[Recording of vestibular myogenic evoked potentials in patients with disturbed inner function].

In this study, we measured VEMP in patients with disturbed function of inner ear. We studied 4 patients with unilateral sensorineural deafness and normal excitability of vestibular organs; 6 persons with unilateral weakness of vestibular function and 10 healthy subjects. We found that in patients with unilateral weakness of vestibular function VEMPs had short latencies and diminished amplitudes whereas in patients with unilateral deafness VEMPs were unchanged. These data strongly suggested that VEMPs were generated by stimulation of vestibular and not cochlear part of inner ear-sacculus. Our d

[Infrared and video oculography--alternatives to electrooculography?].

With the introduction of each new technique for registering eye movements, the question arises concerning whether these provide a reliable and accurate alternative for the diagnosis of labyrinthine dysfunction when compared to the most commonly used electro-oculographic technique. To answer this question we compared mean slow-phase velocity (SPV) using three different recording techniques:electro-oculography (EOG), video-oculography (VOG) and infrared oculography (IROG) during four different types of examinations. The examinations were the eye target tracking test (ETT), horizontal and vertica

Isolated directional preponderance of caloric nystagmus: II. A neural network model.

HYPOTHESIS: The purpose of this study was to simulate an isolated directional preponderance (DP) on bithermal caloric testing by constructing a realistic neural network model. The simulation was designed to capture not only the characteristics of the nystagmus response to caloric stimulation but also the response to rotational stimulation in patients with an isolated caloric DP. BACKGROUND: The nature of an isolated DP--that is, a DP in the absence of a significant spontaneous nystagmus or canal paresis--is outlined in the preceding article. In this article, the authors investigate the possib

Tetra-ataxiometric Posturography in Patients with Migrainous Vertigo.

BACKGROUND: Migraine is a common disorder characterized by headache attacks frequently accompanied by vestibular symptoms like dizziness, vertigo, and balance disorders. Clinical studies support a strong link between migraine and vertigo rather than between other headache types and vertigo or nonvertiginous dizziness. There is a lack of consensus regarding the pathophysiology of migrainous vertigo. Activation of central vestibular processing during migraine attacks and vasospasm-induced ischemia of the labyrinth are reported as the probable responsible mechanisms. Because vestibular examinatio

[Clinical features associated with sudden hearing loss in children].

Sudden sensorineural hearing loss is usually unilateral, and the cause is not identified in most adult cases. However, a specific cause has frequently been found in the case of children, in whom idiopathic sudden sensorineural hearing loss (ISHL) is comparatively rare. We investigated 20 cases of acute unilateral sensorineural hearing loss in children associated with a certain disease, but which was first diagnosed as ISHL. Thirteen patients aged 6 to 16 years old were diagnosed as having psychogenic (functional) hearing loss. Discrepancies in behavioral and objective tests are most valuable w

[Molecular Pathogenesis, Genetic Profiles, and Therapeutic Strategies for NF2-Related Schwannomatosis].

Neurofibromatosis type 2 (NF2), now redefined as NF2-related schwannomatosis (NF2-SWN), is a hereditary tumor syndrome characterized by bilateral vestibular schwannomas, multiple meningiomas, and other central nervous system tumors. The revised diagnostic criteria incorporate genetic testing and consideration of somatic mosaicism, enabling a more accurate and earlier diagnosis. A strong genotype-phenotype correlation has been established: truncating mutations are associated with an earlier onset and severe clinical course, whereas missense or splice-site mutations are often linked to milder ph

Postural function in idiopathic normal pressure hydrocephalus before and after shunt surgery: a controlled study using computerized dynamic posturography (EquiTest).

INTRODUCTION: Postural dysfunction is one of the major features of idiopathic normal pressure hydrocephalus (iNPH). With computerized dynamic posturography (CDP) balance can be assessed objectively. The primary aim of this study was to describe the postural function in iNPH patients pre- and post-operatively in comparison with healthy individuals (HI) using CDP. SUBJECTS AND METHODS: Thirty-five patients (16 M, 19 F) with a mean age of 73 (range 49-81) with iNPH, and sixteen HI (7 M, 9 F) aged 73 (62-89) were included. iNPH patients were operated on with a ventriculo-peritoneal shunt. Vestibulo-oculomotor disturbances caused by industrial solvents.

