Interpersonal style and anxiety.

Agoraphobic anxiety has been linked to traditional feminine sex role attitudes and related dyadic interactions. This research correlated measures of panic experience and agoraphobic anxiety with interpersonal style in a college student population. Significant positive correlations were observed between anxiety scores and the following interpersonal styles: submissive/deferent, self-effacing/obedient, mistrusting/cold, submissive, inhibited, and unassured. Results lend support to clinical reports that some individuals may develop agoraphobic symptoms as a result of the interaction of interperso

An overview of anxiety disorders.

Anxiety disorders are more prevalent than any other mental health problem in the United States. Afflicted persons are often untreated or are treated only by a primary care physician without psychiatric consultation. However, based on current knowledge of the biological and psychological factors involved in these disorders, psychiatry has defined effective treatment that integrates education, psychotherapy, and psychopharmacology. After reviewing current research on possible mechanisms of anxiety, the authors discuss therapeutic interventions and recommend ways to choose appropriate treatment m

Nocturnal panic: response to hyperventilation and carbon dioxide challenges.

To examine the role of ventilatory response in nocturnal panic, subjects experiencing nocturnal panic were compared with those who experienced daytime panic attacks only. In particular, measures of chronic hyperventilation (baseline pCO2) and CO2 hypersensitivity (response to ventilatory challenges) were assessed. Subjective and psychophysiological measures were obtained during baseline, forced hyperventilation, and carbon dioxide inhalation phases of a standardized laboratory-based assessment. The groups did not differ with respect to subjective or physiological measures or to the frequency w

Avoidance behaviour and major depression in panic disorder: a report from the Cross-National Collaborative Panic Study.

Avoidance behaviour and secondary major depression are both frequent in clinical samples of patients with panic disorder. Their status is unclear: indicators of severity of panic disorder or indicators of separate psychiatric disorders. Among the data of the Cross-National Collaborative Panic Study (n = 1,168) we found that especially avoidance behaviour defines more severe states of panic disorder (earlier age at onset, higher frequency of panic attacks and higher level of psychopathology); co-occurrence of major depression is less clearly associated with more severe panic disorder. The resul

Differential reactivity to lactate infusions: the relative role of biological, psychological, and conditioning variables.

Nine patients with panic disorder experienced a lactate-induced panic attack, whereas nine controls did not. Higher preinfusion anxiety levels and heart rates were associated with panic disorder, and high baseline anxiety ratings were associated with atypical, severe lactate-induced panic attacks. Nevertheless, it was difficult to reconcile patients' and controls' reactivity to lactate as entirely secondary to baseline differences. Subjects differed qualitatively in the types of specific symptoms experienced and quantitatively in their anxiety and heart rate responses. In most cases, panic beg

The epidemiology of panic symptomatology and agoraphobic avoidance.

In a random community survey of 1,498 urban adults age 18 to 64 years who were interviewed using the Diagnostic Interview Schedule (DIS), th … d was more common in women (14.6% +/- 1.3%) than in men (6.8% +/- 1.3%). The lifetime prevalence of agoraphobia was 3.8% +/- 0.5%. The occurrence of panic attacks and phobic disorders were frequently related, and in agoraphobic subjects those with more severe agoraphobic avoidance reported more panic symptoms. Indeed, among agoraphobic subjects with at least …

Do panic disorder patients indiscriminately endorse somatic complaints?

Ehlers et al. (1986b) and Margraf et al. (1986) suggested that panic disorder patients indiscriminately endorse somatic complaints and that their responses to lactate infusion are nonspecific. Their Symptom Questionnaire was composed of anxiety/panic/lactate infusion relevant symptoms, while the Somatic Control Scale was composed of "irrelevant" symptoms. In an attempt to address and in part replicate the above findings among panic disorder patients, we adopted the SCS of Margraf et al. (1986) for use with our Acute Panic Inventory, an instrument similar to their Symptom Questionnaire. Contrar

Hyperventilation: is it a cause of panic attacks?

