A simple post mortem room angiography method for the investigation of traumatic basal subarachnoid hemorrhage.

Identifying the site of vascular bleeding in fatal cases of traumatic basal subarachnoid hemorrhage (TBSAH) is important, but can be very difficult to achieve when there is extensive blood clot in the posterior cranial fossa. Post mortem angiography in these circumstances has been reported previously but with mixed results, and is rarely used in current practice within the United Kingdom. We have developed a simple and effective post mortem angiography method, using fluoroscopy and clear modern contrast medium, suitable for use in the autopsy room. Contrast medium was injected through an angio

Contemporary management of combat-related vertebral artery injuries.

BACKGROUND: Vertebral artery injuries (VAIs) following cervical trauma are uncommon. Advances in imaging technology and emerging endovascular therapies have allowed for the improved diagnosis and treatment of VAIs. We aimed to examine the contemporary management of combat-related penetrating VAIs during current US military operations. METHODS: A retrospective review was performed on US casualties with combat-related VAIs evacuated to a single military institution in the US from September 2001 to 2010 for definitive management. Casualty demographics, mechanism of injury, location and type of V

Posterior C2 instrumentation: accuracy and complications associated with four techniques.

STUDY DESIGN: Retrospective review of a single tertiary care spine database to identify patients with C2 instrumentation between January 2001 and September 2008. OBJECTIVE: (1) Evaluate a large series of posterior C2 screws to determine accuracy by computed tomography (CT) scan, (2) assess dimensions of "safe bony windows" with CT, and (3) assess perioperative complication rate related to errant screw placement. SUMMARY OF BACKGROUND DATA: The variable C2 anatomy can make instrumentation challenging and prone to potentially severe complications. New techniques have expanded available options

The pathological spectrum behind migraine aura status: a case series.

BACKGROUND: The recently released International Classification of Headache Disorders-3rd edition (1) includes migraine aura status (MAS) among the complications of migraine (A1.4.5). It is defined as the recurrence of at least three auras over a period of 3 days, in a patient suffering from migraine fulfilling criteria for 1.2 Migraine with aura (MA) or one of its subtypes. CASE SERIES: We describe three cases of MAS secondary to an organic brain lesion: a migrainous infarction, an acute ischemic stroke secondary to a vertebral artery dissection, and an inflammatory demyelinating disease of t…

[Lateral medullary infarction presenting with truncal lateropulsion and decreased pain and temperature sensation below T10 sensory level].

A 45-year-old man noted a tendency to fall to the left side on standing following headache and vomiting. Neurological findings were unremarkable except for truncal lateropulsion to the left side. Brain magnetic resonance imaging (MRI) showed a small infarct in the left inferolateral part of medulla. Clinical and imaging findings suggested vertebral artery dissection as the cause of stroke. On hospital day 6, the patient developed decreased pain and temperature sensation in the right side below T10 sensory level and decreased sweating on the left side of the face with miosis of the left pupil. …

Endovascular recanalization for chronic symptomatic intracranial vertebral artery total occlusion.

Purpose. The outcome of recanalization in patients with chronic symptomatic intracranial vertebral artery (ICVA) total occlusion is poor. This paper reports the technical feasibility and long-term outcome of ICVA stenting in patients with chronic symptomatic total occlusion. Methods. Retrospective review of our prospectively maintained intracranial intervention database to identify patients with symptomatic total occlusion of ICVA with revascularization attempted >1 month after index ischemic event. Results. Eight patients (mean age 58 years) were identified. One had stroke and 7 had recurrent

Isolated basilar traumatic subarachnoid hemorrhage: an observer's 25 year re-evaluation of the pathogenetic possibilities.

This paper was inspired by Leadbeatter's recent review [1] on the subject, and consists primarily of a recapitulation of this author's observations in the 25 years since publication of his paper [2] describing three cases of traumatic basilar subarachnoid hemorrhage resulting from direct trauma to the upper lateral neck. Those observations include personal experience and case reports personally communicated or published. Leadbeatter's analysis makes the following three salient points to which the author considers a response to be appropriate: 1. For the ten years following Contostavlos' paper,

Investigating Outcomes of Pediatric Stroke due to Extracranial Arterial Dissection: A Population-Based Cross-Sectional Study of 11,000 Patients.

Extracranial arterial dissection (EAD) involving the carotid and vertebral arteries poses a significant risk for acute ischemic stroke (AIS) in children and often presents challenges in diagnosis. The investigation of outcomes and interventions in pediatric AIS secondary to EAD remains significantly underexplored. This retrospective cohort study examines the clinical outcomes of pediatric patients experiencing EAD. The National Inpatient Sample was queried from 2015 to 2019 for pediatric patients aged 0-18 years with a primary diagnosis of AIS, using International Classification of Disease 10t

Use of a large-bore 088 intracranial access support catheter for delivery of large intracranial devices: case series with the TracStar LDP in 125 cases.

BACKGROUND: The delivery of neuroendovascular devices requires a robust proximal access platform. This demand has previously been met with a 6Fr long sheath (8Fr guide) that is placed in the proximal internal carotid artery (ICA) or vertebral artery segments. We share our experience with the first 0.088 inch 8Fr guide catheter designed for direct intracranial access. METHODS: We retrospectively reviewed a prospectively maintained IRB-approved institutional database of the senior authors to identify all cases where the TracStar Large Distal Platform (LDP) was positioned within the intracranial

Vertebral artery-to-vertebral artery bypass with interposed radial artery or occipital artery grafts: surgical technique and report of three cases.