Animal experiments show that intoxication with hydrocarbon solvents influences the vestibulo-oculomotor reflex arc and indicate that the solvents block the inhibition of the vestibulo-oculomotor reflex presumably exerted by the cerebellum. The blood concentrations necessary to elicit oculomotor disturbances in animals were smaller than those that disturbed the vestibular function. Accordingly, in human experiments styrene and toluene exposure did not elicit any positional nystagmus but caused an increased saccade speed and a diminished visual suppression of vestibular nystagmus. Some cerebella

[Hearing and equilibrium disorders in patients with neurosyphilis].

83 patients with neurosyphilis and 12 patients with latent syphilis underwent a cochleovestibular examination. Pathological changes were present in 54% of the patients with neurosyphilis. In the group of patients with latent syphilis functional changes could only be found in 25% of the patients. The greatest number of hearing disturbances occurred in persons suffering from general paresis, 65% were affected. Usually damage occurred in both ears. Specific topical diagnosis was often not possible although the results of the stapedial reflex indicated that peripheral cochlear lesions were present

Vestibular function in families with inherited autosomal dominant hearing loss.

The inner ear contains the developmentally related cochlea and peripheral vestibular labyrinth. Given the similar physiology between these two organs, hearing loss and vestibular dysfunction may be expected to occur simultaneously in individuals segregating mutations in inner ear genes. Twenty-two different genes have been discovered that when mutated lead to non-syndromic autosomal dominant hearing loss. A review of the literature indicates that families segregating mutations in 13 of these 22 genes have undergone formal clinical vestibular testing. Formal assessment revealed vestibular dysfu

Vestibular symptoms and otoneurological findings in retired offshore divers.

INTRODUCTION: Inner ear barotraumas and decompression sickness (DCS) may cause acute vestibular symptoms in divers. The result may be irreversible damage to the vestibular end organs or their central connections. We examined a group of offshore divers in order to find out how many divers experience vestibular symptoms later in life and how this was related to occupational history and objective findings. METHODS: A questionnaire was sent to 230 offshore divers (mean age 52 yr) and 166 age-matched non-diving controls. Most of the divers had retired from diving. A subgroup (n=96) of the divers w

Persistent static imbalance among acute unilateral vestibulopathy patients could be related to a damaged velocity storage system.

Background: Acute unilateral vestibulopathy (AUV) is common but, the course of disease recovery is variable. Moreover, the final recovery status might vary between subjects. The remaining symptoms of these patients indicate the poor recovery of static imbalance, which could limit social activities and decrease their quality of life. Objective: To determine the possible predictive parameters of prolonged static imbalance (PSI) among acute AUV, we compared several vestibular function test (VFT) results between control vestibulopathy (CV) and PSI patients. Materials and methods: Subjects were div

Time-resolved directional organization of brain networks: distinct strength and temporal signatures from neurophysiology to disease.

Brain function depends on time-resolved directional organization: influence is continuously redistributed across regions, shaping who drives whom, when, and for how long. Yet dynamic neuroimaging has largely emphasized undirected fluctuations in functional connectivity, limiting biological interpretation of asymmetric interactions. Here we show that directed influence can be resolved into two biologically distinct signatures: strength, reflecting the efficacy of influence, and duration, reflecting its temporal extent. To test this principle, we developed sliding-window prediction correlation (

Ocular vestibular evoked myogenic potentials and intravestibular intralabyrinthine schwannomas.

Intravestibular intralabyrinthine schwannomas (ILSs) are uncommon benign tumors that arise from the saccular, utricular, and lateral and superior ampullary nerves. According to the literature, there is an average delay of 8 years between the onset of symptoms and diagnosis. The diagnosis is based on an audiovestibular examination and magnetic resonance imaging (MRI). We describe a case of intravestibular ILS in which we included the ocular vestibular evoked myogenic potentials (oVEMPs) test in the diagnostic workup. The oVEMPs test is a relatively new neurophysiologic diagnostic modality that

[Vertigo in patients with otosclerosis and post stapedectomy].

The aim of the paper was evaluation of behaviour of the balance system in patients with clinical otospongiosis and after stapedectomy. The investigation was carried out in 131 otosclerotic patients. Before and post operation vertigo was noticed and the spontaneous nystagmus, positional nystagmus and directional preponderance in kinetic test were examined in ENG. Vertigo and objective symptoms of disturbance of the vestibular function were found in clinical otospongiosis, but postoperative subjective and objective symptoms were more frequently.
