Endoprosthetic Reconstruction for Proximal Humerus Tumors.

PURPOSE OF THE REVIEW: Anatomic and reverse endoprosthetic reconstruction are two common surgical options used after tumor resection of the proximal humerus. The purpose of this article is to provide an overview of the functional outcomes and complications of modern anatomic and reverse endoprostheses. RECENT FINDINGS: The anatomic endoprosthesis has traditionally been a successful reconstructive technique as it provided a stable platform upon which the hand and elbow could function. However, the reverse endoprosthesis has gradually replaced the anatomic endoprosthesis given that its semi-con

Posterior calcaneal osteotomy with wedge: cadaver testing of a new procedure for insufficiency of the posterior tibial tendon.

Insufficiency of the posterior tibial tendon is challenging to treat. When the deformity is flexible, treatment options have included tendon transfer, often combined with a medial slide calcaneal osteotomy and/or a lengthening of the lateral column. Posterior calcaneal osteotomy has been shown to give correction, although not full correction. Lengthening of the lateral column also has been shown to give correction and has been used in the more severe flexible deformities, but it involves either fusion of the calcaneocuboid joint or risk of arthritis at this joint. An osteotomy combining the ca…

Selection criteria for surgical correction of equinovarus foot in adults with brain damage: A systematic scoping review.

OBJECTIVE: To report on preoperative outcomes that guide the choice of surgical techniques to correct equinovarus foot in adults with brain … studies, with 62% of studies investigating non-retracted spastic equinovarus foot. Before anterior tendon transfer with the tibialis anterior or another muscle, passive ankle dorsiflexion was reported in only 20% and 46% of studies, respectively, and dynamic tibialis anterior activation during gait in 46% and 56%. Although voluntary recruitment of the tibialis a …

Biceps-to-triceps transfer for active shoulder abduction deficit in brachial plexus birth palsy: a small case series.

Biceps-to-triceps transfer was performed in three children with concurrent deficits in shoulder abduction and elbow extension attributable to cocontraction following natural recovery from brachial plexus birth palsy. Substantial improvements in active shoulder abduction were observed without impairing elbow flexion.

Palmaris longus interposition in revision surgery for recurrent and persistent carpal tunnel syndrome: a case series.

Carpal tunnel syndrome (CTS) is the most frequently operated neurological disorder of the hand. Incidence of patients remaining symptomatic has been reported up to 30% after primary release. Revision surgery remains challenging although multiple surgical options have been described. In this case series a simple novel technique, the palmaris longus interposition, is described for the treatment of recurrent and persistent CTS. Patients who underwent PLI between October 2013 and 2018 and without underlying neurological or hand disorders severely affecting the operated hand were eligible for inclu

Anatomic Repair of Plantar Plate With Flexor Tendon Sheath Reinforcement: Case Series.

UNLABELLED: Plantar plate pathology has gained considerable attention in recent time, and with this increased awareness multiple treatments have been proposed. There have been no comparison studies between these treatments. The authors feel a direct approach for anatomic repair allows for plantar plate repair and tightening to desired tension, without any plantar prominences or potentially irritating hardware. In addition if the plantar plate is found to be attenuated or there is a lack of residual tissue, the described imbrication utilizing the flexor digitorum longus sheath, which is not pos

A New Rerouting Technique for the Extensor Pollicis Longus in Palliative Treatment for Wrist and Finger Extension Paralysis Resulting From Radial Nerve and C5C6C7 Root Injury.

Wrist and finger extension paralysis is a consequence of an injury to the radial nerve or the C5C6C7 roots. Despite these 2 different levels of lesions, palliative treatment for this type of paralysis depends on the same tendon transfers. A large majority of the patients are able to compensate for a deficiency of the extension of the wrist and fingers. However, a deficiency in the opening of the first web space, which could be responsible for transfers to the abductor pollicis longus, the extensor pollicis brevis, and the extensor pollicis longus (EPL), frequently exists. The aim of this work …

Using a double-layered palmaris longus tendon for suspension of facial paralysis.

INTRODUCTION: Facial palsy is a debilitating condition entailing both cosmetic and functional limitations. Static suspension procedures can be performed when more advanced dynamic techniques are not indicated. Since 2006, we have used a double-layered palmaris longus tendon graft through an ovular skin excision in the nasolabial fold for access. The aim of this paper is to present our surgical technique and case series. METHODS: This study was a retrospective case series in a university hospital setting. All patients who had this procedure performed were included before August 2013. Data from

[Hemiplegia and cerebral palsy - multidisciplinary treatment of the spastic upper extremity].

Spastic hemiplegia and cerebral palsy often lead to typical deformities of the upper extremity. Muscle- and joint-contractures may be painful and aesthetically unappealing and may interfere with function and hygiene. Within the first weeks after the cerebral incidence the vital threat is dominating and the exact amount of neurologic impairment is not assessable. During this period, conservative treatment should counteract the development of contractures. After spontaneous neurologic recovery, surgical options should be taken into account. When choosing surgical procedures, factors as volitiona

Combined medial patellofemoral ligament and medial patellotibial ligament reconstruction in skeletally immature patients.

Patellar instability is a common knee disorder encountered in young athletes. Patients with normal osseous anatomy and mechanical alignment of the lower extremity are candidates for soft-tissue reconstructive procedures. In skeletally immature patients, surgical techniques that address patellar instability must avoid disruption of open physes and therefore must rely on soft-tissue techniques. Biomechanical research demonstrates that the medial patellofemoral ligament is the primary soft-tissue restraint to lateral subluxation of the patella, and the medial patellotibial ligament is an importan

[Anterior cruciate ligament reconstruction - graft options and fixation techniques].

With the introduction of arthroscopy most of the surgeon have changed their technique in ACL-reconstruction from open to arthroscopically performed techniques. Several new techniques have been developed in the past, including new fixation devices and different grafts. The cruciate ligament reconstruction comprises a composition of both, the graft and the fixation to the bone. Well accepted autografts are the patellar midthird, the semitendinosus/gracilis or the quadriceps tendon. In special cases allografts are alternatives. The fixation can cause failure due to overstrain or creep during the

[Differential transplant selection in cruciate ligament surgery].

Over the past century numerous graft materials have been used for the reconstruction of the cruciate ligament of the knee. Among the autologous tissues that are currently recommended as graft materials, the central bone patellar tendon bone graft, a quadrupled hamstring graft and the central quadriceps tendon graft have the greatest clinical significance. With some limitations, allograft materials can also be used. Each of the three mentioned grafts has specific features regarding morphological and structural properties, graft fixation and graft incorporation. Clinical studies have failed to i

Transplantation of teres major muscle for infraspinatus muscle in irreparable rotator cuff tears.

The authors suggest transfer of teres major muscle for a detached and atrophic infraspinatus muscle in irreparable rotator cuff tears. Original studies were done on cadavers. In the period between November 1993 and June 1994, we used this technique on 6 patients. Those patients were evaluated with the Constant-Murley test and improved from an average of 40 points (minimum 27 to maximum 54) before surgery to an average of 62 points (minimum 47 to maximum 78) after surgery. Active external rotation recovered 35 degrees in abduction and 24 degrees in adduction. The teres major transferred on grea

[Severe and chronic lateral sprains of the wrist with axial deviation, and their treatment].

Firmly established lateral carpal instability may be practically corrected by a ligamentoplasty of the distal scaphoidal ligamentous complex. It may be entirely and definitively cured by a double ligamentoplasty of the distal scaphoidal complex and of the scapholunate ligament. Four cases with a postoperative follow-up of more than 2 years are presented.
