[An autopsy case with lower motor neuron disease showing a transient-appearance of anti-GM1 antibody and an improvement of conduction block after gamma-globulin administration].

We report a 63-year-old man who died of respiratory failure. He was well until 1992 (57 years of his age), when he had an onset of progressive weakness of the bilateral upper limbs. He showed no improvement with TRH administration in other hospital. On January 12, 1994, he admitted to our department because of the progressive muscle weakness. Neurologic examination revealed a muscular atrophy associated with severe weakness and hyporeflexia in both upper limbs, and fasciculation were seen in his tongue. Electrophysiological studies revealed mild conduction block in the left medial nerve, and F

Mental health treatment experiences before, during, and after incarceration: Perspectives of formerly incarcerated Black men.

Black men who have been incarcerated experience worse mental health outcomes than Black men without incarceration histories, reflecting both the trauma of incarceration and barriers to adequate care. This study describes the perspectives and experiences of formerly incarcerated Black men (FIBM) with mental health treatment before, during, and after incarceration. Using a qualitative design with semistructured interviews and embedded quantitative surveys, we interviewed 29 FIBM. Quantitative data described the sample and treatment histories, while qualitative data were analyzed using reflexive

Using large language models to assist qualitative thematic analysis of student reflections on advance care planning education.

BACKGROUND: Advance care planning (ACP) education has gained importance since Taiwan’s 2019 Patient Right to Autonomy Act, which empowers individuals to make advance medical decisions. Yet younger populations remain unfamiliar with ACP. This study examined how large language models (LLMs) can assist in qualitative thematic analysis of 1,761 reflection cards written by college students after an interactive ACP lecture. METHODS: Four LLMs—GPT-4, Gemini Advanced 2.0, DeepSeek, and Claude 3.5 (Sonnet)—were each tested under two prompting conditions: a short generic instruction and a longer role-ba

Surgical evacuation of an isolated interhemispheric subdural hematoma presenting with falx syndrome in the subacute period: A case report.

BACKGROUND: Traumatic isolated interhemispheric subdural hematoma (IHSDH) is rare, and falx syndrome is an uncommon but distinctive manifestation. Although many cases improve with conservative management, the optimal timing of surgery remains controversial. CASE DESCRIPTION: An 89-year-old man receiving edoxaban developed progressive left lower-extremity weakness 2 days after a minor head injury. Computed tomography (CT) showed a right isolated IHSDH measuring 14 mm in maximal thickness. Neurological examination revealed left lower-extremity monoparesis with persistent foot drop. Deep tendon

Effects of human embryonic stem cell-derived Schwann cells for nerve regeneration after facial nerve injury.

Facial nerve transection frequently results in incomplete functional recovery despite microsurgical repair due to axonal misalignment and limited intrinsic regenerative capacity. Schwann cells (SCs) are essential for peripheral nerve repair, supporting axon elongation, remyelination, and neurotrophic signaling. Human embryonic stem cell-derived Schwann cells (hESC-SCs), including Schwann cell precursors (SCPs) and immature SCs, constitute a scalable source of cells for tissue-engineered nerve grafts. This study investigated whether silicone conduits loaded with hESC-SCs could enhance facial ne

From Molars to Bronchus; The Tale of an Aspirated Dental Instrument.

BACKGROUND: Aspiration of dental instruments is a rare event that carries potentially serious complications when it occurs, especially in older patients, where protective reflexes are diminished. Clinical management may be difficult because of the risk of granulation tissue and different retrieval strategies according to the object's characteristics. In this report, we describe the case of an 84-year-old man who aspirated a dental bur during a routine dental procedure, necessitating immediate intervention. Timely and precise management was essential to avert severe complications. CASE PRESENT

Focused ethnography of nurses' interactions and behaviours in the therapeutic positioning of patients with traumatic brain injury in Pakistan: study protocol.

BACKGROUND: Proper positioning in trauma care is essential to prevent associated complications by stabilising haemodynamics, preserving the spine, securing the airway and protecting skin integrity. Motor vehicle crashes are the leading cause of traumatic brain injury (TBI) in Pakistan, necessitating rigorous safety measures and high-quality care due to their severe impact and high mortality rates. Resource-limited settings face significant challenges in providing adequate neurosurgical services. Moreover, there is a notable gap in the existing literature regarding effective care of patients wi

Assessing bedside measures of autonomic nervous system dysregulation in the PICU.

