 2010, 31.9% of U.S. women aged 20 to 39 years met the definition of obesity, a BMI of 30 kg/m or greater. Across the life span, obesity is associated with increased risks of hypertension, cardiovascular disease, diabetes, sleep apnea, and other diseases. During pregnancy, increasing levels of prepregnancy BMI are ass

Early educational and behavioral RCT interventions to reduce maternal symptoms of psychological trauma following preterm birth: a systematic review.

Early behavioral and educational interventions have been developed to reduce maternal symptoms of psychological trauma (depression, anxiety, parenting stress, acute stress disorder, and posttraumatic stress disorder) following preterm birth (PTB). Aims of this systematic review were to critically assess study methodology and provide a synthesis of existing randomized control trial (RCT) interventions and to estimate effects of the interventions in reducing the maternal symptoms across studies. Four electronic databases were systematically searched to locate relevant RCTs using preestablished e

Hyperoxia in very preterm infants: a systematic review of the literature.

Supplemental oxygen plays a critical role in the management of infants born at the lower limit of viability, but not without the risk of complications resulting from high levels or prolonged exposure. Longitudinal studies of very premature infants, born at less than 28 weeks' gestation, establish a clear relationship between pulse oximetry saturation readings above 92%, or hyperoxia, and development of severe retinopathy of prematurity, chronic lung disease, and brain injury. Hyperoxia is neither natural nor random. It is an unintended consequence of intervention. A comprehensive review of the

Continuity and change in mothers' narratives of perinatal hospice.

OBJECTIVES: (1) To broadly explore the experiences of women who chose to continue pregnancies affected by lethal fetal diagnoses and (2) to develop knowledge useful to nurses and other healthcare professionals who provide perinatal hospice (PH) care. DESIGN: Qualitative descriptive study using narrative analysis. PARTICIPANTS: Fifteen women who learned during their pregnancies of a lethal fetal diagnosis and chose to continue the affected pregnancies. METHOD: Participants' stories of their PH experiences were recorded in face-to-face interviews. A qualitative approach using narrative analys

Preeclampsia: pathophysiology and implications for care.

Nurses are increasingly encountering pregnant/postpartum women with hypertensive disorders of pregnancy, of which preeclampsia is one of the most common. The Joint Commission published a Sentinel Event Alert in 2010 on prevention of maternal death. This report notes that one of the 5 leading causes of pregnancy-related mortality between 1991 and 1997 was "hypertensive disorder." Preeclampsia presents significant risk to the health of the mother and the fetus. Clearly, nurses must understand the pathophysiology, assessment, management, recurrence risk, and long-term implications of preeclampsia

Postnatal steroids for the treatment of bronchopulmonary dysplasia: a complex case presentation.

Postnatal steroids are a controversial intervention for the prevention and treatment of bronchopulmonary dysplasia in preterm infants. Studies demonstrating a potential for steroid-related adverse effects, including growth and neurodevelopmental impairment, have significantly changed clinical practice over the past decade. There are circumstances in which the potential benefits may outweigh the risks associated with postnatal steroids. This case presentation details the hospital course and treatment plan for an extremely low-birth-weight infant who remained ventilator dependent at the age of 3

Acute volume resuscitation following obstetric hemorrhage.

Hemorrhage remains a leading cause of pregnancy-related morbidity and mortality. A rapid collaborative approach to management of the woman experiencing excessive blood loss is required to optimize outcomes. Successful treatment of the hemorrhaging pregnant woman is dependent on early recognition of bleeding and clinical manifestations of shock, treatment of the underlying cause of the hemorrhage, and simultaneous restoration of adequate circulating volume as well as blood component therapy. Early communication with providers, identified triggers for changes in patient condition, and drills/sim

Exploring the role of uterine artery embolization in the management of postpartum hemorrhage.

Postpartum hemorrhage is a potentially life-threatening obstetric emergency that requires prompt nursing and medical interventions. In the majority of cases, initial strategies such as fundal massage and uterotonic medications will effectively stop excessive bleeding. Unfortunately, the incidence and severity of postpartum hemorrhage are on the rise and peripartum hysterectomy remains a life-saving intervention in cases of intractable bleeding. As an emerging alternative to hysterectomy, uterine artery embolization (UAE) has demonstrated success rates of more than 90% in controlling postpartum

Complications of the late preterm infant.

One of the goals of Healthy People 2010 (set in 1998) was to reduce preterm birthrates from 11.6% to 7.6%. However, in 2004, the preterm birthrate of 12.5% was actually higher than the rate in 1998. Approximately 65% of this increase in prematurity rate is attributed to the increasing birthrate of the late preterm infant. Care of the late preterm infant is far more complicated than many hospital policies and clinical guidelines imply. It cannot be stressed enough to frontline clinicians that late preterm infants are not full-term infants. Their care should not be defined by the same policies a

Applications of probiotics for neonatal enteric diseases.

Probiotics are bacteria and yeast that have a beneficial effect on the maintenance of health. Probiotic microorganisms are inherently present in fermented food products. A renewed interest in probiotic therapy designed for both treatment and prevention of intestinal and extraintestinal disorders has resulted in studies in infants and children. This review focuses on the applications of probiotics in enteric conditions experienced by neonates and discusses future evaluations to develop recommendations for their use.

Surgical considerations in early pregnancy: ectopic pregnancy and ovarian torsion.

Ectopic pregnancy and ovarian torsion are 2 of the most common causes of acute abdominal pain in early pregnancy. The following article will discuss the physiology and pathophysiology, evaluation, diagnosis, clinical management options, and nursing considerations for ovarian torsion and ectopic pregnancy. Both conditions require timely diagnosis to prevent mortality and minimize morbidity, and both may require surgical management.

Assessment and evaluation of the woman with cardiac disease during pregnancy.

Maternal heart disease complicates 0.2 to 3% of pregnancies and is responsible for 10% to 25% of maternal deaths. Many healthy women manifest subtle signs of cardiac failure during uncomplicated pregnancy and birth. Classic symptoms of heart disease mimic common symptoms of late pregnancy, such as palpitations, shortness of breath with exertion, and occasional chest pain. A complete cardiovascular examination assists the healthcare team to fully assess and evaluate the pregnant woman with known heart disease. Detailed assessment of the woman throughout pregnancy may lead to initial discovery o

Neonatal lung remodeling: structural, inflammatory, and ventilator-induced injury.

The developing lung is subject to events, both prenatal and postnatal, that alter the normal developmental process. The degree of insult or injury affects how the lung functions at birth and then responds to the insult throughout childhood. In this article, only 3 of the influences are examined: structural, inflammatory, and mechanical. It is recognized that there is a plethora of other factors that influence lung remodeling.

Pregnancy following bariatric surgery.

Gastric bypass surgery for morbid obesity is considered an appropriate intervention when other weight-loss measures have proven unsuccessful. Weight loss often brings about improvement in overall health by lessening the effects of obesity-related comorbidities such as chronic hypertension and diabetes. In fact, the ability to become pregnant is enhanced, as weight loss often allows for a normalization of sex hormones. However, the nutrition challenges brought about by the surgery may have a profound impact on maternal health and pregnancy outcome. Surgical procedures for morbid obesity may be
