Axial patellar engagement index and patellar tilt after medial patello-femoral ligament reconstruction in children and adolescents.

BACKGROUND: The medial patello-femoral ligament (MPFL) is a major patellar stabiliser whose reconstruction in adults involves graft fixation within a femoral tunnel. In skeletally immature patients, in contrast, the graft is fixed to the soft tissues to allow normal growth. The primary objective of this prospective study was to perform computed tomography (CT) and magnetic resonance imaging (MRI) assessments of medium-term correction of patellar tilt and of the axial patellar engagement index (AEI) after a paediatric variant of MPFL reconstruction in skeletally immature patients. HYPOTHESIS:

Posterior capsular release is a biomechanically safe procedure to perform in total knee arthroplasty.

PURPOSE: Surgeons may attempt to strip the posterior capsule from its femoral attachment to overcome flexion contracture in total knee arthroplasty (TKA); however, it is unclear if this impacts anterior-posterior (AP) laxity of the implanted knee. The aim of the study was to investigate the effect of posterior capsular release on AP laxity in TKA, and compare this to the restraint from the posterior cruciate ligament (PCL). METHODS: Eight cadaveric knees were mounted in a six degree of freedom testing rig and tested at 0°, 30°, 60° and 90° flexion with ± 150 N AP force, with and without a 710

Anterior knee pain in the young athlete: diagnosis and treatment.

The underlying etiology of anterior knee pain has been extensively studied. Despite many possible causes, often times the diagnosis is elusive. The most common causes in the young athlete are osteosynchondroses, patellar peritendinitis and tendinosis, synovial impingement, malalignment, and patellar instability. Less common causes are osteochondritis dissecans and tumors. It is always important to rule out underlying hip pathology and infections. When a diagnosis cannot be established, the patient is usually labeled as having idiopathic anterior knee pain. A careful history and physical examin

Correlation Analysis between Tibial Tuberosity-Trochlear Groove Distance and Other Patellar Stability Parameters in Young and Middle-aged Populations.

OBJECTIVE: The exact mechanism of patellofemoral instability has not been clearly clarified. The current study aims to explore the correlation between the tibial tuberosity-trochlear groove (TT-TG) distance and other patellar stability parameters. METHODS: A total of 60 individuals aged 18 to 40 years who underwent knee computed tomography (CT) examination between September 2014 and December 2017 were retrospectively recruited. Five reference sites were selected on the femoral trochlear articular surface in every CT image. The TT-TG distance and the trochlear groove angles (TGA) at the five r

Defining the role of TT-TG and TT-PCL in the diagnosis of lateralization of the Tibial tubercle in recurrent patellar dislocation.

BACKGROUND: The radiological indicators can help doctors determine whether to make tibial tubercle transfer. But which indicator is better is still in question. METHODS: 117 knees in 103 patients who had undergone patellar surgery and 60 knees in 58 patients who had no history of patellar dislocation from 2014 to 2019 were analyzed. Significant differences of tibial tubercle-trochlear groove (TT-TG) on CT and tibial tubercle-posterior cruciate ligament (TT-PCL) on MRI between the case group and the control group were estimated by an unpaired t test. Significant differences between TT-TG on CT

Magnetic Resonance Imaging Evaluation of Patellofemoral Joint.

BACKGROUND: Patellofemoral pain is the leading cause of knee pain in young adults. Magnetic resonance imaging plays an important role in early diagnosis of patellofemoral joint pathology. This study was carried out to evaluate the patellofemoral joint using magnetic imaging resonance and describe various predisposing factors for patellofemoral instability. METHODS: The study was carried in Department of Radiology, BPKIHS over a period of six months from February 2020 to August 2020. All patients with clinical diagnosis of patellar instability were included and Magnetic resonance imaging was d

[Diagnosis and therapy of chronic patellofemoral instability].

The chronic patellofemoral instability is a multifactorial disease, which is mostly congenital. Luxation of the patella is mainly atraumatic. Sole conservative treatment is often unsuccessful. Surgical treatment is needed in most cases and requires consideration of all pathologic changes of the patella-stabilizing anatomic structures. Rupture of the medial patellofemoral ligament is almost pathognomonic. In addition, the frontal mechanical axis, rotation of femur and tibia, the trochlear shape, the distance from the tibial tuberosity to the trochlear sulcus and the patellar height play an impo

Arthroscopic removal of a loose body osteophyte fragment after superior patellar dislocation with locked osteophytes.

