Prognostic value of left atrial size in hypertensive African Americans undergoing stress echocardiography.

BACKGROUND: Left atrial (LA) enlargement is a marker of increased risk in the general population undergoing stress echocardiography. African American (AA) patients with hypertension are known to have less atrial remodeling than whites with hypertension. The prognostic impact of LA enlargement in AA with hypertension undergoing stress echocardiography is uncertain. AIM: To investigate the prognostic value of LA size in hypertensive AA patients undergoing stress echocardiography. METHODS: This retrospective outcomes study enrolled 583 consecutive hypertensive AA patients who underwent stress e…

Oral Magnetoelectric Neuroimmunomodulation via the Gut-Brain-Spleen-Heart Axis for Noninvasive Atrial Fibrillation Therapy.

Atrial fibrillation (AF), the most common cardiac arrhythmia, is closely associated with autonomic imbalance and inflammation. While low-level vagus nerve stimulation (LL-VNS) offers therapeutic benefits, its clinical application is limited by the need for surgical implants. Here, we present a noninvasive, orally delivered magnetoelectric neuroimmunomodulation platform that targets the gut-brain-spleen-heart axis to deliver LL-VNS for AF therapy. The platform comprises ingestible microparticles (MPs) embedding magnetostrictive Fe3O4@BaTiO3 nanogenerators (FO@BTO NGs) within a conductive matrix

Targeted Quantitative Metabolomics and Lipidomics Reveal Dysregulated Metabolic Networks and a Serum Candidate Biomarker for Atrial Fibrillation.

BACKGROUND: Atrial fibrillation (AF) is the most prevalent clinical arrhythmia with severe cardiovascular complications, yet its metabolic molecular mechanisms remain poorly defined. Omics-based metabolic profiling provides a powerful strategy to systematically decode AF-associated metabolic disorders. METHODS: In this work, high-coverage targeted liquid chromatography-tandem mass spectrometry (LC-MS/MS) metabolomics and lipidomics were applied to absolutely quantify 746 serum metabolites from AF patients and healthy controls. RESULTS: We systematically characterized global metabolic perturb

Idiopathic Atrial Flutter With Tachycardia-Induced Cardiomyopathy Detected at a Routine Infant Health Checkup: A Case Report.

Idiopathic atrial flutter (AFL) is an uncommon supraventricular tachyarrhythmia in early infancy. When sustained, AFL may lead to tachycardia-induced cardiomyopathy (TIC), a potentially reversible cause of ventricular dysfunction after rhythm control. We report a 3-month-old boy in whom severe TIC was detected incidentally at a routine infant health checkup despite minimal symptoms. The examining pediatrician noted persistent age-inappropriate tachycardia and referred the infant urgently. On admission, the ventricular rate fluctuated between 160 and 240 beats/min, likely reflecting variable at

Bradycardia-induced transient myocardial dysfunction in 3 dogs.

Bradycardia-induced cardiomyopathy is a reversible form of myocardial dysfunction caused by prolonged bradyarrhythmia, most commonly due to high-grade atrioventricular block or sinus node dysfunction. Although bradycardia-induced cardiomyopathy is well recognized in human medicine, it remains underreported in veterinary literature. This case series describes 3 dogs diagnosed with 3rd-degree atrioventricular block that presented with clinical signs including exercise intolerance, syncope, and cardiomegaly. Each dog underwent comprehensive diagnostic evaluation, including physical examination, t

Progressive Right-Sided Heart Failure and Secondary Tricuspid Valve Disease in Pediatric Lutembacher Syndrome: A Case Report.

BACKGROUND: Lutembacher syndrome (LS) is a rare condition characterized by the coexistence of mitral stenosis (MS) and an atrial septal defect (ASD). Clinical manifestations vary according to the severity of MS and the size of the ASD. CASE PRESENTATION: We report a 13-year-old female with congenital MS and a secundum ASD who presented with decompensated right heart failure after being lost to follow-up for 10 years. Progressive worsening of MS and enlargement of the ASD resulted in significant left-to-right shunting, severe right heart volume overload, pulmonary hypertension, tricuspid annul

Gut microbiome and atrial fibrillation: mechanistic insights metabolites and comorbidities. Systematic review.

