Twenty-one patients (25 knees) with recurrent dislocation of the patella due to patellofemoral dysplasia were treated with a combined distal and proximal realignment of the extensor mechanism of the knee. In addition to traditional medial transfer of the tibial tuberosity, lateral release, and medial joint capsule duplication, vastus medialis transposition and deepening of the intercondylar sulcus on the proximal joint surface of the femur were done. A follow-up ranging from 1-14 years (mean 6 years) found the results excellent or good in 22 knees, fair or poor in three. One redislocation occu

Several techniques have been suggested for correction of clawtoe deformities in the lateral toes, ranging from multiple arthrodesis of the interphalangeal joints to phalanx resections, partial or total amputation of the impaired toes. An evaluation of the flexor-extensor transfer procedure is the basis of this study. Between 1977 and 1983, 39 feet were operated on in 31 patients, presenting a wide variation in etiology, degree of deformity, and symptoms. Average age at the time of surgery was 57.5 (16 to 80). All of the patients were available for recall and were able to return for an intervie

Lateral instability of the patella was found after recurrent, habitual and permanent dislocations. Contracture of soft tissues lateral to the patella only occurred with habitual and permanent dislocations, but medial laxity was present in all cases. After adequate release of the lateral contracture, the medial stability was improved by transposition of the lower three-quarters of the pes anserinus to the medial border of the patella, and the patellar ligament. This created a relatively unstretchable physiological sling which ensured dynamic stability of the patella. Six recurrent, 21 habitual

Silver sulfadiazine (Silvadene, U.S.; Flamazine, U.K.; Flammazine, N.; Sulplata, S.A.) is the newest topical antimicrobial agent available following worldwide clinical trials. Good control of infection is achieved without pain or other demonstrable side effects, using either dressings or the exposure technic. Many burned areas kept free of infection heal without grafting. Where necessary, early preparation for and good take of grafts has been attained by utilizing this new therapeutic agent. Markedly reduced mortality from burn wound sepsis has generally been observed.

CASE: We present the case of a 24-year-old woman who sustained a left midshaft clavicle fracture with acute subclavian artery compression, subclavian vein laceration, and complete brachial plexus palsy after a motor vehicle collision. The patient underwent urgent open reduction internal fixation of the clavicle and repair of the subclavian vein. Two years later, she underwent opponensplasty and flexor digitorum profundus tendon transfers. She regained satisfactory hand function, allowing her to return to full-duty work.
CONCLUSION: Clavicle fractures with associated acute neurovascular and br…

CASE: An 11-month-old male child presented with severe congenital thumb hypoplasia, syndactyly of the thumb and index finger, and radial longitudinal deficiency. A novel three-stage surgical approach was performed to achieve functionality while respecting familial preference for thumb preservation using sequential syndactyly release, structural reconstruction using bone graft, and muscle and tendon transfers.
CONCLUSION: The reconstructed thumb demonstrated early wound healing, stable alignment, and maintained a 45-degree palmar abduction even up to 5 years postoperatively. Despite minor fine…

Tissue expansion in the upper extremity is a valuable technique with expanding indications and expected outcomes. The primary indications are coverage problems after trauma, but the technique is also applicable to defects caused by treatment of hemangioma, nevi, tattoos, or hypertrophic scars as well as correction of unstable tissue or as an alternative to free tissue transfer. The advantage of expansion-specifically, match in tissue texture and retention of sensibility-may be more important in the hand and upper extremity than in other areas. The inevitable capsule that forms around the expan

In seventy patients with 104 club feet that were treated at our hospital and followed for ten to twenty-seven years after treatment, the functional results were satisfactory according to our rating system in 88.5 per cent of the feet, and 90 per cent of the patients were satisfied with both the appearance and function of the club foot. However, in the majority of the patients, foot and ankle motion was limited and the talocalcaneal angles as seen on the anteroposterior and lateral roentgenograms were not fully corrected. The amount of motion in the joints of the foot and ankle and the correcti

BACKGROUND: Donor site morbidity is an important consideration in the overall decision-making algorithm for fasciocutaneous free flap reconstruction of the head and neck.
METHODS: A retrospective case series was conducted of donor site complications occurring within 30 days of surgery among 226 consecutive anterolateral thigh (ALT) or radial forearm free flap (RFFF) microvascular free tissue transfers performed by multiple reconstructive surgeons between 2005 and 2010.
RESULTS: A greater number of donor site complications occurred among patients undergoing RFFF versus ALT free flaps (40; 35.

