HIV/AIDS: occupational risk, attitude and behaviour of surgeons in southeast Nigeria.

Our objective was to evaluate the level of occupational risk, attitude and behaviour of surgeons towards HIV-infected patients. A questionnaire survey of 264 randomly selected surgeons in training or in practice in five different health institutions in southeast Nigeria within a five-month period was carried out. A 94% response rate was obtained with completed questionnaires from 264 surgeons. The respondents included obstetricians and gynaecologists (n = 78), general surgeons (n = 121), orthopaedic surgeons (n = 40), dental surgeons (n = 10), ophthalmologists (n = 6), urologists (n = 5), and

Virtual Care Remuneration Policy and Postdischarge Follow-Up Trends.

IMPORTANCE: Timely follow-up after pediatric hospitalization is critical to support safe discharge, yet many children face barriers to in-person visits. Virtual care may facilitate follow-up; however, its association with follow-up trends remains unknown. OBJECTIVE: To examine whether a system-wide virtual care remuneration policy, characterized by virtual care physician billing codes, was associated with changes in 7-day postdischarge follow-up rates overall and by hospitalization type and equity factors. DESIGN, SETTING, AND PARTICIPANTS: This population-based, repeated cross-sectional stu

Social inequalities in patient outcomes after total hip replacement surgery for osteoarthritis in England: A population-based cohort study of the National Joint Registry.

BACKGROUND: Reducing health inequalities is of national importance. Total hip replacement (THR) is a commonly used elective surgical procedure. Few studies have examined area-level inequalities for a wide range of outcomes following THR. The aim of this study is to compare area-level socioeconomic differences in outcomes following primary THR surgery for osteoarthritis in England. METHODS AND FINDINGS: This is a population-based prospective cohort study of the National Joint Registry (NJR). Data from the NJR were linked to national mortality, Hospital Episode Statistics and Patient Reported O

Preoperative anemia is an unsuspecting driver of machine learning prediction of adverse outcomes after lumbar spinal fusion.

BACKGROUND CONTEXT: Preoperative risk assessment remains a challenge in spinal fusion operations. Predictive modeling provides data-driven estimates of postsurgical outcomes, guiding clinical decisions and improving patient care. Moreover, automated machine learning models are both effective and user-friendly, allowing healthcare professionals with minimal technical expertise to identify high-risk patients who may need additional preoperative support. PURPOSE: This study investigated the use of automated machine learning models to predict discharge disposition, length of hospital stay, and re

Trends in short-term and delayed unplanned readmission in patients with adult spinal deformity undergoing posterior spinal fusion.

OBJECTIVE: Adult spinal deformity (ASD) affects many people in the US, often causing significant back pain and disability and disrupting activities of daily living. As a result, surgical intervention for deformity correction can help improve quality of life. Unplanned readmissions after surgery can significantly impact patients and the value of care. The aim of this study was to assess the trends in short-term and delayed unplanned readmissions following treatment for ASD. METHODS: A retrospective cohort study was performed using the 2016-2019 Nationwide Readmissions Database. All adult patie

Out-of-pocket health care spending among patients with acute limb ischemia stratified by income.

OBJECTIVE: Acute limb ischemia (ALI) results in high health care costs due to the near-universal need for surgical procedures and long hospital stays. Beyond the system costs incurred due to ALI, out-of-pocket (OOP) costs have a significant impact on patients' well-being and compliance with care. The objective of this study was to quantify OOP costs among individuals with ALI compared with the general population. METHODS: This was a cross-sectional study of individuals aged 18 years or older in the Agency for Healthcare Research and Quality Medical Expenditure Panel Survey (MEPS) from 2017 to

Endophthalmitis Rates and Types of Treatments After Intraocular Procedures.

IMPORTANCE: Long-term trend analyses of overall endophthalmitis rates and treatment patterns are scarce. It is also unknown if the deviation from the recommendations of the Endophthalmitis Vitrectomy Study toward decreased utilization of vitrectomy is associated with different vision outcomes. OBJECTIVE: To determine whether the rate of endophthalmitis after intraocular procedures or the primary treatment (prompt vitrectomy vs tap and inject) for endophthalmitis has changed over the past 20 years. DESIGN, SETTING, AND PARTICIPANTS: This cohort study examined data for cohorts created by query

Comparing binary & ordinal definitions of urinary & stool continence outcomes: Data from the National Spina Bifida Patient Registry.

INTRODUCTION: The National Spina Bifida Patient Registry (NSBPR) assesses bladder and bowel incontinence using ordinal categories, but prior NSBPR analyses employed binary classification. Our aims were to 1) perform the first NSBPR analysis of bladder and bowel incontinence as ordinal outcomes to compare to the binary definition and subject variables; 2) explore the correlation of incontinence with undergarment usage, and 3) assess incontinence status following continence surgeries. METHODS: Data from NSBPR participants' most recent clinic visit from 2013 to 2020 were analyzed. Ordinal catego

Long-term outcomes after lower extremity bypass in the actively smoking claudicant.

OBJECTIVE: Smoking is known to increase complications, including poor wound healing, coagulation abnormalities, and cardiac and pulmonary ramifications. Across specialties, elective surgical procedures are commonly denied to active smokers. Given the base population of active smokers with vascular disease, smoking cessation is encouraged but is not required the way it is for elective general surgery procedures. We aim to study the outcomes of elective lower extremity bypass (LEB) in actively smoking claudicants. METHODS: We queried the Vascular Quality Initiative Vascular Implant Surveillance

The Impact of Obesity on Total Hip Arthroplasty Outcomes: A Retrospective Matched Cohort Study.

Aim Previous research has shown that obesity is associated with worse postoperative outcomes. We aim to determine how rates of specific complications after total hip arthroplasty (THA) align with obesity status. We hypothesize that obese patients would have higher rates of complications and cost and thus have worse outcomes than non-obese patients. Methods Data were collected from a large commercial insurance database between 2011 and 2020. Patients underwent a hip replacement under current procedural terminology (CPT) and International Statistical Classification of Diseases (ICD-9/ICD-10) cod

Vitamin D Supplementation and the Incidence of Cataract Surgery in Older Australian Adults.

PURPOSE: Observational studies suggest that higher serum 25-hydroxy vitamin D (25(OH)D) concentration may be associated with lower risk of cataract. However, no randomized controlled trials have assessed the effect of vitamin D supplementation on the incidence of cataract. We aimed to assess whether vitamin D supplementation reduces the incidence of cataract surgery. DESIGN: We conducted an ancillary study of the D-Health Trial, a randomized, double-masked, placebo-controlled trial of monthly vitamin D conducted from 2014 through 2020 within the Australian general population. PARTICIPANTS: W

The Impact of Mental Health and Substance Use on Opioid Demand After Hip Fracture Surgery.

INTRODUCTION: Mental health and substance use and abuse disorders have been associated with poor patient-reported outcomes. Despite the prevalence of hip fractures in the United States, the relationship between opioid demand and these factors in hip fracture surgery is not well understood. The purpose of this study is to describe opioid filling volume and rates after hip fracture surgery and to identify mental health risk factors for increased demand. The study hypothesis is that psychiatric comorbidities such as depression and psychoses as well as substance use and abuse indicators such as pr