BACKGROUND: The treatment of unclippable vertebral artery (VA) aneurysms incorporating the posterior inferior cerebellar artery with parent artery preservation is among one of the most formidable challenges for cerebrovascular microsurgery and endovascular surgery. We propose that intracranial VA reconstruction using an extracranial VA-to-intracranial VA (VA-VA) bypass with a radial artery graft or an occipital artery graft may be an additional technique in the armamentarium to treat these formidable lesions. The rationale, surgical technique, and complications are discussed. METHODS: Three i

Treatment of aneurysms by excision or trapping with arterial reimplantation or interpositional grafting. Report of three cases.

The authors report three cases of ruptured, large or giant aneurysms that were treated by excision or trapping, followed by revascularization of distal vessels by means of arterial reimplantation or superficial temporal artery interpositional grafting. In the first case, a large serpentine aneurysm arising from the anterior temporal branch of the right middle cerebral artery (MCA) was excised and the distal segment of the anterior temporal artery was reimplanted into one of the branches of the MCA. In the second case, a giant aneurysm, fusiform in shape, arose from the rolandic branch of the M

Evaluation of the vertebrobasilar system in thoracic aortic surgery.

BACKGROUND: We evaluated the probability of vertebrobasilar system malperfusion due to occlusion of the left subclavian artery as assessed by preoperative magnetic resonance angiography in patients scheduled to undergo thoracic aortic surgery. METHODS: (Study 1) From January 2000 through March 2009, we studied variations of vertebral arteries in 301 patients scheduled to undergo thoracic aortic surgery. We classified vertebral artery variations into 3 categories according to the findings on preoperative magnetic resonance angiography: connection type, interrupted right vertebral artery, and i

From Cervical Trauma and Head Injury to Malignant Cerebral Edema: Blunt Cerebrovascular Injury Causing Large-Vessel Occlusion Treated With Thrombectomy and Neurocritical Care.

Blunt cerebrovascular injury (BCVI) is an uncommon but potentially life-threatening complication of cervical trauma that may lead to delayed ischemic stroke. Early diagnosis is often challenging because neurological symptoms may initially be absent or mild. We report a case of traumatic internal carotid artery dissection associated with cervical fracture that resulted in large-vessel occlusion and required both endovascular treatment and intensive neurocritical care. A 57-year-old woman fell down stairs and sustained cervical trauma. Initial computed tomography revealed a fracture of the atlas

Antithrombotic therapy for stroke in young adults.

Stroke in young adults is a markedly heterogeneous disease, and remains an understudied phenomenon. While advances are being made in our understanding of the pathophysiology of its underlying conditions, treatment concerns are controversial, and clinical trials are sorely lacking. This review presents an overview of some of the relevant management issues in hypercoagulable states, migraine, patent foramen ovale, vascular dissection and venous sinus thrombosis.

Comparative Cadaveric Analysis for Surgical Corridor and Maneuverability: Far-Lateral Approach and Its Transcondylar Extension.

OBJECTIVE: The necessity of partial occipital condyle (OC) resection for lesions in the ventral craniocervical junction is debatable. This study's purpose was to compare the surgical exposure of the classic far-lateral approach (FLA) and transcondylar FLA. METHODS: The classic FLA and transcondylar FLA were performed in 12 human cadaveric heads (24 sides). The surgical corridor of 3 levels (a: vagus nerve, b: from the midpoint of proximal ends of the vagus and hypoglossal nerves to the midpoint of the distal ends of each nerve, c: hypoglossal nerve) and the maneuverability (the area between n

Transient cortical blindness following a Hangman's fracture: a case report and review of literature.

We describe a case with transient cortical blindness after trauma with no obvious structural damage to the vertebral artery in the presence of a C2 spondylolisthesis. A patient complaining of blindness in a setting of polytrauma should always alert the possibility of a cervical spine injury with vertebral artery ischaemia.

Assessment of skin extensibility and joint hypermobility in patients with spontaneous cervical artery dissection and Ehlers-Danlos syndrome.

Ehlers-Danlos syndrome (EDS) is a hereditary connective tissue disorder. One important clinical characteristic of classical type EDS is skin hyperextensibility. Examination of clinical evidence and electron microscopic views of skin biopsies suggest that connective tissue abnormalities resembling very mild EDS are present in a sizable proportion of patients with spontaneous cervical artery dissection (sCAD). Manual assessment of skin extensibility is difficult. Therefore, non-invasive machine-aided measurement of skin extensibility was used and compared with manual assessment of skin extensibi

Sequence analysis of the COL5A2 gene in patients with spontaneous cervical artery dissections.

The authors searched for mutations in the gene that codes for the alpha2 chain of type V procollagen in 10 patients with spontaneous cervical artery dissections (sCAD). Two patients carried a missense mutation affecting the predicted C-propeptide (T1227S; D1429V). A third patient carried two mutations (V509A and P830L) in the same alpha2(V) chain. The T1227S mutation and the V509A/P830L haplotype also were detected among 50 healthy subjects. The D1429V substitution was detected neither in a series of 150 healthy control subjects nor among 50 additional patients with sCAD.
