Abnormalities in control of ventilation in newborn infants with myelomeningocele.

OBJECTIVE: The objective of this study was to assess respiratory responses of newborn infants with myelomeningocele through pneumograms and carbon dioxide challenge, and to evaluate the possibility of predicting which patients with myelomeningocele acquired respiratory symptoms related to the Arnold-Chiari deformity and brain-stem dysfunction. METHODS: All surviving infants with spina bifida who were born at the University of Iowa Hospitals and Clinics (UIHC) or were transferred there on the first day of life between January 1987 and January 1991 were assessed with a pneumogram and CO2 challe

Severe infantile neuropathy with diaphragmatic weakness and its relationship to SMARD1.

A group of 13 patients with early onset diaphragmatic palsy in association with a progressive neuropathy is presented. All eight of those tested were found to have mutations in the same gene encoding the immunoglobulin mu-binding protein 2 (IGHMBP2) in patients with spinal muscular atrophy (SMA) with respiratory distress type 1. Six out of these eight patients had either homozygous or compound heterozygous mutations, and two had only a single heterozygous mutation. Detailed analysis of the clinical picture and the neurophysiological and histopathological findings indicated that these patients

Comparison of Shoulder Field Anesthesia (SFA) with Interscalene Block (ISB) for Analgesia After Arthroscopic Shoulder Surgery: A Randomized Noninferiority Trial.

BACKGROUND: An interscalene brachial plexus block (ISB) is commonly used for analgesia after shoulder surgery but is associated with neurologic and respiratory complications, including hemidiaphragmatic paralysis. Surgeon-performed shoulder field anesthesia (SFA) is a landmark-guided, phrenic nerve-sparing technique that may provide effective analgesia with fewer complications. The purpose of this study was to compare SFA with ISB for pain control after arthroscopic shoulder surgery. We investigated whether SFA would provide noninferior analgesia as measured by cumulative 48-hour postoperative

Loss of compensation afforded by accessory muscles of breathing leads to respiratory system compromise in the mdx mouse model of Duchenne muscular dystrophy.

Despite profound diaphragm weakness, peak inspiratory pressure-generating capacity is preserved in young mdx mice revealing adequate compensation by extra-diaphragmatic muscles of breathing in early dystrophic disease. We hypothesised that loss of compensation gives rise to respiratory system compromise in advanced dystrophic disease. Studies were performed in male wild-type (n = 196) and dystrophin-deficient mdx mice (n = 188) at 1, 4, 8, 12 and 16 months of age. In anaesthetised mice, inspiratory pressure and obligatory and accessory respiratory EMG activities were recorded during baseline a

Barium toxicity and the role of the potassium inward rectifier current.

INTRODUCTION: Barium is a stable divalent earth metal and highly toxic upon acute and chronic exposure. Barium is present in many products and involved in a number of industrial processes. Barium targets the potassium inward rectifier channels (IRCs) of the KCNJx gene family. Extracellular barium enters and strongly binds the potassium selectivity filter region resulting in blockade of the potassium conducting pore. IRCs are involved in numerous physiological processes of the human body and the most barium sensitive IRCs are highly expressed in all muscle types. OBJECTIVE: Our purpose was cor

Mechanisms of toxicity, clinical features, and management of acute chlorophenoxy herbicide poisoning: a review.

INTRODUCTION: Chlorophenoxy herbicides are used widely for the control of broad-leaved weeds. They exhibit a variety of mechanisms of toxicity including dose-dependent cell membrane damage, uncoupling of oxidative phosphorylation, and disruption of acetylcoenzyme A metabolism. Between January 1962 and January 1999, 66 cases of chlorophenoxy herbicide poisoning following ingestion were reported in the literature. FEATURES FOLLOWING INGESTION: Adjuvants in the formulations may have contributed to some of the features observed. Vomiting, abdominal pain, diarrhea, and, occasionally, gastrointestin

Endoscopic arytenoid abduction lateropexy for the treatment of neonatal bilateral vocal cord paralysis - Long-term results.

