BACKGROUND: Migration plays a major role in the emergence and resurgence of lymphatic filariasis (LF) in many countries. Because of the high prevalence of Imported Bancroftian Filariasis (IBF) caused by nocturnally periodic Wuchereria bancrofti and the intensive movement of immigrant workers from endemic areas, Thailand has implemented two doses of 6 mg/kg diethylcarbamazine (DEC) with interval of 6 months to prevent IBF. In areas where immigrants are very mobile, the administration of DEC may be compromised. This study aimed to evaluate DEC administration and its barriers in such areas.

Filarial lymphedema is complicated by frequent episodes of dermatolymphangioadenitis (DLA). Severe systemic symptoms during attacks of DLA resemble those of septicemia. The question we asked was whether bacterial isolates can be found in the peripheral blood of patients during the episodes of DLA. The long-term effect of 4 strategies for control of bancroftian filariasis using mass diethylcarbamazine (DEC) chemotherapy was assessed and compared in 4 endemic communities in Tanzania over a period of 4 years. The strategies were the standard 12 d treatment (strategy I), semi-annual single dose treatment (strategy II), monthly low dose treatment (strategy III), and DEC-medicated salt treatment (strategy IV). Treatment was given only during the first year. All strategies resulted in considerable reductions in microfilaraemia, with maximum effects occurring 1-2 years after start of treatment.

Clinical and parasitological surveys were carried out concurrently during 1986 in Pondicherry. Levels of C-reactive protein (CRP) were evaluated by enzyme immunoassay in patients infected with the filarial parasite Wuchereria bancrofti.