Emerging evidence indicates that the gut microbiome (GM) and its metabolites may play a role in the development of cardiometabolic diseases, including atrial fibrillation (AF). Although the current data are limited, studies suggest that the GM composition in AF mirrors that observed in associated conditions such as heart failure, hypertension, diabetes, and coronary artery disease (characterized by increased genera Lactobacillus, Enterococcus, Streptococcus, and decreased genus Faecalibacterium). Additionally, patients with AF appear to have distinct microbial alterations, including increased

The Hidden Shunt: An Asymptomatic Atrial Septal Defect Revealed by Echocardiography in a High-Performance Teenage Football Player.

Pre-participation cardiovascular evaluation aims to detect occult cardiac conditions that may increase the risk of sudden cardiac events in young athletes. Although echocardiography is not universally recommended for systematic screening, it can reveal silent congenital anomalies, particularly in highly trained individuals. In this report, we describe a case of a 14-year-old competitive football player with no prior medical history who underwent routine pre-participation evaluation. Clinical examination and resting electrocardiogram were normal. Transthoracic echocardiography (TTE), however, r

Characterization of Biomarkers of Thrombo-Inflammation in Patients with First-Diagnosed Atrial Fibrillation.

Patients with first-diagnosed atrial fibrillation (FDAF) exhibit major adverse cardiovascular events (MACEs) during follow-up. Preclinical models have demonstrated that thrombo-inflammation mediates adverse cardiac remodeling and atherothrombotic events. We have hypothesized that thrombin activity (FIIa) links coagulation with inflammation and cardiac fibrosis/dysfunction. Surrogate markers of the thrombo-inflammatory response in plasma have not been characterized in FDAF. In this prospective longitudinal study, patients presenting with FDAF (n = 80), and 20 matched controls, were included. FI

P-wave terminal force in lead V1 is associated with recurrence after catheter ablation in patients with paroxysmal atrial fibrillation and normal left atrial size.

BACKGROUND: A previous investigation reported that an abnormal P-wave terminal force in lead V1 (PTFV1) is a marker for electrical remodeling of the left atrium (LA). We aimed to assess the relationship of PTFV1 with LA tachyarrhythmia (LATA) recurrence after radiofrequency catheter ablation (RFCA) in patients with paroxysmal atrial fibrillation (PAF) and normal LA size. METHODS: Patients with PAF and normal LA size (LA volume index < 34 ml/m2) who underwent RFCA were consecutively included between January 2018 and December 2020 and divided into two groups based on the presence (recurrence gr

A pilot study for risk stratification of ventricular tachyarrhythmia in hypertrophic cardiomyopathy with routine echocardiography parameters.

Ventricular tachyarrhythmia (VTA) are frequent arrhythmias in patients with hypertrophic cardiomyopathy (HCM). Representing a major risk factor for sudden cardiac death, Holter ECG at first clinical presentation appears insufficient. This study aims to investigate the ability of routinely obtained parameters associated with myocardial remodeling in stratifying for VTA in HCM. In this monocentric analysis, patients with HCM underwent 12-channel electrocardiography and echocardiography, including tissue doppler imaging. The study's primary endpoint was the documentation of non-sustained and sust

Molecular Implications of Natriuretic Peptides in the Protection from Hypertension and Target Organ Damage Development.

The pathogenesis of hypertension, as a multifactorial trait, is complex. High blood pressure levels, in turn, concur with the development of cardiovascular damage. Abnormalities of several neurohormonal mechanisms controlling blood pressure homeostasis and cardiovascular remodeling can contribute to these pathological conditions. The natriuretic peptide (NP) family (including ANP (atrial natriuretic peptide), BNP (brain natriuretic peptide), and CNP (C-type natriuretic peptide)), the NP receptors (NPRA, NPRB, and NPRC), and the related protease convertases (furin, corin, and PCSK6) constitute

Cardiotoxicity of Uremic Toxins: A Driver of Cardiorenal Syndrome.

