Endarterectomy for narrowing of the external carotid artery in a patient who underwent extracranial-intracranial bypass for occlusion of the internal carotid artery--case report.

A 56-year-old man had undergone extracranial-intracranial (EC-IC) bypass surgery for occlusion of the right internal carotid artery. Six years later, he complained of transient episodes of numbness in the left arm, occurring periodically over the previous 3 months. Neuroimaging showed the right external carotid artery was severely narrowed and cerebral blood flow (CBF) was diffusely low. Endarterectomy of the right external carotid artery was performed. Postoperative CBF was markedly improved, suggesting that EC-IC bypass became effective again by endarterectomy of a parent artery.

Use of the excimer laser in high-flow bypass surgery of the brain.

The technique of laser-assisted anastomosis for high-flow bypass surgery using the excimer laser is described in 15 rabbits and in one patient. The left common carotid artery of the rabbits was excised and, with two anastomoses, connected to the right common carotid artery. An end-to-side anastomosis technique was used that obviated the temporary occlusion of the recipient artery. The end of the donor artery was connected for its full circumference with the exterior of the recipient artery and, with the aid of an excimer laser catheter (introduced via an artificial side branch of the donor art

Bifrontal interhemispheric approach for carotid-ophthalmic aneurysms.

The authors report their experience with the surgical treatment of carotid-ophthalmic aneurysms in 29 cases, and describe their surgical technique. The technique can be summarized as follows. When dissecting the aneurysm, temporary vascular occlusion of the common carotid artery and external carotid artery is done in the neck under the administration of cerebral protective substances. Through a bifrontal craniotomy, wide dissection of the Sylvian fissures and the interhemispheric fissure is performed. When necessary, the anterior clinoid process and the roof of the optic canal are removed. Thi

[Changes in CBF pattern after bypass surgery studied by CT with Xe enhancement].

UNLABELLED: Using a new method for rCBF measurement by serial CT scanning with non-radioactive xenon enhancement, CBF was measured before and/or after microsurgical anastomosis in five cases of focal cerebral ischemia. MATERIALS AND METHODS: Studies were carried out on 2 cases of MCA occlusion, 2 of IC occlusion, and 1 of "Moyamoya" disease. CBF was measured both before and after surgery in 4 cases, and the remaining case was measured after anastomosis. Pre-operative CBF was measured 1.4 +/- 0.5 months after the onset and post-operative CBF was 2 +/- 1 months after surgery. While 50 to 70% no

Cerebral revascularisation in stroke prophylaxis.

Over 700 cerebral vascular operations have been undertaken on the Surgical Unit of St Bartholomew's Hospital, London, for stroke prophylaxis, the majority being carotid endarterectomies. One of the unique features of the latter procedure is the marked instability of postoperative blood pressure. The relation of this change to interference with carotid baroreceptor function was studied by per-operative carotid sinus nerve recording. It was possible to relate nerve activity to the postend-arterectomy changes in the mechanical properties of the vessel wall in the region of the carotid sinus. Post

Ischaemic intermittent claudication of the masticatory muscles: two case reports.

Intermittent claudication of the masticatory muscles is an unusual symptom sometimes described in association with temporal arteritis. We describe here two cases where this symptom was due to insufficient blood supply to the masticatory muscles caused by atherosclerotic changes of carotid vessels. In one of our cases surgical revascularization was followed by the disappearance of this symptom.

Pulsatility index of superficial temporal artery was associated with cerebral infarction after direct bypass surgery for moyamoya disease.

OBJECTIVE: Direct bypass surgery by superficial temporal artery (STA) - middle cerebral artery anastomosis is an established procedure for moyamoya disease (MMD). However, some patients may develop cerebral infarction (CI) due to the watershed shift phenomenon after the surgery. This study sought to investigate the correlation between the postoperative changes of STA flow as well as cerebral blood flow (CBF) and the incidence of CI after direct bypass surgery for MMD. METHODS: We conducted a retrospective study of 62 hemispheres in 50 subjects who underwent direct bypass surgery for MMD. All

A rare case of moyamoya disease in a 20-year-old Puerto Rican female U.S. soldier.

Moyamoya disease is a progressive, occlusive pathology involving the cerebral vasculature with particular involvement of the circle of Willis and its tributaries. The cause of moyamoya disease is unknown, but is believed to be hereditary. Females 20 to 39 years old with moyamoya represent 0.5% of all acute cerebral ischemia and infarcts with risk factors including smoking, estrogen-containing birth control use, coagulopathy, neoplasm, and congenital malformation. This case reports on a 20-year-old Puerto Rican female U.S. soldier with a 1-year history of migraine headaches with worsening right

Intentional staged angioplasty with superficial temporal artery-superior cerebellar artery "safety bypass" for symptomatic severe vertebral artery stenosis: illustrative case.

