We report an unusual case of unilateral spontaneous lens dislocation in a 52-year-old woman with a posterior polar cataract. We postulate that the increasing lens size secondary to nuclear sclerosis may have exerted pressure on a thinned posterior capsule, causing the capsule to rupture spontaneously. The unusual weak posterior capsule in posterior polar cataract is a well-recognized risk factor in phacoemulsification surgery. We believe this is the first report case of spontaneous lens dislocation secondary to posterior capsule rupture in posterior polar cataract.

We describe special techniques to visualize the donor tissue in Descemet's stripping endothelial keratoplasty and to prevent the tissue or stripped recipient Descemet's membrane from dropping onto the retina. The techniques were used successfully to treat corneal decompensation in 3 consecutive cases with no iris or lens present. The results indicate that with careful planning and a good technique, aniridic aphakic patients can benefit from the rapid corneal rehabilitation provided by endothelial keratoplasty.

This article describes the clinical presentation of glaucoma in eyes operated on for congenital cataract and reviews current theories as to its pathophysiology. The literature does not provide a conclusive explanation for the seeming increase in aphakic glaucoma in children. Long-term studies comparing like groups may help resolve this question.

Implantation of intraocular lenses in children was performed in 42 cases in our department. In 26 children it concerned primary implants, in 16 secondary implants. No major postoperative complications were observed in the period between the 6th and the 24th month after surgery. The visual acuity of 0.5 or better showed 70 p.c. of children after the primary and 66 p.c. after secondary implants.

A technique for controlling intraocular pressure in the immediate postoperative period with Molteno implants is described. Filtration tubes were closed with polypropylene sutures in 13 patients. Filtration tubes closed with 7-0 sutures provided the most satisfactory partial to complete lumen closure. The ligatures in six patients later were opened by argon laser.

This study presents the results and complications of 389 patients who were treated with the Nd:YAG laser between September 1982 and November 1983 with at least a six-month follow-up. The majority of patients had a secondary discission of the posterior capsule.
