A 2.5-cm single-port video-assisted thoracoscopic surgery for stage III tuberculous empyema: a case report.

BACKGROUND: Stage III tuberculous empyema is a common disease of tuberculosis. Traditionally, it has been treated by thoracotomy or video-assisted thoracoscopic surgery with two to four incisions. But conventional surgery has large trauma, large bleeding volume and long recovery time. To our knowledge it is the first report of surgery for stage III tuberculous empyema with a mini single-port approach. CASE PRESENTATION: A 23-year-old woman admitted to our hospital with complaints of intermittent chest pain for half a year. We got the diagnosis of stage III tuberculous empyema after medical tr

Invasive thymoma presenting as classic superior vena cava syndrome: a case of venous spread metastasis.

The approach to an intrinsic cause of superior vena cava syndrome (SVCS) is usually difficult but rewarding. We report a case of a middle-aged man who presented with progressive oedema of the upper half of the body, dyspnoea, cough and weight loss for a 1-year duration. He was a non-smoker without prior hospitalisation. Chest radiography showed right-sided pleural effusion with an apparent normal superior mediastinum. Contrast-enhanced CT of the chest revealed a right atrial mass extending and completely obliterating to superior vena cava. The differentials were tuberculosis, invasive fungal g

Idiopathic myelofibrosis with disseminated hepatosplenic, mesenteric, renal and pulmonary extramedullary haematopoeisis, portal hypertension and tuberculosis: initial presentation and 2 years follow-up.

A 35-year-old man with a 12-year history of idiopathic myelofibrosis (IMF) presented in 2014 with fatigue and abdominal distension. CT scan revealed massive hepatosplenomegaly with focal splenic lesions, soft tissue around renal pelvis, mesenteric masses compressing bowel loops and perilymphatic nodules in lungs. There was portal hypertension, ascites, pleural effusion, bilateral psoas abscesses and necrotic retroperitoneal lymphadenopathy. MRI additionally revealed hypointense periportal infiltrative lesions in liver, not seen on CT scan. None of these lesions showed diffusion restriction. Bi

A rare case of fever of unknown origin: subcutaneous panniculitis-like T-cell lymphoma (SPTCL).

A 26-year-old man presented with high-grade fever, chills, productive cough and episodic abdominal pain of 6 months duration. Physical examination revealed that the patient was febrile and had multiple, ill-defined, tender, indurated, erythematous nodules and plaques over the trunk and thighs. Systemic examination and investigations revealed bilateral exudative pleural effusion with an increased adenosine deaminase (ADA) level. Pulmonary tuberculosis was suspected and the patient was started on a standard four-drug antitubercular regimen. Since his fever persisted, biopsy of the plaque over th

Human natural killer cells expressing the memory-associated marker CD45RO from tuberculous pleurisy respond more strongly and rapidly than CD45RO- natural killer cells following stimulation with interleukin-12.

Natural killer (NK) cells are known as innate immune lymphocytes that respond rapidly when challenged by pathogens but little is known about adaptive immune features including memory related to NK cells from human beings. In the present study, we demonstrate for the first time that human NK cells expressing the memory-associated marker CD45RO were persistent in pleural fluid cells (PFCs) from tuberculous patients. CD45RO(+) NK cells produced significantly more interferon-γ and were more cytotoxic compared with CD45RO(-) NK cells from PFCs when stimulated with interleukin-12 (IL-12). Consistent

Health risks of gold miners: a synoptic review.

Health problems of gold miners who worked underground include decreased life expectancy; increased frequency of cancer of the trachea, bronchus, lung, stomach, and liver; increased frequency of pulmonary tuberculosis (PTB), silicosis, and pleural diseases; increased frequency of insect-borne diseases, such as malaria and dengue fever; noise-induced hearing loss; increased prevalence of certain bacterial and viral diseases; and diseases of the blood, skin, and musculoskeletal system. These problems are briefly documented in gold miners from Australia, North America, South America, and Africa. I

Management of bronchopleural fistulas.

During the ten years 1972 to 1981, 16 patients with persistent bronchopleural fistulas were treated. Eleven patients had postoperative fistulas, and five had fistulas which were spontaneous in origin. The spontaneous fistulas were due to tuberculosis (three) and lung abscess (two). The postoperative bronchopleural fistulas occurred after pulmonary resections for cancer (six), aspergillosis (three) and bronchiectasis (two). All of the postoperative fistulas developed in patients in whom the nonresected lung failed to fill the pleural cavity. The pneumothorax space then became infected, and brea

[Hemothorax as a complication of thoracotomy].

