Avoiding hyperoxemia at the start of cardiopulmonary bypass while optimizing gas flow and temperature.

There seems to be a wide range of practice in relation to the optimum oxygen setting before, and at the start of, cardiopulmonary bypass. Even manufacturers of blood oxygenators vary in their suggestions for this phase of extracorporeal circulation. Most of these suggestions are based on peak performance, Association for the Advancement of Medical Instrumentation (AAMI) standards, experience, and legal considerations. Therefore, suggested gas:blood flow ratios will vary from no gas flow at the start of bypass, to a ratio setting of 1:1. On the other hand, suggested inspired oxygen concentratio

Benefits and risks of episiotomy: a review of the English-language literature since 1980. Part I.

The professional literature on the benefits and risks of episiotomy was last reviewed critically in 1983, encompassing material published through 1980. This paper reviews the evidence accumulated since then. (Part II follows in this issue.) It is concluded that episiotomies prevent anterior perineal lacerations (which carry minimal morbidity), but fail to accomplish any of the other maternal or fetal benefits traditionally ascribed, including prevention of perineal damage and its sequelae, prevention of pelvic floor relaxation and its sequelae, and protection of the newborn from either intracr

Sample size variation in single-time post-dose assessment vs multi-time post-dose assessment.

BACKGROUND: Many randomized trials measure a continuous outcome simultaneously at baseline and after taking the drug. For a single continuous post-treatment outcome, the sample size calculation is simple, but if there are assessments at multiple time point post-treatment then this longitudinal data may give more insights by analyzing the data using the repeated measures method. Also, if the sample size is calculated using the single time-point method for longitudinal data, it may lead to a larger than required sample size, increasing the cost and time. METHODS: In this research, an effort is

Pharmacological and Non-Pharmacological approaches to manage Tension-Type Headache: a Systematic Review.

INTRODUCTION: Tension-Type Headaches (TTH), particularly in its chronic form, constitutes a persistent pain syndrome that can substantially impair physical, cognitive, emotional, and social functioning. Although this is the most common type of headache, there is a lack of clear consensus on the optimal long-term treatment strategy. OBJECTIVES: A systematic review was conducted to evaluate the evidence for the effectiveness of pharmacological and non-pharmacological treatments in patients diagnosed with episodic or chronic TTH, focusing on improvements in headache pain and related clinical out

Blood pressure targets in adults with hypertension.

This is a protocol for a Cochrane review (intervention). The objectives are as follows: Primary objective To evaluate the benefits and harms of lower targets of blood pressure (BP; i.e. BP ≤ 135/85 mmHg) versus standard targets (i.e. BP ≤ 140 to 160/90 to 100 mmHg) for total mortality and morbidity in people 18 years or older, with elevated BP. Secondary objectives To evaluate the benefits and harms of lower targets versus standard targets for achieved systolic and diastolic BP in people 18 years or older, with elevated BP. To evaluate the benefits and harms of lower targets versus standard ta

Fragility of randomized clinical trials comparing surgical approaches for inguinal hernia repair: a systematic review.

INTRODUCTION: Randomized clinical trials (RCTs) remain the reference standard for estimating causal effects of surgical interventions when appropriately designed and conducted. However, P values alone do not describe how sensitive a dichotomous statistical-significance classification is to small changes in outcome counts. The Fragility Index (FI) and Reverse Fragility Index (RFI) are post hoc descriptive metrics that quantify the number of outcome-status changes required to alter statistical significance. This study aimed to evaluate FI/RFI for inguinal hernia repair (IHR) RCT findings, assess

Testing of Pre-Operative Peripheral Nerve Blocks in Randomised Controlled Trials: A Scoping Review.