OBJECTIVES: Bilateral vocal cord paralysis often causes severe dyspnea requiring an early airway intervention in neonates. Endoscopic arytenoid abduction lateropexy (EAAL) with suture is a quick, reversible, minimally-invasive vocal cord lateralizing technique to enlarge the glottis. The arytenoid cartilage is directly lateralized to a normal abducted position. It can be performed even in early childhood with the recently-introduced pediatric endoscopic thread guide instrument. The long-term results and the stability of the lateralization were evaluated. METHODS: Three newborns had inspirator

Respiratory function deterioration is not time-linked with upper-limb onset in amyotrophic lateral sclerosis.

INTRODUCTION: In amyotrophic lateral sclerosis (ALS), symptoms apparently spread following regional rules, and depending on the site of onset. We examined if respiratory function deterioration appears earlier or is more severe in patients with upper-limb onset. MATERIAL & METHODS: We compared the results of various pulmonary function tests (PFT) obtained at diagnosis depending on the site of onset in 49 ALS patients. In a longitudinal study, we compared the deterioration of forced vital capacity (FVC) in relation to the site of onset, and analyzed the time elapsed to reach values below 80% of

Central vocal cord paralysis and paresis presenting as laryngeal stridor in children.

Vocal cord paralysis or paresis as the initial presenting symptom for intracranial tumors in children are rare. Recently, two pediatric patients who were later diagnosed as having intracranial tumors presented with the symptom of voice changes and stridor. Telescopic examination revealed bilateral vocal cord paresis and paralysis as demonstrated by video recordings. The majority of pediatric brain tumors present with both generalized and localized complaints; however, by discussing these two rare cases, we hope to underscore the importance of a thorough workup of the paralyzed or paretic vocal

Muscular, bronchomotor and cardiovascular reflexes elicited by mechanical stimulation of the respiratory tract.

1. The effects of mechanical stimulation in the nose, epipharynx, laryngopharynx and tracheobronchial tree, and of chemical irritation of the nasal mucosa, were studied on various somatic and autonomic functions in cats.2. Action potentials were recorded from the diaphragm and rectus abdominis muscles of spontaneously breathing cats, and from the phrenic and lumbar nerves of paralysed, artificially ventilated cats. Expulsive processes such as sneezing and coughing evoked from the nasal, laryngopharyngeal and tracheobronchial mucosae were characterized by strong diaphragmatic and abdominal expi

Antiviral activity of amenamevir alone and in combination with ganciclovir against equine herpesvirus 1.

Equine herpesvirus 1 (EHV-1) causes respiratory disease, abortion, and equine herpesvirus myeloencephalopathy (EHM), a neurological disorder characterized by ataxia and hindlimb paralysis. Although EHM remains a major concern in the equine industry, effective antiviral treatment options are limited. This study evaluated the antiviral activity of amenamevir, a helicase-primase complex inhibitor, against multiple EHV-1 strains using real-time cell analysis (RTCA) and quantitative PCR (qPCR), and compared its potency with those of ganciclovir, acyclovir, and penciclovir. Eight EHV-1 strains assoc

Fusion of molecular mimicry, epigenetic predisposition, and new onset GBS: a narrative review of current understanding and future directions.

Guillain-Barré syndrome (GBS) is a severe immune-driven polyneuropathy marked by the acute onset of flaccid paralysis, areflexia, and in severe cases, life-threatening autonomic or respiratory failure. Although the clinical presentation and diagnostic criteria are widely established, the precise mechanisms underlying GBS are complex and poorly understood. This review summarizes current literature on the interplay of post-infectious triggers, molecular mimicry, and host susceptibility as influenced by genetic and epigenetic variables. Infectious pathogens such as Campylobacter jejuni, cytomegal

In-Hospital Outcomes of Type A Aortic Dissection Repair in Patients With Obstructive Sleep Apnea: A Population Study of National Inpatient Sample From 2015 to 2020.

BackgroundObstructive sleep apnea (OSA) is a recognized risk factor for aortic dissection development due to its association with arterial hypertension, negative intrathoracic pressures during respiratory efforts, and further development of aortic false lumen. However, the impact of OSA on the outcomes of type A aortic dissection (TAAD) repair has not been extensively investigated. Therefore, this study aimed to perform a population-based analysis of the impact of OSA on in-hospital outcomes following TAAD repair.MethodsPatients who underwent TAAD repair were identified in National Inpatient S
