S1 tuberculosis treated with segmental lumbopelvic fixation: a case report.

STUDY DESIGN: A case report. OBJECTIVE: To describe an effective surgical option for sacral tuberculosis (TB). SUMMARY OF BACKGROUND DATA: Sacral TB is a rare cause of low back pain. A differential diagnosis of TB should always be made, especially in India where TB cases are on a rampant rise with increasing drug resistance and immunosuppressed population. METHODS: A retrospective review. RESULTS.: We report on a 24-year-old woman with low back pain and radiculopathy. Magnetic Resonance Imaging (MRI) showed a destructive lesion in S1 body. Empirical antitubercular treatment was started else

The results of simultaneous posterior-anterior-posterior surgery in multilevel tuberculosis spondylitis associated with severe kyphosis.

We retrospectively studied the cases with tuberculous spondylitis of thoracolumbar region with two or more levels of involvement that underwent posterior instrumentation and fusion and anterior fusion with titanium mesh following anterior decompression using simultaneous successive posterior-anterior-posterior surgery. Among all patients with tuberculous spondylitis accompanied by medium or severe kyphosis, 20 patients who underwent simultaneous successive decompression, fusion and instrumentation with posterior-anterior-posterior surgery between 1999 and 2004 were included in the study. Bilateral Tendoachilles Xanthoma in a Young Female: A Rare Case Report.

INTRODUCTION: Tuberculosis (TB) is a leading cause of spinal infections, frequently presenting as spondylodiscitis. However, its clinical and radiological features can resemble those of Hodgkin's Lymphoma (HL). While Gene X-pert MTB/Rif Ultra and computed tomography (CT)-guided biopsies are common diagnostic tools for TB, they may sometimes be inconclusive. In such cases, HL should be considered as a differential diagnosis. Positron emission tomography-CT (PET-CT) scans are useful in assessing whether the disease is metabolically active. Accurate and timely diagnosis is essential for effective

Etiological and Radiological Spectrum of Longitudinal Myelitis: A Hospital-Based Study in North East India.

Introduction  An inflammatory lesion of the spinal cord where three or more than three vertebral segments of the cord is involved is called longitudinal extensive myelitis (LETM). It has several varied causes out of which neuromyelitis optica (NMO) and its spectrum disorder have received a distinct entity. Various radiological and clinical features help us to suspect an etiology which then further guides us into the treatment protocol and prognosis of the patients. Materials and Methods  A retrospective study performed in a referral center in North East India in 15 months. Thirty-two patients

Hybrid Minimally Invasive Technique for Treatment of Thoracolumbar Spondylodiscitis and Vertebral Osteomyelitis.

OBJECTIVE: Spondylodiscitis and vertebral osteomyelitis cause significant morbidity and mortality, and typically occur in patients with multiple comorbidities. The use of minimally invasive spinal surgery in the previous decade has offered the advantages of reduced intraoperative blood loss and postoperative pain for patients. In the present report, we have described our experience with using a hybrid minimally invasive (HMI) technique (combining percutaneous fixation with a mini-open approach for decompression and debridement) for the treatment of thoracolumbar spondylodiscitis, reporting the

Bacteremia coexisting with tophaceous gout of the spine mimicking spondylodiscitis: a case report.

STUDY DESIGN: A case report and review of the literature. OBJECTIVE: To report a rare case of bacteremia coexisting with spinal gout initially suspected as pyogenic spondylodiscitis. SUMMARY OF BACKGROUND DATA: Gouty spine is a rare disease. It is even rare when coexisting with bacteremia. The presentation as pain, fever, and positive blood culture will make it more difficult to distinguish from spinal infection. METHODS: A 64-year-old woman with type II diabetes mellitus and chronic renal insufficiency was admitted to the emergency department with a fever of 39 degrees C, chills, and back

Tuberculous optochiasmatic arachnoiditis: a devastating form of tuberculous meningitis.

Tuberculous meningitis is primarily a disease of the meninges of brain and spinal cord along with adjacent brain parenchyma. The characteristic pathological changes are meningeal inflammation, basal exudates, vasculitis and hydrocephalus. Tuberculous meningitis has a strong predilection for basal parts of the brain. Exudates, if dominantly present in the interpeduncular, suprasellar and Sylvian cisterns, result in optochiasmatic arachnoiditis and tuberculoma. Optochiasmatic arachnoiditis and tuberculoma are devastating forms of tuberculous meningitis and often associated with profound vision l

Primary demyelination as a nonspecific consequence of a cell-mediated immune reaction.

Primary demyelination occurs in a variety of human and experimental diseases known to be associated with the presence of inflammatory cells. However, the mechanism of demyelination remains unclear. The possibility that myelin can be damaged as a nonspecific consequence of a specific delayed type of hypersensitivity reaction directed at nonnervous tissue antigens was investigated. Guinea pigs were sensitized to tuberculin with Freund's complete adjuvant, and were challenged in the central and peripheral nervous system either with live or killed sonicated tubercle bacilli, Old Tuberculin, or tub

[Spondylitis: borderline findings in magnetic resonance tomography].

MR studies of 41 patients with confirmed spondylitis were evaluated with regard to imaging findings resembling metastases or fracture. 30 patients had MR results considered typical for spondylitis (contiguous changes in two vertebrae and disc, soft tissue tumour). 11 patients had MR studies differing from this pattern. Absence of soft tissue involvement and discontinuous marrow changes may be misdiagnosed as bone marrow metastases.

Total hip arthroplasty after seven decades of a Girdlestone situation due to tuberculous coxitis in childhood.

INTRODUCTION: Tuberculous coxitis accounts for about 15% of all osteoarticular tuberculosis cases and is the second most common after spinal tuberculosis. In extensive cases, Girdlestone resection arthroplasty is an option for primary surgical treatment followed in the course of time, with total hip arthroplasty (THR) to increase function. However, the remaining bone stock is generally poor. In these cases, the Wagner cone stem offers good pre-conditions for bone restoration even seven decades after Girdlestone, as presented here. CASE REPORT: A 76-year-old male patient was admitted to our de