BACKGROUND: Peripheral nerve blocks are widely used for anaesthesia in upper and lower limb surgery, but the methods used to assess their success vary substantially. This scoping review examined contemporary research practises and the extent to which trials report on peripheral nerve block evaluation. METHODS: A search was conducted on PubMed for randomised controlled trials published between 2014 and 2025 in anaesthesia journals, involving pre-operative peripheral nerve blocks in adults undergoing limb surgery. Two independent authors screened and extracted data for each trial. Outcomes incl

Optimal delivery of enteral protein in the critically ill: A systematic review and meta-analysis of randomised controlled trials.

BACKGROUND AND AIM: Critically ill patients experience acute muscle wasting, associated with impaired clinical outcomes. It has been suggested that greater dietary protein delivery may attenuate muscle wasting and improve outcomes, but the optimal dose is unknown. The aim of this systematic review and meta-analysis was to evaluate the effect of enteral protein delivered to achieve doses recommended within international guidelines (1.2-2.0 g/kg bodyweight/day) compared to enteral protein delivered below international guidelines (<1.2 g/kg/day) on mortality and clinical, patient-centred, and mus

An integrated individual patient-level database from JCOG trials in esophageal cancer: A research platform (JCOG2512A).

Randomized controlled trials (RCTs) provide robust evidence in esophageal cancer. Nonetheless, many clinically relevant questions cannot be fully addressed within a single trial. Nationwide registries provide complementary real-world insights but are limited by heterogeneity in patient selection, treatment practices, data quality, data completeness, and follow-up, which constrain the detailed evaluation of time-dependent outcomes and disease-specific endpoints. To address these limitations, the Japan Esophageal Oncology Group of the Japan Clinical Oncology Group established JCOG2512A, an integ

Effects of working memory training on depressive symptoms: a systematic review and meta-analysis of randomized controlled trials.

Working memory (WM) deficits are commonly observed across the depression spectrum and may represent a modifiable cognitive mechanism. This systematic review and meta-analysis evaluated the effects of WM training (WMT) on depressive symptoms and WM performance, and examined potential moderators and the association between WM gains and symptom change. Systematic searches of PsycINFO, Embase, and PubMed were conducted in December 2022 and updated in January 2025. Eligible studies were randomized controlled trials (RCTs) using WMT as the sole cognitive intervention with depressive symptoms as outc

Early Rehabilitation Interventions for Neurological Function Recovery in Patients with Acute Cerebral Infarction: A Meta-Analysis.

Acute cerebral infarction causes substantial neurological disability, and the efficacy of early rehabilitation-based interventions remains uncertain. This meta-analysis evaluated their effects on neurological recovery. PubMed, Cochrane Library, Embase, Web of Science, and China National Knowledge Infrastructure were searched for randomized controlled trials comparing early rehabilitation-based interventions with standard care. Random-effects models with restricted maximum likelihood and Hartung-Knapp adjustment were applied, and risk of bias was assessed using the Cochrane Risk of Bias 2 tool.

Immunotherapy failure in glioblastoma: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Immunotherapy has transformed outcomes in several solid tumors but has repeatedly failed to demonstrate benefit in glioblastoma. Early-phase signals of activity have not translated into improved survival in randomized trials, creating uncertainty regarding its true clinical value. METHODS: A systematic review and random-effects meta-analysis of randomized controlled trials (RCTs) evaluating immunotherapy in adult glioblastoma was conducted in accordance with PRISMA 2020. PubMed, Embase, and Scopus were searched through December 2025. Overall survival (OS) was the primary outcome;

Postnatal interventions to support newborn survival: updates for the Lives Saved Tool.

BACKGROUND: The period immediately after birth represents a highly vulnerable time for newborns, particularly in contexts without access to high-quality secondary care for small and/or sick newborns. Scaling up existing evidence-based interventions can avert much of the mortality and morbidity that occurs during the neonatal period. We summarise the evidence for interventions delivered postnatally and propose a set of effect estimates for inclusion in the Lives Saved Tool (LiST). METHODS: We first conducted a set of systematic reviews and updates on the most recent evidence from randomised co