During transcutaneous PCO2 (PtcCO2) monitoring of 15 freely ambulant patients suffering from panic attacks, all the patients experienced a typical attack. Seven were identified as hyperventilators, as PtcCO2 fell to abnormally low levels during the attack. This group could not be distinguished on the basis of either their usual symptoms of panic or the hyperventilation provocation test. There was no apparent association between absolute levels of PCO2 and the nature of symptoms. The anxiety ratings of hyperventilators were lower than those of non-hyperventilators. These data do not support the

Tonic arousal and activity: relationships to personality and personality disorder traits in panic patients.

Personality theorists have long predicted a relationship between personality traits and autonomic activation. In this study, 48 patients with panic disorder underwent personality assessment by questionnaire (Eysenck Personality Inventory: 48 patients) and by interview (Personality Disorders Examination: 35 patients). Ambulatory heart rate and activity were measured by the Vitalog method and were used as measures of activation and autonomic arousal. There was a significant positive correlation between histrionic traits and activity level and a significant negative correlation between sociabilit

Panic disorder, the ventilatory response to carbon dioxide and respiratory variables.

Increased sensitivity of the central carbon dioxide chemoreceptors to carbon dioxide has been hypothesized as a biologic determinant of panic attacks in panic disorder and on voluntary inhalation of carbon dioxide mixtures. The ventilatory response to carbon dioxide, a measure of sensitivity, was assessed in 12 patients with panic disorder and 12 matched normal controls. While mean sensitivity was similar for both groups, for the panic disorder patients, mean sensitivity due to the tidal volume component was significantly less than for the normal controls, and mean sensitivity due to the frequ

Anxiety sensitivity in agoraphobics.

This study further validated the "Reiss-Epstein-Gursky Anxiety Sensitivity Index" (ASI) as a measure of the fear of anxiety. Agoraphobics scored high on the ASI before, but not after, behavioral treatment. Residual anxiety sensitivity, however, did not predict resurgence of agoraphobic avoidance at six months follow-up. Indeed, anxiety sensitivity continued to decline during the follow-up period. Multiple regression analyses indicated that the ASI predicted the number of fears in agoraphobics beyond that predicted by the level of general anxiety. This finding supports the hypothesis that high

In marriage, whatever happens to one partner inevitably has its effect upon the other. Sometimes, when one overbalances the other partner is brought down also. At other times it is as though one is able to stay upright at the expense of the other remaining down.

Imipramine in the treatment of panic disorder and its complications.

The recognition of the efficacy of imipramine for treating panic disorder and its complications has had major implications for clinical practice, diagnostic classification, theoretic understanding, and research strategies in the anxiety disorders. It has also resulted in benefit for a great number of affected individuals and promises to yield even greater relief of suffering as the findings become more widely disseminated.

A test of the repression hypothesis in agoraphobics.

In a recent study Turner et al. (1983) employed Bell & Byrne's (1978) Repression-Sensitization (R-S) scale to test the hypothesis that agoraphobics utilize repression as a method of avoiding anxiety-arousing forms of cognition. However, no support was found for this view. Rather, the R-S scores indicated general sensitization and attention to negative affect. In this paper it is argued that, on both theoretical and psychometric grounds, the R-S scale cannot be considered a suitable measure of the repression-sensitization process. A study was carried out in which a number of other defensivity m

Psychological profile of the spouse of the female agoraphobic patient. Personality and symptoms.

It has been contended that psychological characteristics of the partner of the agoraphobic patient are important factors in the development and maintenance of the patient's symptoms. To examine this hypothesis, male partners of female agoraphobics were compared with those of non-phobic psychiatric patients and of normal controls on a total of 48 measures referring to several symptom complexes and traits, including defensiveness. The partners of agoraphobics as a group were not found to be more defensive or psychologically more disturbed than their control counterparts. Additional within-couple

Suitability for insight-oriented psychotherapy as a prognostic factor in treatment of phobic women.

Suitability for insight-oriented psychotherapy comprises many factors that may have a positive relation to outcome in individual psychotherapy. We wanted to test the hypothesis that patients suitable for insight-oriented psychotherapy (I patients) improve more than not-suitable patients (NI patients) in non-dynamic therapies. Pairs of phobic female NI and I patients were formed with age, type of phobia, global rating of severity, therapist and type of therapy as matching variables. All women received a simple basal therapy in the form of standardized information, self-exposure instructions and
