We studied 50 consecutive cases of severe penetrating ocular trauma with posterior segment involvement for which vitreous surgery and scleral buckling were performed as secondary reparative procedures. Eyes with a better prognosis, such as those with anterior segment injuries alone and intraocular foreign bodies, were excluded. Of 50 eyes, 31 (62%) had a successful outcome. There was a significant difference in visual outcome between those eyes that had received blunt and sharp penetrating trauma. Traumatic cataracts occurring after ocular trauma were removed by phacoemulsification, cryoextraction or extracapsular cataract extraction, depending on the degree of lens absorption. In a survey of 58 aphakic eyes examined 5 to 10 years after surgery, 12% had episcleral vessels in the inner aspect of the cataract incision and almost half of these eyes showed evidence of mild intraocular hemorrhage. Analysis of these and 33 other eyes indicates that argon laser treatment is only temporarily effective in preventing intraocular hemorrhage. Limbal cryotherapy is effective in a one- to four-year follow-up. While these vessels are a common cause of transient intraocular hemorrhage in aphakic eyes, serious sequelae are rare and treatment is usually unnecessary.

We used the retinal cryoprobe in the surgical management (biopsy and excision) of orbital and ocular adnexal tumors in four patients. This investigation is based on 262 cases of retinal detachment treated with episcleral silastic sponge implants and cryopexy. Anatomical restoration of the retina after one operation or more was successful in 89 percent. Drainage of subretinal fluid was carried out in 52 percent of the cases. Supplementary light-coagulation 3 or 4 weeks after the operation was applied in 43 cases (17 percent) in order to seal off retinal holes on the buckle due to inadequate chorioretinal adhesion following cryopexy. Orbital desmoid fibromatosis (DF) is a rare, non-metastasizing, locally destructive fibroblastic neoplasm, posing diagnostic and therapeutic challenges.
