Herbst appliance with skeletal anchorage versus dental anchorage in adolescents with Class II malocclusion: study protocol for a randomised controlled trial.

BACKGROUND: The Herbst appliance is an orthodontic appliance that is used for the correction of class II malocclusion with skeletal discrepancies. Research has shown that this is effective. However, a potential harm is excessive protrusion of the lower front teeth. This is associated with gingival recession, loss of tooth support, and root resorption. This trial evaluates a method of reducing this problem. METHODS/DESIGN: The study is a single-center, randomised, assessor-blinded, superiority clinical trial with parallel 1:1 allocation. Male and female young people (10-14 years old) with prom

Relationship Between Orthodontics and Temporomandibular Disorders: A Prospective Study.

AIMS: To investigate the possible relationship between the orthodontic treatment of Class II malocclusion and the development of temporomandibular disorders (TMD). METHODS: A total of 40 patients was evaluated at four time points: the day before the start of treatment employing bilateral Class II elastics (baseline), as well as at 24 hours, 1 week, and 1 month after the start of treatment. The development of TMD pain complaints in the orofacial region and changes in the range of mouth opening were assessed at these times. Shapiro-Wilk, McNemar, and Friedman tests with 5% significance level we

The extraction of maxillary lateral incisors for the treatment of a Class II crowded malocclusion: a case report.

BACKGROUND: The extraction of an upper lateral incisor for orthodontic purposes is rare and must be adequately justified. AIM: The present case report describes the management of a skeletal Class II crowded malocclusion that was facilitated by the extraction of upper lateral incisors and lower first premolars. METHODS: A 14-year-old male patient presented with a skeletal Class II crowded malocclusion with associated speech and chewing difficulties. Phase I of treatment involved the extraction of the upper lateral incisors and functional appliance therapy. Phase II included the extraction of

Asymmetrical distalization of maxillary molars with zygomatic anchorage, improved superelastic nickel-titanium alloy wires, and open-coil springs.

INTRODUCTION: In nongrowing patients with skeletal Class II malocclusion, premolar extraction or maxillary distalization can be used as camouflage treatment. Zygomatic anchorage enables distalization in uncooperative or noncompliant patients. We describe 1 such procedure in a 24-year-old woman. METHODS: We used novel improved superelastic nickel-titanium archwires combined with nickel-titanium open-coil springs to provide a constant and continuous low force to the dentition. RESULTS: We were able to successfully eliminate the protrusive profile and correct the Class II molar relationship usi

An esthetic treatment outcome of orthognathic surgery and dentofacial orthopedics in class II treatment: a cephalometric study.

AIM: The main objective of any orthodontic treatment is to achieve well-established stable occlusal relationship with a definite positive change in facial profile. The purpose of this study was to determine, if such a goal is achievable for patients who could be classified as borderline surgical cases without the invasive use of the actual surgery or, with the use of the recently developed and rapidly spreading fixed functional appliance system (Forsus) and a comparison of the esthetic treatment outcome with the two systems. MATERIALS AND METHODS: Twelve postadolescent borderline skeletal cla

Combined surgical therapy of temporomandibular joint ankylosis and secondary deformity using intraoral distraction.

Temporomandibular joint (TMJ) ankylosis is a pathological process caused by damage of the mandibular condyle. When this event takes place in subjects during the developmental age, it results in an alteration of the entire maxillofacial complex. Therefore, surgical methods able to remove the temporomandibular ankylosis also include necessary operations to correct the secondary maxillofacial deformity. The distraction osteogenesis has induced our center to modify the surgical protocol for the therapy of patients who have developed TMJ ankylosis and secondary maxillomandibular deformity. We have

Comprehensive mixed dentition treatment.

At the University of the Pacific, mixed dentition treatment is an integral part in the graduate program in orthodontics. One patient had a severe skeletal ANB discrepancy of 7 degrees, lower incisors crowding, and 12 mm of overjet. He was considered an excellent candidate for Phase I treatment to reduce the severity of the problem. It is our philosophy to take advantage of the growth potential in a patient by initiating Phase I treatment between the ages of 7 and 9 years, depending on the eruption of the permanent first molars and incisors and the severity of the malocclusion. We use leeway sp
