Classification and clustering on nocturnal polysomnography: distinctions and overlaps between central disorders of hypersomnolence.

STUDY OBJECTIVES: Differential diagnosis of narcolepsy type 2 (NT2) from type 1 (NT1) and idiopathic hypersomnia (IH) is challenging due to overlapping symptoms. We developed an automated method using nocturnal polysomnography (nPSG) data to differentiate these conditions and clinical controls (CCs), and explored varying sleep phenotypes within NT1, NT2, IH, and CCs. METHODS: We analyzed nPSG data from drug-free individuals with NT1, NT2, and IH, or CCs. Sleep features were derived at whole-night and per-quarter-night levels, including hypnogram, transition probability, hypnodensity, spindle,

Determinants of LV Myocardial Dyssynchrony in CHF: Impact of Mechanical and Electrical Dyssynchrony.

Chronic heart failure (CHF) patients often present with heterogeneous patterns of cardiac dyssynchrony. Although QRS prolongation (>150 ms) and left bundle branch block (LBBB) are classical markers of electrical dyssynchrony, their direct association with mechanical dyssynchrony remains controversial. This study aimed to identify key determinants of left ventricular (LV) synchrony using machine learning and explainable artificial intelligence techniques. A cohort of 412 CHF patients was stratified using integrated echocardiographic and electrocardiographic criteria: synchronous group (SG, n =

Computable Phenotype for Identifying Undiagnosed Hypermobile Ehlers-Danlos Syndrome: Protocol for a Development and Validation Study.

BACKGROUND: Hypermobile Ehlers-Danlos syndrome (hEDS) is a multisystemic hereditary connective tissue disorder characterized by generalized joint hypermobility, chronic pain, and a complex spectrum of comorbidities. Diagnosis relies on complex clinical criteria, leading to poor recognition by clinicians and fragmented care. Consequently, patients navigate the health care system for an average of 22.1 years before receiving a diagnosis, which substantially delays appropriate management. Electronic health records (EHRs) contain rich longitudinal data that, if systematically analyzed, could ident

Microbial Community Patterns Associated with Automated Teller Machine Keypads in New York City.

In densely populated urban environments, the distribution of microbes and the drivers of microbial community assemblages are not well understood. In sprawling metropolitan habitats, the "urban microbiome" may represent a mix of human-associated and environmental taxa. Here we carried out a baseline study of automated teller machine (ATM) keypads in New York City (NYC). Our goal was to describe the biodiversity and biogeography of both prokaryotic and eukaryotic microbes in an urban setting while assessing the potential source of microbial assemblages on ATM keypads. Microbial swab samples were

Optimising hospital pharmacy inventory for high-value, intermittent-demand drugs using a machine-learning approach with LightGBM-Tweedie regression: a single-centre, retrospective study.

OBJECTIVE: Hospital pharmacies operate under a persistent dilemma: ensuring uninterrupted access to essential medicines while minimising capital tied up in inventory. This challenge is most acute for drugs with lumpy or erratic demand, where traditional manual ordering often fails because of limited forecasting capability. Therefore, this study aimed to develop and evaluate a LightGBM-Tweedie regression model for forecasting weekly demand of high-value intermittent-demand drugs. METHODS: We conducted a retrospective machine learning-based study using weekly pharmacy usage data from a large te

Infrared spectroscopy with statistical analysis and machine learning for cancer risk assessment in inflammatory bowel disease patients.

Patients with inflammatory bowel disease (IBD) undergo regular colonoscopic surveillance due to their elevated risk of developing colorectal cancer (CRC). However, current clinical, endoscopic and histopathological risk stratification methods can be limited in sensitivity and objectivity, highlighting the need for complementary molecular approaches. In this study, Attenuated Total Reflectance-Fourier Transform Infrared Spectroscopy (ATR-FTIR) was used to acquire mid-infrared (MIR) spectra from IBD-associated endoscopic biopsy samples (30 patients analysed; 10 who developed dysplastic pre-cance

Uncovering complex correlations between multidimensional factors and breastfeeding duration using XGBoost.

BACKGROUND: Breastfeeding duration is influenced by a complex interplay of obstetric, physiological, and sociodemographic factors. Traditional linear statistical models often struggle to capture the high-dimensional, non-linear synergistic effects among these variables, while complex deep learning models are prone to overfitting on small clinical datasets. METHODS: To address these challenges, this study proposes an interpretable machine learning framework based on eXtreme Gradient Boosting (XGBoost). We analyzed clinical follow-up data from 210 postpartum women. The framework incorporates Ex

Double deletion mutation induced active site remodelling modulates substrate specificity in KPC-14 β-lactamase.

The emergence of KPC variants with altered substrate profiles poses a major challenge for β-lactam antibiotic therapy. KPC-14, which contains a double deletion of residues G242 and T243 (Y241_ΔGT_A244), exhibits enhanced resistance to ceftazidime and ceftazidime-avibactam while displaying reduced carbapenemase activity. However, the structural basis underlying this altered catalytic behaviour remains poorly understood. In this study, we built upon previous investigations and employed machine learning-driven enhanced sampling molecular dynamics simulations combined with unsupervised deep learni

Integrative multi-omics analysis identifies programmed cell death-related biomarkers and their environmentally relevant toxicological interactions in abdominal aortic aneurysm.

Abdominal aortic aneurysm (AAA) is a life-threatening vascular condition driven by interactions between genetic regulation and environmental influences, yet reliable biomarkers are lacking. Although individual programmed cell death (PCD) pathways have been implicated in AAA, their integrated dysregulation, particularly in environmental and toxicological contexts, remains unclear. Here, we conducted an integrative multi-omics analysis combining bulk transcriptomic datasets, single-cell RNA sequencing cohorts, and independently sequenced clinical samples to systematically characterize PCD dysreg

CFCDBN: Personalized Directional Brain Network Modeling of Cross-Frequency Coupling Alterations in Adolescent Anxiety Disorders.

Anxiety disorders (AD) are prevalent psychiatric conditions that profoundly impact adolescent neural development. Abnormal delta-beta cross-frequency coupling (CFC) has been identified as a key electrophysiological marker of altered neural dynamics in individuals with AD. However, most existing studies focus on static analysis within restricted brain regions and predefined frequency bands, which limits the understanding of large-scale dynamic neural communication. Therefore, we propose a novel cross-frequency coupling directed brain network (CFCDBN) framework, which integrates personalized CFC

ANXA5: A Multi-Omics-Derived, UPR-Associated Dual-Function Biomarker for Prognosis and Immunotherapy Response Prediction in Glioma.

Glioma, a highly aggressive brain cancer, thrives in an immunosuppressive tumor microenvironment (TME). The Unfolded Protein Response (UPR) is a key adaptive stress pathway implicated in therapy resistance. To discover clinically actionable biomarkers, we employed an integrated multi-omics strategy, combining single-cell and bulk transcriptomic data from multiple glioma cohorts. Using hdWGCNA network analysis, we identified a UPR-associated gene module that defined two molecular subtypes with stark survival differences. Through a rigorous machine learning pipeline (Random Survival Forest, LASS