The authors report the case of a loose body from a fractured osteophyte after a superiorly dislocated patella with locked osteophytes. Few cases of superiorly dislocated patellae have been reported in the literature and no cases of osteophyte fracture fragments after locked osteophytes with subsequent arthroscopic loose body removal have been reported. The loose body was removed and the distal pole of the patella was debrided arthroscopically. This patient and the majority of previously reported cases, herein reviewed, had patella alta with pre-existing patellofemoral arthrosis. Patella alta i

Hemarthrosis as a major complication after arthroscopic subcutaneous lateral retinacular release: a prospective study.

The use of arthroscopic lateral release and medial imbrication of the vastus medialis obliquus for recurrent dislocation of the patella was evaluated prospectively in 31 patients. Patients were examined clinically as well as radiologically; the average length of follow-up was 34.4 months. Seventy-one percent of the patients were satisfied with the operative result. Seven patients experienced redislocation of the patella. Postoperative effusion occurred in 13 (42%) patients, with 4 patients requiring aspiration and 3 patients requiring open revision because of hemarthrosis. Seven patients devel

Frequency of vascular injury with blunt trauma-induced extremity injury.

Vascular injury associated with select blunt trauma-induced orthopedic injuries is thought to occur frequently enough to require angiography in most cases. We sought to establish the actual frequency of such vascular injuries by reviewing all orthopedic and vascular patients seen during 4 years at one institution. A total of 569 at-risk parajoint fractures or dislocations were seen with a 1.5% incidence of vascular injury; all were recognized on clinical examination and vascular consultation then obtained. Vascular injury in this setting is uncommon, associated with particular clinical present

Subluxation of the patella. Computed tomography analysis of patellofemoral congruence.

Fifty patients who had patellar subluxation and thirty control subjects were examined using axial roentgenograms of the patellofemoral joint that were made with the knee in 30 and 45 degrees of flexion, as well as computed tomography scans that were made with the knee in full extension. The amount of lateral patellar tilt was quantitatively assessed using the lateral patellofemoral angle, as described by Laurin et al., and the congruence angle, as described by Merchant et al. In both the control subjects and the patients, the angle of Laurin et al. changed significantly when the knee was flexe

Extensor mechanism complications following total knee arthroplasty.

Extensor mechanism complications following 281 knee arthroplasties that included patellar resurfacing, performed by two surgeons in one hospital over a 6-year period, were reviewed. The mean follow-up period was 42 months. There were 28 (10%) extensor mechanism complications: 3 quadriceps tendon ruptures, 5 patellar fractures, 4 patellar tendon ruptures, 11 recurring patellar subluxations, 4 cases of patellar pain, and 1 malrotated patella. Nine (3%) required further surgery. Surgical technique may have contributed to the tendon ruptures; patellar fractures occurred mainly in patients who had

[Femoropatellar joint. Aspects of anatomy, physiology and pathophysiology].

The variety of anatomic conditions is critically confronted with the so-called dysplasias of the patella. Is chondromalacia or anterior knee pain a clinical diagnosis? The physiological aspects of the tension forces acting on the patella with its position in the midst of tendons and ligaments in valgus and in varus knees are discussed. What may the automatic rotation in extension effect on the patellofemoral joint-compartment?

The Elmslie-Trillat procedure for recurrent patellar subluxation after total knee arthroplasty.

Patellofemoral problems are the most common complications after total knee arthroplasty (TKA). We report a patient who had patellar subluxation twice within 7 months after primary TKA. Postoperative radiographs and computed tomography scans revealed a valgus knee with no evidence of malposition of the prostheses. To eliminate the recurrent patellar subluxation, we finally performed an Elmslie-Trillat procedure in combination with extensive lateral release and succeeded in achieving normal patellar tracking. At 1-year postoperative follow-up, the patient was satisfied with the results. The pate

Alpine ski injuries and their prevention.

Alpine skiing is a popular sport with significant risk of injury. Since the 1970s, injury rates have dropped from approximately 5 to 8 per 1000 skier-days to about 2 to 3 per 1000 skier-days. The nature of the injuries has also been transformed over the same period. Lower leg injuries are becoming less common while the incidence of knee sprains and upper extremity injuries is becoming more common. Much of this change can be attributed to advancements in binding technology, which effectively reduces lower leg injury, but does not adequately address the issue of knee sprains. Along with design,
