Recent trends in the management of maculopathy secondary to pathological myopia.

BACKGROUND: Pathological myopia is a frequent cause of secondary visual disturbance in young individuals worldwide. Myopic maculopathy describes a spectrum of clinical changes that comprise the main cause of visual loss among highly myopic individuals. Our aim is to describe current trends in the medical and surgical management of maculopathy secondary to pathological myopia. METHODS: The epidemiology, natural history, medical and surgical treatment modalities for choroidal neovascular membrane (CNV) and vitreomacular disorders secondary to pathological myopia (PM) are reviewed and evaluated.

Confluent laser photocoagulation for the treatment of retinopathy of prematurity.

PURPOSE: To describe the efficacy, outcomes, and complications of confluent laser technique in a cohort of premature infants with threshold retinopathy of prematurity. Laser photocoagulation has a good treatment outcome in the management of retinopathy of prematurity; however, the number and density of laser spots remains debatable. Laser treatment can be done in a scattered pattern, a near-confluent pattern, or a confluent treatment pattern. METHODS: A retrospective chart review of patients with threshold retinopathy of prematurity treated between 2003 and 2006 was conducted. Confluent laser

Refractive outcomes of lens-sparing vitrectomy for retinopathy of prematurity.

OBJECTIVE: To evaluate the refractive outcomes of 3-port lens-sparing vitrectomy (LSV) for subtotal retinal detachments owing to retinopathy of prematurity. METHODS: The study included 9 infants who had undergone complete ablative laser treatment for threshold retinopathy of prematurity in both eyes, subsequently developed stage 4A retinal detachment in 1 eye for which they underwent LSV, and maintained complete retinal attachment bilaterally. Eyes that underwent LSV were compared with fellow eyes. Cycloplegic refraction was performed, and corneal curvature, axial length, lens thickness, lens

Long term functional and structural outcomes of laser therapy for retinopathy of prematurity.

AIMS: To assess the long term functional and structural outcomes of premature babies who received diode laser photocoagulation for threshold retinopathy of prematurity (ROP). METHODS: 25 patients (43 eyes) treated with laser were recalled for assessment at a mean follow up of 11 years. A further seven patients (14 eyes) with subthreshold ROP, which had regressed spontaneously without laser treatment, were also examined. All children underwent distance acuity, near acuity, contrast sensitivity (CS), and colour vision assessments followed by a dilated fundal examination and cycloplegic autorefr

Long-term efficacy and visual acuity following transscleral diode laser photocoagulation in cases of refractory and non-refractory glaucoma.

AIMS: Transscleral diode laser cyclophotocoagulation (TSCP) is widely accepted in the treatment of refractory glaucoma especially in eyes with poor vision. However, until recently, there has been a reluctance to extend its use to eyes with good visual acuity (VA). The aims of this study were to evaluate the long-term efficacy and safety of TSCP for a range of glaucoma conditions, with particular emphasis on post-treatment VA in seeing eyes. METHODS: A retrospective analysis of case notes of 74 treated eyes was conducted over a period of 4-30 months (mean 12.5 months). RESULTS: Mean (SD) intr

[Retinopathy of prematurity].

This review summarizes the knowledge and highlights recent advances in clinical research of retinopathy of prematurity (ROP). This disease is still important, because improvements in neonatal intensive care during the last years have increased the survival of the most immature newborns, but did not diminish the ROP frequency. Ophthalmologic screening and, if applicable, laser treatment at the optimal time for advanced ROP prevents blindness in most cases. The history, classification and the modern concepts of ROP pathophysiology are described. The results of various multicenter treatment trial

Ulcers and nonvariceal bleeding.

Peptic ulcer remains the commonest and most significant cause of nonvariceal upper gastrointestinal bleeding. The incidence of peptic ulcer bleeding is rising in elderly patients, particularly for duodenal ulcer. Patients presenting with upper gastrointestinal bleeding who have low Rockall scores are at low risk of rebleeding and death. These patients currently utilize considerable health-care resources, but could safely be managed at home. The Rockall score can be used to predict the risk of rebleeding and death following variceal bleeding, but for patients with ulcer bleeding, its ability to

Anomaloscope matches in patients with diabetes mellitus.

BACKGROUND: Functional visual deficits can occur in patients with diabetes mellitus who show no visible morphological alterations in the retina. In this study we examined the colour vision of diabetic patients using metameric matches. Patients with and without retinopathy, as well as those who had been treated with laser photocoagulation, were examined to ascertain how the functional alterations in the diabetic eye alter with disease status. METHODS: Rayleigh (red-green) and Moreland (blue-green) metameric matches were determined in a total of 51 diabetic patients (24 patients with no retinop

[The outlook for the use of diode laser radiation for trabeculoplasty in treating primary open-angle glaucoma].

Clinical efficacy of diode laser trabeculoplasty is assessed in 35 eyes of 30 patients with primary open-angle glaucoma during during the initial and well-developed stages of diseases. Changes in the anterior segment of the eye, intraocular pressure, ocular hydrodynamics, visual functions, and status of the optic nerve were assessed 6-12 months after the operation. Diode laser trabeculoplasty is characterized by areactive clinical course and leads to a significant decrease of intraocular pressure, stabilizes and improves visual functions, and ameliorates intraocular humor discharge. The treatm

Failure of Nd-YAG photocoagulation therapy as treatment for Barrett's oesophagus--a pilot study.

INTRODUCTION: Acid suppression therapy and surgical antireflux procedures have not consistently led to regression of Barrett's epithelium. We reasoned that laser photocoagulation in combination with powerful acid suppression might destroy the metaplastic columnar epithelium and allow the area to be replaced by normal squamous mucosa. PATIENTS AND METHODS: Eight patients with endoscopic and histologically proven Barrett's oesophagus were recruited into the study. Four patients were randomized to receive omeprazole 40 mg daily and four patients received the same medical therapy plus repeated la