In 1969 to 1989 in the Research Institute of Tuberculosis and Respiratory Diseases in Prague a total of 3583 thoracotomies were performed. Of these during the postoperative period 29 patients were re-operated on account of continuing haemorrhage into the pleural cavity. This is approximately 0.8% of all thoracotomies. Sixteen patients had to be re-operated within 12 hours after operation, 12 patients were re-operated later than 12 hours after operation but within 24 hours, one patient was re-operated later than 24 hours after operation. The cause of haemothorax was in the first place capillary

[Adenosine deaminase. A pluridisciplinary enzyme].

Adenosine deaminase is an enzyme that actively participates in the metabolism of the adenine nucleotides. It catalyzes the irreversible hydrolytic deamination of deoxyadenosine and adenosine with the production of deoxyinosine and inosine respectively and of ammonia. This enzyme thus plays an important role in lympho-monocyte maturation and activation. The increase in its activity in different biological fluids (pleural, pericardial, peritoneal, intra-articular and cerebrospinal fluids) has been used as a rapid diagnostic test in tuberculosis infection. In human immunodeficiency virus infectio

Prevalence of pulmonary involvement in rheumatoid arthritis and its relationship to some characteristics of the patients. A radiological and clinical study.

The chest radiographs of 309 patients with rheumatoid arthritis (RA) were compared with those of 309 controls. In RA, lung nodules were present in 0.3% and pleural effusion in 0.6% only. Diffuse reticulonodular fibrosis occurred in 4.5% of the RA patients and 0.3% of the controls (p less than 0.001) and was related to subcutaneous nodules, antinuclear antibodies and high Waaler-Rose titres. Diffuse reticular fibrosis occurred in 6.8% and 5.2% respectively and was related only to age greater than 60 and cigarette smoking. Healed tuberculosis occurred in 17.2% of the RA males and in 6.1% of the

Giant Intrapulmonary Solitary Fibrous Tumour With a Possible Liver Metastasis: A Rare Case.

Intrapulmonary solitary fibrous tumours (SFTs) are exceptionally rare mesenchymal neoplasms, particularly when presenting as giant thoracic masses. We report a case of a 57-year-old woman with progressive chest pain and dyspnoea who initially presented with pleural effusion and hypoglycaemia and received empirical anti-tuberculosis therapy without improvement. Contrast-enhanced CT revealed a left lower lobe mass enlarging to 11 × 16.5 × 15.2 cm on repeat imaging, with rightward cardiac displacement, pericardial effusion and a hepatic nodule suspicious for metastasis to the liver. Diagnostic value of nanopore-based targeted sequencing technology for subclinical tuberculosis.

BACKGROUND: Subclinical Tuberculosis (TB), with low bacterial loads and non-specific symptoms, is difficult to diagnose and promotes transmission. This study aimed to evaluate the diagnostic performance of nanopore-based targeted sequencing (NTS) for subclinical TB in comparison with Mycobacteria Growth Indicator Tube (MGIT) culture and GeneXpert MTB/RIF (Xpert MTB/RIF) assays. METHODS: This retrospective research included 103 subclinical TB suspects. We tested sputum, bronchoalveolar lavage fluid, or pleural effusion from each patient using NTS, MGIT culture, and Xpert. Aspergillus niger pneumonia as superinfection in a patient with pulmonary tuberculosis.

We report a patient with severe cavitary pulmonary tuberculosis and Aspergillus niger superinfection, whose only comorbidity was untreated diabetes mellitus. A. niger pneumonia was proven by PCR, sequencing and culture of pleural and respiratory secretions. The patient was successfully treated with a four-drug antituberculous regimen, liposomal amphotericin B (up to 5 mg/kg/d) and pleuro-pneumonectomy. Histology of the resected lung revealed destroyed lung tissue with inflammatory cells and fungal conidia. There were large deposits of polarising material, which was found to be calcium oxalate.

Carcinomatous pleuritis and pericarditis accompanied by pulmonary tuberculosis.

Although both lung cancer and pulmonary tuberculosis (TB) commonly occur in clinical practice, little attention has been paid to their coexistence. A 62-year-old female was admitted with acute dyspnoea secondary to cardiac tamponade. During her admission, a mass lesion harbouring air bronchograms in the right upper lobe rapidly increased in size. Surgical lung, pericardial, and pleural specimens yielded TB from a nodule in the right upper lobe and lung adenocarcinoma from the pericardium and pleura. Anti-tuberculous therapy was administered and gefitinib was subsequently started after the posi
