Common areas of litigation related to care during labor and birth: recommendations to promote patient safety and decrease risk exposure.

Reducing the risk of liability exposure and avoiding preventable injuries to mothers and infants during labor and birth can be relatively easy when all members of the perinatal care team (nurses, nurse-midwives, and physicians) agree to follow two basic tenets of clinical practice: use applicable evidence and/or published standards and guidelines as the foundation for care and whenever a clinical choice is presented, choose patient safety rather than production. Adhering to these two principles could theoretically eliminate the need for extensive and overly detailed policy and procedure manual

Trauma in pregnancy.

Of all cases that come to hospital emergency rooms, the traumatized pregnant patient presents one of the most complicated dilemmas because nurses and doctors who commonly treat trauma victims rarely have comparable expertise in the management of pregnancy. Treatment and care of the traumatized pregnant patient are challenging because advanced pregnancy influences the pattern of trauma, alters laboratory values and clinical assessments, and changes hemodynamic parameters. Pregnancy may alter the usual trauma routines, and trauma may affect the outcome of the pregnancy. It is imperative that bot

Umbilical cord blood transplants: treatment for selected hematologic and oncologic diseases.

Umbilical cord blood transplantation is a rapidly growing form of treatment for many types of cancer and hematologic disorders. The concepts behind the use of umbilical cord blood transplantation are based on information gained from experience in bone marrow transplantation. Previously discarded as human waste, the blood in the umbilical cord remnant and the placenta has been observed to be rich in hematopoietic stem cells. Techniques for collecting these stem cells from the placenta may vary among the institutions, physicians, and other health care providers, including midwives and nurse prac

Renal function in the neonate.

Renal function is altered in the neonate in comparison to the adult. Although structurally complete in terms of numbers of nephrons by 36 weeks, the newborn kidney is still functionally immature. Renal function undergoes rapid maturation during the first weeks postbirth in both term and preterm infants. During this time there is little margin for error in regulation of fluid and electrolyte balance, especially for the very low-birth-weight infant. Significant risks for all infants include overhydration, dehydration, and electrolyte imbalances. This article describes development of the renal sy

The management of urologic disorders in the neonate.

The urologic management of a neonate with a disorder of the urinary tract can present with a variety of problems. The increased use of antenatal ultrasounds has greatly changed the morbidity and mortality for many children with urinary tract abnormalities. This technology has enabled practitioners to be aware much earlier of these potential problems. Hydronephrosis is the most common presenting symptom for many of these infants. Some of the disorders to be discussed include posterior urethral valves, megaureter, ureteropelvic junction obstruction, and undescended testes.

Infants of diabetic mothers: current perspectives.

A thorough understanding of the metabolic alterations seen in pregnancies complicated by diabetes can help to provide the framework for comprehensive care of the mother and her infant. Nursing research may help to clarify the scope of problems encountered as well as delineating strategies to reduce mortality and morbidity. The problems encountered in infants of diabetic mothers can be better managed with a thorough understanding of the physiologic changes encountered. Assessment strategies can then be developed specific to the identified needs. Finally, interventions can be selected that promo

Expectant fathers' response to pregnancy: review of literature and implications for research in high-risk pregnancy.

It is evident that couvade syndrome exists in the industrialized culture. However, there are questions about the occurrence of couvade given the large range of reported incidence. Clinton found no difference in the occurrence of symptoms between expectant fathers and nonexpectant men throughout the three trimesters of pregnancy. Differences were noted, however, in the types and perceived seriousness of symptoms between these men, with expectant fathers reporting more colds, unintentional weight gain, insomnia, and restlessness. Significant differences in health deviation were reported by the n

Assessment and management of conjoined twins.

Although a rare anomaly, conjoined twins provide a unique challenge to the health care professional. At times, the ethical issues present difficult dilemmas for parents and hospital staff. An awareness of the moral dilemmas that tax the emotions of the most experienced hospital staff is the first step toward providing the appropriate support to these families. Nurses must have an understanding of the classification of conjoined twins and the preoperative and postoperative management. Additionally, awareness of the special needs of the family prior to discharge both before and after surgical se

Delivery of the second twin.

The course of twin pregnancy is most amenable to positive intervention during the antepartal period. However, once a laboring woman arrives on the labor floor, there is still opportunity to influence outcome. The management of multiple gestation is best accomplished through use of a multidisciplinary team. Intrapartal events are handled with greater confidence when each member of the team is aware not only of his or her specific role, but also of all the tasks that need to be accomplished. Rehearsal prior to the delivery and communication during the delivery are essential. Each team member can

Incidents and Adverse Events Associated With Peripherally Inserted Central Catheters in Neonatology: A Scoping Review.

BACKGROUND AND AIM: Peripherally inserted central catheters (PICCs) are essential in neonatal intensive care units for prolonged venous access. Despite their benefits, PICCs are associated with multiple complications. This scoping review mapped incidents and adverse events related to PICC use in neonates. METHODS AND MATERIALS: The protocol was registered on the Open Science Framework (DOI: 10.17605/OSF.IO/HKU4C). The review followed the Joanna Briggs Institute methodology and PRISMA-ScR guidelines. Searches were conducted in PubMed, EMBASE, Scopus, LILACS, BDENF, WPRO, Cochrane Library, Goog

Nurse-Led Early Ambulation After Caesarean Birth: A Cluster Randomized Controlled Trial of Recovery and Well-being Outcomes.

PURPOSE: To evaluate the effectiveness of a structured, nurse-led early ambulation program on postoperative pain, uterine involution, gastrointestinal recovery, and psychological outcomes among women after cesarean delivery. METHODS: A cluster-randomized controlled trial was conducted in a tertiary hospital in India among post-cesarean women assigned to either a structured early ambulation program or routine care by study week (10 clusters, 1:1 ratio). The intervention began 8-10 hours after surgery and continued for 4 days under nurse supervision. Primary outcome was pain intensity; secondar

Perinatal and Neonatal Palliative Care: Challenges and Contextual Solutions in Low- and Middle-Income Countries.

PURPOSE: The purpose of this review was to explore the primary obstacles to providing comprehensive, family-centered palliative care during the perinatal and neonatal period, using insights from existing literature and clinical practice. BACKGROUND: Perinatal and neonatal palliative care (PNPC) represents a holistic, interdisciplinary, and family-centered approach aimed at improving the quality of life for newborns diagnosed with life-limiting or life-threatening conditions. Despite its clinical and ethical significance, the provision of such care in low- and middle-income countries, such as

Development and Validation of a Clinical Simulation Scenario to Enhance Postpartum Hemorrhage Management.

PURPOSE: To develop and validate a clinical simulation scenario for postpartum hemorrhage management. BACKGROUND: Postpartum hemorrhage remains the leading cause of maternal mortality globally, despite being preventable in most cases. Structural deficiencies, limited access to care, and inadequate professional training contribute to its persistence. Clinical simulation offers an effective strategy for professional training in a safe, controlled, and context-appropriate environment. METHODS: This methodological study was conducted in three phases: systematic review, scenario updating, and val

Neonatal Eye Infections: Microbial Risks, Transmission Routes, and the Paradox of Traditional Human Milk Applications.

PURPOSE: This review critically evaluates the microbial risks, transmission routes, and cultural context of using expressed human milk as a topical remedy for neonatal eye conditions, and to reconcile its traditional use with current evidence-based neonatal care. BACKGROUND: Breastfeeding is essential for neonatal nutrition, promoting growth and immune protection. In some cultures, expressed human milk is used as a remedy for neonatal eye issues like redness and infections, due to its perceived antimicrobial properties. However, recent microbiological evidence reveals that milk can also harbo
