Complete atrioventricular dissociation and His bundle tachycardia in a newborn: problems in prenatal diagnosis and postnatal management.

A newborn infant with complete atrioventricular dissociation and infranodal tachycardia, detected at 33 weeks gestation by fetal echocardiography, is described. In the perinatal period, infra or juxta-nodal tachycardia was noted, compromising the hemodynamic state of the newborn. A combination of flecainide and propranolol terminated the arrhythmia.

Contrastive effects of prostaglandin F2 alpha on normal cardiac rhythm and ouabain-induced cardiac arrhythmias in cats: possible neural basis.

The effects of prostaglandin F2 alpha (PGF2 alpha) on normal cardiac rhythm and ouabain-induced cardiac arrhythmias were investigated in chloralose-anaesthetized cats. PGF2 alpha (1-16 micrograms/kg i.v. bolus) produced ventricular arrhythmias and few incidences of AV conduction disturbances in normal cats. Changes in heart rate and blood pressure caused by PGF2 alpha in normal cats were complex, namely a decrease, an increase, or an initial decrease followed by an increase. Bilateral vagotomy antagonized the ventricular arrhythmias, AV conduction disturbances and hemodynamic changes produced

An implantable antitachycardia pacemaker with back-up pacing and scanning burst mode.

We used an automatically triggered antitachycardia device (PASAR 4171) which is able to provide up to 7 stimuli in the atrium of three patients with AV-nodal tachycardia. A PASAR 4172 was implanted in a patient with WPW which gave two stimuli in the right ventricle. One revision was needed due to exit block. Temporary undersensing occurred in one patient. Atrial fibrillation occurred for the first time postoperatively in the WPW patient, without major side-effects. Treatment with low dosage amiodarone was initiated to prevent recurrence. At follow-up, magnet application was useful to induce su

Spontaneous termination of paroxysmal supraventricular tachycardia following disappearance of bundle branch block ipsilateral to a concealed atrioventricular accessory pathway: the role of autonomic tone in tachycardia diagnosis.

We present a case of an 18-year-old man with a history of palpitations in whom episodes of paroxysmal supraventricular tachycardia were easily initiated by administered atrial premature beats. In all 15 control episodes of tachycardia, functional left bundle branch block (LBBB) seen at the onset, resolved within 10-20 cycles (mean, 13.1 +/- 0.95). The tachycardia ended with the normalized QRS complex in each episode. Eleven episodes ended because of block within the antegrade pathway (ended with a P-wave), and four episodes stopped because of block within the retrograde pathway (ended without

Termination of paroxysmal supraventricular tachycardia with a single oral dose of diltiazem and propranolol.

The efficacy of a single oral dose combination of 120 mg diltiazem and 160 mg propranolol in terminating paroxysmal supraventricular tachycardia (PSVT) was evaluated in 15 patients. All 15 patients underwent electrical induction of PSVT that lasted longer than 15 min, and all underwent randomized crossover placebo and diltiazem and propranolol studies on 2 consecutive days. On each day PSVT was induced and placebo or diltiazem and propranolol was administered 15 min later. Electrical conversion of PSVT was performed when severe symptoms occurred or at the end of 240 min. With placebo PSVT last

Intrahisian functional bundle branch block.

A patient with functional complete left bundle branch block is presented. The site of block was localized to the area of the His bundle. This case demonstrates that functional complete left bundle branch block may be due to longitudinal dissociation in the His bundle.

Pharmacological basis for the therapeutic applications of slow-channel blocking drugs.

Excitation-contraction coupling in cardiac muscle as well as in smooth muscle is mediated by the transmembrane fluxes of calcium. In the case of cardiac muscle, this transfer occurs through the slow-inward channel. Agents that selectively inhibit the myocardial slow-channel also block calcium entry in smooth muscle cells, particularly in arteries. Thus, such selective inhibitors of the slow channel, exemplified by verapamil, nifedipine, and diltiazem, produce a marked negative inotropic effect in cardiac muscle; in whole animals or in man, such a propensity is largely nullified or even reverse

A His bundle electrocardiographic analysis of cardiac conduction in the pediatric and adolescent patient.

