Appropriate blood sampling sites for measuring Tg concentrations for forensic diagnosis.

Previous studies have reported that thyroglobulin (Tg) concentrations in heart blood are high in cases of asphyxia caused by neck compression such as hanging, strangulation, and throttling and in those with fatal traumatic brain injuries. However, even in cases without these findings presumed to increase the Tg concentration in the previous studies, we previously reported that in some cases the Tg concentration in right heart blood (RHB) and left heart blood (LHB) exceeded the standard value for diagnosis (200 ng/mL) defined in previous studies and the Tg concentration in RHB was significantly

Cerebellar vermian atrophy after neonatal hypoxic-ischemic encephalopathy.

BACKGROUND AND PURPOSE: Although pathologic evidence of cerebellar injury due to birth asphyxia is well described, neuroimaging evidence is sparse. The primary purpose of this retrospective study was to evaluate the early and late imaging findings in the cerebellum of patients who had neonatal hypoxic-ischemic encephalopathy with thalamic edema shown by neonatal CT. The secondary aims were to validate thalamic edema shown by neonatal CT as a marker of thalamic injury and to assess the late cerebral cortical abnormalities associated with neonatal thalamic edema. METHODS: Fifty-five neonates wi

Comprehensive Multimodal Diagnostics and Rehabilitation in Persistent Post-Concussion Syndrome Following Motor Vehicle Collision.

Motor vehicle collisions (MVCs) are among the most common mechanisms of mild traumatic brain injury (mTBI), and a meaningful proportion of affected individuals develop persistent post-concussion syndrome (PPCS) characterized by prolonged multidomain dysfunction. Conventional neuroimaging frequently fails to capture the underlying pathophysiology, leaving clinicians without objective diagnostic evidence. We describe a 57-year-old woman who sustained mTBI in a rear-end MVC in March 2023. Head computed tomography (CT) performed at the emergency department and a subsequent brain magnetic resonance

Fast ripple-delta coupling as an early biomarker for post-traumatic epileptogenesis in repetitive brain injury.

OBJECTIVE: Traumatic brain injury (TBI) can induce posttraumatic epilepsy (PTE), but early biomarkers for epileptogenesis are lacking. We aimed to investigate electrographic biomarkers before and during posttraumatic seizure development. METHODS: We used a repetitive diffuse TBI model in mice with continuous video-EEG monitoring up to 4½ months postinjury. RESULTS: Twenty-five percent of mice developed posttraumatic seizures (PTS) with highly variable latency (5-126 days postinjury). Most significantly, we identified fast ripple-delta DOWN state coupling as an early biomarker that was detect

The Effect of a Single Session of Transcranial Direct Current Stimulation on Attention in Pediatric Acquired Brain Injury: Characterizing Interindividual Structural and Functional Network Response Variability.

BACKGROUND: Approximately one in four children who sustain an acquired brain injury (ABI) have attention difficulties impacting education, employment, and community participation. These difficulties arise from dysfunction in attention-related brain networks, incentivizing the use of transcranial direct current stimulation (tDCS). We investigated whether a single tDCS session improved attention following childhood ABI and whether baseline structural connectivity (sc), functional connectivity (fc), attention, and/or simulated electric fields (E-fields) explained variability in response. Arterio-jugular differences of oxygen (AVDO2) for bedside assessment of CO2-reactivity and autoregulation in the acute phase of severe head injury.

Autoregulation and CO2-reactivity can be impaired independently of each other in many brain insults, the so-called 'dissociated vasoparalysis'. The theoretical combination of preserved CO2-reactivity and impaired or abolished autoregulation can have many clinical implications in the daily management of brain injured patients. To optimize their treatment, a bedside assessment of autoregulation and CO2-reactivity is desirable. When cerebral metabolic rate of oxygen is constant, changes in arterio-jugular differences of oxygen (AVDO2) reflect changes in CBF. In these situations relative changes i

Continuum of Irreversible and Potentially Reversible Traumatic Brain Injury Sequelae: Axonal Injury and Hydrocephalus.

Chronic traumatic encephalopathy (CTE) is a neurodegenerative tauopathy linked to repetitive head trauma and definitively diagnosed postmortem. Its proposed clinical correlate in living patients is traumatic encephalopathy syndrome (TES). Within this spectrum, diffuse axonal injury (DAI) and post-traumatic hydrocephalus (PTH) represent interacting mechanisms contributing to long-term neurocognitive decline after traumatic brain injury (TBI). While DAI reflects largely irreversible axonal damage, PTH constitutes a potentially reversible disturbance of cerebrospinal fluid (CSF) dynamics. Their c

Inflammatory mechanisms underlying cortical injury in progressive multiple sclerosis.

Multiple sclerosis (MS) is a lifelong inflammatory demyelinating disease of the central nervous system (CNS). While there has been substantial progress in the development of therapeutic strategies for relapsing disease, the field has lagged behind in its understanding and management of progressive stages of the disease, including secondary progressive and primary progressive MS, respectively. It is now thought that distinct but temporally overlapping mechanisms underlie relapsing and progressive aspects of the disease. Relapsing disease is characterized by waves of peripheral immune cell activ

Integrated miRNA-proteomic profiling identifies chronic vesicle-trafficking and proteostasis disruptions after mild traumatic brain injury.