Cardiovascular disease (CVD) is highly prevalent in the setting of chronic kidney disease (CKD). Such coexistence of CVD and CKD-the so-called "cardiorenal or renocardiac syndrome"-contributes to exponentially increased risk of cardiovascular (CV) mortality. Uremic cardiomyopathy is a characteristic cardiac pathology commonly found in CKD. CKD patients are also predisposed to heart rhythm disorders especially atrial fibrillation. Traditional CV risk factors as well as known CKD-associated CV risk factors such as anemia are insufficient to explain CV complications in the CKD population. Accumul

Links between Vitamin D Deficiency and Cardiovascular Diseases.

The aim of the present paper was to review the most important mechanisms explaining the possible association of vitamin D deficiency and cardiovascular diseases, focusing on recent experimental and clinical data. Low vitamin D levels favor atherosclerosis enabling vascular inflammation, endothelial dysfunction, formation of foam cells, and proliferation of smooth muscle cells. The antihypertensive properties of vitamin D include suppression of the renin-angiotensin-aldosterone system, renoprotective effects, direct effects on endothelial cells and calcium metabolism, inhibition of growth of va

Gestational hypertension and the developmental origins of cardiac hypertrophy and diastolic dysfunction.

The developmental origins of health and disease refer to the theory that adverse maternal environments influence fetal development and the risk of cardiovascular disease in adulthood. We used the chronically hypertensive atrial natriuretic peptide knockout (ANP-/-) mouse as a model of gestational hypertension, and attempted to determine the effect of gestational hypertension on left ventricular (LV) structure and function in adult offspring. We crossed normotensive ANP+/+ females with ANP-/- males (yielding ANP+/-(WT) offspring) and hypertensive ANP-/- females with ANP+/+ males (yielding ANP+/

Left atrial ligation alters intracardiac flow patterns and the biomechanical landscape in the chick embryo.

BACKGROUND: Hypoplastic left heart syndrome (HLHS) is a major human congenital heart defect that results in single ventricle physiology and high mortality. Clinical data indicate that intracardiac blood flow patterns during cardiac morphogenesis are a significant etiology. We used the left atrial ligation (LAL) model in the chick embryo to test the hypothesis that LAL immediately alters intracardiac flow streams and the biomechanical environment, preceding morphologic and structural defects observed in HLHS. RESULTS: Using fluorescent dye injections, we found that intracardiac flow patterns f

Role for staphylococci in misguided thrombus resolution of chronic thromboembolic pulmonary hypertension.

OBJECTIVE: Acute pulmonary emboli usually resolve within 6 months. However, in 0.1% to 3.8% of cases thrombus transforms into fibrous masses. If vascular obstruction is severe, the resulting condition is chronic thromboembolic pulmonary hypertension (CTEPH). Patients who carry ventriculo-atrial (VA-) shunts for the treatment of hydrocephalus and report a history of shunt infection are at an increased risk for CTEPH. Because CTEPH lacks traditional plasmatic risk factors for venous thromboembolism, we hypothesized that delayed thrombus resolution rather than abnormal coagulation is important, a

Effect of optimisation to contemporary HFrEF medical therapy with sacubitril/valsartan (Entresto) and dapaglifloziN on left Ventricular reverse remodelling as demonstrated by cardiac magnetic resonance (CMR) Imaging: the ENVI study.

INTRODUCTION: Heart failure with reduced ejection fraction (HFrEF) guidelines recommend 'four pillars' of medical therapy and device therapy if left ventricular ejection fraction (LVEF) remains ≤35% after 3 months optimum medical therapy. We conducted the first study to examine the effects of optimisation to contemporary medical therapy on cardiac reverse remodelling, as demonstrated by cardiac magnetic resonance imaging (CMR).We hypothesised a proportion of patients would undergo beneficial remodelling and LVEF improvement above the threshold for complex device prescription after 6 months. Aortic valve replacement for aortic stenosis in patients with concomitant mitral regurgitation: should the mitral valve be dealt with?

Co-existent mitral regurgitation may adversely influence both morbidity and mortality in patients undergoing aortic valve replacement for severe aortic stenosis. Whilst it is accepted that concomitant mitral intervention is required in severe, symptomatic mitral regurgitation, in cases of mild-moderate non-structural mitral regurgitation, improvement may be seen following aortic valve replacement alone, avoiding the increased risk of double-valve surgery. The exact benefit of such a conservative approach is, however, yet to be adequately quantified. We performed a systematic literature review