BACKGROUND: Free tissue transfer has not been fully adopted as the primary mode of treating foot and ankle defects in potentially indicated patients, partly because of the lack of sizable vessels and the potential in the long term of differential growth in the flap and the recipient site. Also lacking are long-term outcome studies in these growing patients.
METHODS: Between 1989 and 2002, 91 children with 93 foot and ankle defects underwent microsurgical reconstructions. The patients underwent reconstruction with flaps with a cutaneous component or with muscle flaps (skin-grafted).
RESULTS:

From all of the flaps reviewed, it is important to know how to select the most suitable choice in each case. Aside from the technical expertise of the surgeon, the indication depends on the size and the location of the substance loss. For large defects in any location, the radial forearm flap remains the most reliable and safest choice. For children and women, the authors prefer distant pedicled transfers or free flaps to minimize cosmetic donor site morbidity. For small or medium defects that cannot be managed by a local transposition flap, the indication is based on the location of the wound

INTRODUCTION: Irreparable rotator cuff tears (IRCTs) are large tears that can't be surgically repaired due to poor tissue quality, degeneration, or severe tendon retraction. These tears often involve multiple tendons and lead to fatty infiltration, humeral head migration, and tendon retraction. Patients with IRCTs typically present with pseudoparalysis, muscle atrophy, or anterosuperior escape. While various surgical options exist, outcomes are often inconsistent. This case series presents a novel technique for managing massive IRCTs, showing excellent, consistent results and offering a promis

This study reports on 9 patients with obstetrical brachial plexus injury who underwent a latissimus dorsi (LD) transfer to reconstruct external rotation of the upper extremity. Transfer of the LD to the rotator cuff is widely used for restoring shoulder abduction and external rotation in patients with obstetrical brachial plexus injury. Patients were classified according to type of paralysis, age of surgery, and shoulder function. This retrospective article evaluated the results of the LD transfer in a group of 9 patients. The study shows that in the near future, profit can be made in active a

BACKGROUND: There has been concern for iatrogenic injury to the peroneal nerve with posterolateral femoral tunnel placement in double-bundle anterior cruciate ligament reconstruction.
HYPOTHESIS: The common peroneal nerve and biceps tendon are at increased risk for injury by the guide wire as the knee is brought into increased extension.
STUDY DESIGN: Controlled laboratory study.
METHODS: An anatomical descriptive study was performed on 10 cadaveric knees (ages 49-67 years). After the native anterior cruciate ligament was removed arthroscopically, the posterolateral femoral tunnel starting

OBJECTIVE: To report the effect of weakening the superior rectus and inferior oblique (IO) muscles on ocular rotations.
DESIGN: Observational case series. We reviewed a 10-year period of medical records of consecutive patients who underwent bilateral 5- to 11-mm (mean, 8.0 mm; SD, 1.1 mm) superior rectus muscle recessions combined with an IO muscle recession, myectomy, or anterior transposition. The effects on ocular rotations and eyelid position were recorded for the 37 patients (69 eyes) who were followed up for at least 6 months postoperatively. Nonparametric 1-way analysis of variance was

PURPOSE: The forearm in children with cerebral palsy often assumes uncontrolled dynamic positioning in pronation when the hand is put into use. This hypertonic positioning seems to be an expression of dystonia rather than spasticity. This article reports the effect of pronator teres rerouting (PTR) using the technique of tendon Z-lengthening and repair on active motion and on forearm positioning during use.
METHODS: Thirty-one patients who had pronator teres rerouting were observed before and after surgery for active and passive forearm range of motion, changes in dynamic forearm positioning,

PURPOSE: Macular translocation is an optional surgical treatment for age-related macular degeneration. However, this technique induces postoperative torsional complaints and surgical counterrotation of the globe is mandatory. The purpose of this study is to report the effect of compensatory extraocular muscle surgery upon the torsional complaints in patients who underwent a macular translocation procedure.
DESIGN: The pre- and postoperative data on the first 35 patients in our department who underwent a counterrotation procedure after macular translocation surgery are reviewed.

To evaluate the effects of transfer of the trapezius and/or latissimus dorsi with the teres major for treatment of dysfunction of the shoulder in obstetrical brachial plexus palsy (OBPP), 34 patients with paresis of the abductors and external rotators, as well as co-contraction of the adductors in abduction, who had undergone reconstructive operations, were followed-up for at least 1 year. Of these, transfer of the latissimus dorsi with attached teres major to the insertion of the infraspinatus (single procedure), was performed in 25 cases, and transfer of both latissimus dorsi with teres majo