BACKGROUND: Mild traumatic brain injury (mTBI) often produces persistent deficits, yet the molecular mechanisms driving chronic pathology remain undefined. OBJECTIVE: We aimed to identify mechanistic drivers of long-term dysfunction after mTBI by integrating proteomics, transcriptomics, and behavioral outcomes. METHODS: Adult rats were subjected to a modified Marmarou weight-drop mTBI model (diffuse closed-head injury) or a sham procedure. Cortical tissue was analyzed at 21 days post-injury (chronic phase) by quantitative proteomics and small RNA sequencing, while neurological and motor func

Association of Glymphatic System Dysfunction with Cardiac Injury and Cognitive Impairment in Heart Failure: A Multimodal MRI Study.

RATIONALE AND OBJECTIVES: Glymphatic dysfunction, a critical pathway to cognitive impairment, remains underexplored in heart failure (HF). This study investigated glymphatic dysfunction in patients with HF and the relationship between cardiac injury and cognitive performance. METHODS: 46 HF patients and 39 age- and sex-matched healthy controls (HCs) underwent brain MR to assess glymphatic markers: choroid plexus (CP) volume, perivascular space (PVS) score, white matter fractional free water (FW), diffusivity along perivascular spaces (ALPS), blood-oxygen-level-dependent (BOLD) signals, and ce

A Pediatric Concussion Model in Mice: Closed Head Injury with Long-Term Disorders (CHILD).

Concussions and mild traumatic brain injuries (mTBI) during childhood represent a significant health endangerment, with many patients later in life exhibiting debilitating physiological, neurological, and psychosocial outcomes. The cellular and molecular pathophysiological mechanisms are relatively unknown, and this significant gap effectively precludes investigations into specific therapeutic strategies to mitigate chronic sequelae. No single animal concussion model recapitulates all the features reported in human subjects, and current models are designed to answer specific research questions

A Multi-Method Approach to a Comprehensive Examination of the Psychiatric and Neurological Consequences of Intimate Partner Violence in Women: A Methodology Protocol.

The number of women in the United States that experience blows to the head during assaults by intimate partners is substantial. The number of head blows that result in a traumatic brain injury (TBI) is virtually unknown, but estimates far exceed numbers of TBI in parallel populations (e.g., blast exposure, accidents, sports) combined. Research on the impact of TBI on post-traumatic stress disorder (PTSD) in survivors of intimate partner violence (IPV) is sparse. This methodology paper describes the comprehensive, multi-method approach used by a multi-disciplinary team of investigators from sev

Evaluation of a filament perforation model for mouse subarachnoid hemorrhage using 7.0 Tesla MRI.

The filament perforation model (FPM) in mice is becoming increasingly popular to elucidate the molecular pathogenesis of neuronal injury after subarachnoid hemorrhage (SAH). We evaluated brain MRI in a mouse FPM. A total of 28 male C57Bl/6J mice were used. Seventeen animals underwent SAH induction by FPM. In two animals, transient middle cerebral artery occlusion (MCAo) was induced. Nine mice served as controls. T1-weighted images (T1WI), T2-weighted images (T2WI), T2(∗)-weighted images (T2*WI) and apparent diffusion coefficient maps were acquired at day 0 and at various time points following

Marked increase of beta-amyloid(1-42) and amyloid precursor protein in ventricular cerebrospinal fluid after severe traumatic brain injury.

Severe traumatic brain injury (TBI) may result in widespread damage to axons, termed diffuse axonal injury. Alzheimer's disease (AD) is characterised by synaptic and axonal degeneration together with senile plaques (SP). SP are mainly composed of aggregated beta-amyloid (Abeta), which are peptides derived from the amyloid precursor protein (APP). Apart from TBI in itself being considered a risk factor for AD, severe head injury seems to initiate a cascade of molecular events that are also associated with AD. We have therefore analysed the 42 amino acid forms of Abeta (Abeta1-42) and two solubl

Association of Fractional Anisotropy in White Matter Bundles with Plasma Biomarkers of Neurodegeneration and Glial Reactivity in Individuals with Cognitive Complaints without dementia.

Age-related cognitive decline is preceded by subtle neurochemical and structural brain alterations that remain insufficiently characterized, particularly in individuals with cognitive complaints but without dementia. Circulating plasma biomarkers reflecting amyloid pathology, tau phosphorylation, astroglial reactivity, and axonal injury have emerged as accessible indicators of early neurodegenerative processes; however, their relationship with early white matter microstructural changes remains unclear. In this study, we investigated associations between plasma Aβ42/Aβ40, phosphorylated tau 217

Association of cortical microstructure with amyloid-β and tau: impact on cognitive decline, neurodegeneration, and clinical progression in older adults.

Noninvasive biomarkers of early neuronal injury may help identify cognitively normal individuals at risk of developing Alzheimer's disease (AD). A recent diffusion-weighted imaging (DWI) method allows assessing cortical microstructure via cortical mean diffusivity (cMD), suggested to be more sensitive than macrostructural neurodegeneration. Here, we aimed to investigate the association of cMD with amyloid-β and tau pathology in older adults, and whether cMD predicts longitudinal cognitive decline, neurodegeneration and clinical progression. The study sample comprised n = 196 cognitively normal

Degeneration of core neural tracts for emotional regulation in a patient with traumatic brain injury: A case report.

RATIONALE: Several brain structures, including the orbital prefrontal cortex, ventrolateral prefrontal cortex, dorsolateral prefrontal cortex, amygdala, and anterior cingulate cortex, are considered key structures in the neural circuitry underlying emotion regulation. We report on a patient showing behavior changes and degeneration of core neural tracts for emotional regulation following traumatic brain injury (TBI). PATIENT CONCERNS: A 51-year-old male patient suffered an in-car accident. The patient lost consciousness for approximately 30 days, and his Glasgow Coma Scale score was 3. He und