BACKGROUND: In the endovascular treatment of symptomatic severe vertebral artery (VA) stenosis without collateral blood flow, intraprocedural acute occlusion due to thrombus rupture or lesion dissection can result in catastrophic ischemic complications in the posterior circulation. In this report, the authors describe an intentional staged angioplasty approach in which bypass surgery was performed before percutaneous transluminal angioplasty (PTA) at the stenotic lesion. This "safety bypass" was expected to provide distal collateral flow for inadvertent occlusion of the basilar artery due to d

[Long-term prognosis and blood flow reactivity to acetazolamide in atherothrombotic brain ischemia].

We report the results of a prospective follow-up study which was designed to identify the sequential change of regional vasodilatory capacity and its relation to long-term prognosis in patients with atherothrombotic brain ischemia. We evaluated a total of 139 acetazolamide tests using 123I-IMP SPECT in 76 patients with occlusion or greater than 75% stenosis of the carotid or middle cerebral arteries. Reduced vasodilatory capacity was demonstrated in 34 patients (44.7%) at entry. Patients were divided into two groups on the basis of the initial acetazolamide test, i.e., impaired and non-impaire

Posterior Cerebral Artery Involvement in Paediatric Moyamoya: Angiographic Patterns and Stroke Burden.

UNLABELLED: <p>Introduction: The posterior circulation is frequently overlooked when managing patients with moyamoya angiopathy, particularly the non-Asian population. This study aimed to identify the presence of angiopathy in the posterior cerebral artery (PCA), its patterns, and the consequences thereof in a paediatric cohort. METHODS: Retrospective clinical data, MRI scans, and cerebral angiograms of all patients referred to a single centre for cerebral revascularization were analysed for PCA involvement. Angiographic patterns of PCA involvement were defined. Associated stroke burden was e…

External carotid artery stenting in symptomatic internal carotid artery occlusion.

We report the case of a patient with an anterior ischemic stroke due to tandem occlusion of the left M2 segment and ipsilateral internal carotid artery (ICA), with concomitant severe stenosis of the ipsilateral external carotid artery (ECA) and contralateral ICA, and moderate stenosis of the left vertebral artery (VA); as thrombectomy was not possible, stenting of the right ICA was performed. Two days after significant recovery, the patient showed neurological deterioration when in upright position, and brain magnetic resonance imaging confirmed decreased cerebral blood flow on the left hemisp

Stent placement for the treatment of a symptomatic intracranial arterial dissection in an adolescent.

Intracranial arterial dissection is an important cause of stroke in young patients. Treatment options include observation, antiplatelet or anticoagulation regimens, and endovascular stent placement. The authors describe the case of a 14-year-old boy who presented with a symptomatic, posttraumatic dissection extending from the intracranial internal carotid artery to the middle cerebral artery. Images obtained approximately 48 hours after this incident revealed a subacute right frontal lobe infarct, and a CT stroke study (CT angiography and CT perfusion) confirmed the vascular injury and associa

Ruptured pseudoaneurysm of the petrous internal carotid artery caused by chronic otitis media.

A 60-year-old man presented with a rare case of pseudoaneurysm in the petrous internal carotid artery (ICA) caused by chronic otitis media manifesting as right otorrhagia after the earpick procedure. Computed tomography showed the destroyed petrous bone; the ICA and the aneurysm, which were exposed to the right middle ear; and a massive hematoma in the right middle ear. Right carotid angiography revealed a pseudoaneurysm arising from the ICA coursing through the middle ear. Two weeks later, the right ICA was trapped after external carotid artery-middle cerebral artery bypass surgery using a ve

Bright-light amaurosis fugax: an unusual symptom of retinal hypoperfusion corrected by external carotid revascularization.

The eye often serves as an important monitor of carotid artery disease by manifesting visual disturbances before cerebral events. The most typical ocular symptom complex is amaurosis fugax characterized by loss of vision in one eye that occurs suddenly and clears within seconds to minutes. This symptom is a hallmark of carotid artery disease and is usually caused by an embolic event. Although monocular visual disturbance on a hemodynamic basis is a more unusual manifestation of carotid artery disease, significant stenosis in combination with a low-flow state can cause transient, unilateral tel

Four-Dimensional Flow Magnetic Resonance Imaging Analysis of Long-Term Blood Flow in High-Flow Extracranial-Intracranial Bypass Surgery With Internal Carotid Artery Ligation.

OBJECTIVE: High-flow bypass with a radial artery graft (RAG) treats complex internal carotid artery (ICA) aneurysms. We used four-dimensional flow magnetic resonance imaging (4D flow MRI) to quantify long-term cerebral hemodynamics, compare maintained and reduced bypass flow, evaluate differences between ruptured and unruptured aneurysms, and identify predictors of flow decline. METHODS: We retrospectively studied 21 patients (2010-2024) who underwent ICA ligation with RAG. Serial postoperative 4D flow MRI measured blood flow volume (BFV) in the bypass (BFVbypass), contralateral ICA (BFVcICA)