BACKGROUND: Autonomic nervous system (ANS) dysregulation is common during critical illness and is often measured using heart rate variability (HRV). It is unknown if other forms of ANS function, such as pupillary light reflex and thermoregulation, are altered in critically ill children. We aimed to determine whether automated pupillometry and delta (central-to-peripheral) skin temperatures were associated with HRV. METHODS: In this prospective observational pilot, inclusion criteria were admission to the pediatric intensive care unit (ICU) and mechanical ventilation. HRV was calculated using

Isolated Aphasia From a Left Parietotemporal Infarct Secondary to a Patent Foramen Ovale: A Case Report.

Isolated aphasia (IA) as the sole manifestation of stroke is rare and is often attributed to metabolic, toxic, or psychiatric disorders. Without typical stroke symptoms, such as hemiplegia or cranial nerve deficits (CND), IA can be easily overlooked. We present the case of a 41-year-old male accountant with a history of hyperlipidemia who presented to the emergency department with the sudden onset of an inability to read or speak, accompanied by confusion and anxiety. On examination, he had no CNDs, and his strength, sensation, reflexes, and cerebellar function were normal. His vital signs, bl

Population ageing, incarceration and the growing digital divide: Understanding the effects of digital literacy inequity experienced by older people leaving prison.

BACKGROUND: Digital inequity refers to the inequality and exclusion experienced by those who lack the same opportunities or circumstances to support the development of digital skills as the rest of modern society. One rapidly growing and highly vulnerable group to digital inequity is older people attempting to reintegrate into society after release from prison, where technology access is limited. Inadequate support for digital skills in this population entails widespread consequences for public health, human rights, social welfare and recidivism. This qualitative study is the first to: examine

A novel laser resection approach: efficacy of rotatable bi-channel en bloc resection of bladder tumor in a pilot in-vivo study.

En bloc resection of bladder tumor (ERBT) involves removing bladder tumors and their base. Laser resection has been used to reduce complications including bleeding and obturator nerve reflex (ONR). We developed a novel approach (rotatable bi-channel en bloc resection of bladder tumor (RBC-ERBT)) and assessed its efficacy in a pilot in-vivo study to enhance laser ERBT's applicability in challenging bladder regions. In the laser RBC-ERBT procedure, lesions were excised by inserting a holmium laser through the rotating external working channel, while forceps were inserted through the internal wor

Can upper cervical manual therapy affect the blink reflex in subjects with migraine and neck pain?

BACKGROUND: Neck pain is a common complaint among migraineurs possibly due to the anatomic connections between cervical and trigeminal afferents in the trigeminocervical complex (TCC). Manual therapy (MT) is used in the management of headache disorders, with demonstrable neurophysiological effects. The blink reflex (BR) is one method of analyzing neurophysiological effects in headache patients. The purpose of this study was to investigate the effect of upper cervical spine MT on BR in subjects with migraine and neck pain. METHODS & MATERIALS: Twenty subjects were assigned to a medication plus

Magnetic Resonance Imaging of Lung Perfusion.

"Lung perfusion" in the context of imaging conventionally refers to the delivery of blood to the pulmonary capillary bed through the pulmonary arteries originating from the right ventricle required for oxygenation. The most important physiological mechanism in the context of imaging is the so-called hypoxic pulmonary vasoconstriction (HPV, also known as "Euler-Liljestrand-Reflex"), which couples lung perfusion to lung ventilation. In obstructive airway diseases such as asthma, chronic-obstructive pulmonary disease (COPD), cystic fibrosis (CF), and asthma, HPV downregulates pulmonary perfusion

Case Report: A 72-Year-Old Woman With Progressive Motor Weakness, Dry Eyes and High Levels of Serum Neurofilament Light Chain.

BACKGROUND: Diagnosis of Amyotrophic Lateral Sclerosis (ALS) is challenging as initial presentations are various and diagnostic biomarkers are lacking. The diagnosis relies on the presence of both upper and lower motor neuron signs and thorough exclusion of differential diagnoses, particularly as receiving an ALS diagnosis has major implications for the patient. Sjögren's syndrome may mimic peripheral ALS phenotypes and should be considered in the work-up. CASE: A 72-year-old female presented with a mono-neuropathy of the right leg and a complaint of dry eyes and mouth. Initial diagnostic wor

Musicogenic epilepsy in paraneoplastic limbic encephalitis: a video-EEG case report.

Musicogenic epilepsy (ME), a peculiar form of reflex epilepsy, represents a neurological rarity and yet another demonstration of the extraordinary power of music on the human brain. Despite the heterogeneity of the reported musical triggers, patients' emotional response to music is thought to play a crucial role in provoking seizures. Accordingly, the mesial temporal structures (especially of the non-dominant hemisphere) appear most involved in seizure generation, although a more complex fronto-temporal epileptogenic network was documented in some cases. Autoimmune encephalitis has been recent