Bipolar electrode catheter recordings of His bundle electrograms with three simultaneously recorded surface electrocardiographic leads were obtained from 30 pediatric and adolescent patients (aged 3 to 18 years). In 14 patients, cardiac murmurs were proved to be innocent by cardiac catheterization. The control conduction intervals were compared to those of 13 patients with congenital heart disease, and three with acquired heart disease (myocardiopathy, rheumatic valvular disease, and Friedreich's ataxia). P-R, intra-atrial (P-A), A-V nodal (A-H), and intraventricular (H-V) conduction intervals

Late-Onset Atrioventricular Block and Significant Conduction Disturbances After Cardiac Interventions in Children.

INTRODUCTION: Late-onset postoperative atrioventricular (AV) block is a rare condition with a reported incidence ranging between 0.3% and 0.7%, and it may be associated with high mortality rates (28%-100%). The aim of this study was to evaluate the clinical characteristics of pediatric patients who developed late AV block at our center. METHODS: In this retrospective case series, pediatric patients evaluated for permanent pacemaker implantation during the study period were retrospectively reviewed. Patients who developed late-onset (>1 month) atrioventricular block or another clinically signi

Curing reentrant atrial arrhythmias. Targeting protected zones of slow conduction by catheter ablation.

Radiofrequency catheter ablation has become the treatment of choice for paroxysmal supraventricular tachycardia involving dual atrioventricular nodes or an accessory pathway. For reentry confined to the atrium where the arrhythmia itself or the ventricular response cannot be controlled with drugs, catheter ablation of the His bundle is a treatment option, but requires implantation of a permanent pacemaker and does not restore normal rhythm. In the atria, important anatomic obstacles, such as the great veins and the ostium of the coronary sinus, interrupt the normal arrangement of myocardial fi…

Precision Use of Ivabradine in Contemporary HFrEF: From Heart Rate Reduction to Physiological Optimization.

AIMS: To reassess the contemporary role of ivabradine in heart failure with reduced ejection fraction and propose a precision-oriented framework for individualized heart rate management in the era of foundational guideline-directed medical therapy. METHODS AND RESULTS: Ivabradine selectively reduces sinus rate without direct negative inotropic effects, with established benefits in selected patients with heart failure with reduced ejection fraction and persistent sinus tachycardia. However, recent meta-analyses differ substantially in study populations, endpoint availability, and background th

Sinus arrest in a patient with acute coronary syndrome due to a rare anatomical variation in the sinoatrial nodal artery.

BACKGROUND: Sinus node dysfunction can occur in patients with acute coronary syndrome (ACS), particularly in inferior myocardial infarction. Because the sinoatrial nodal artery (SANa) shows considerable anatomical variation, ischemia-related sinus arrest may occur depending on the origin and course of the artery. We report a case of sinus arrest associated with distal right coronary artery occlusion involving a rare SANa origin from the posterolateral branch. CASE PRESENTATION: A 59-year-old man presented with chest discomfort and marked bradycardia. Electrocardiography demonstrated sinus arr

GPER, 8-isoprostaglandin, and tau protein as potential biomarkers in supraventricular tachycardia: An observational study.

Supraventricular tachycardia (SVT) is a common arrhythmic disorder characterized by abnormal electrical impulses arising from supraventricular tissues, most commonly the atria or atrioventricular node. This study aimed to investigate the association between G protein-coupled estrogen receptor (GPER) levels and biomarkers of oxidative stress and neurodegeneration in the pathophysiology of SVT. This study included 51 patients diagnosed with SVT and 30 healthy controls. Plasma levels of the oxidative stress marker 8-iso-prostaglandin F2α (8-iso-PGF2α), the neurodegeneration-associated protein Tau…

A Novel Approach to Explore Internal Cardiac Electrophysiological Pattern under Emotional Stress.

Numerous psychological and clinical studies have confirmed a correlation between mental and cardiac health. We aim to explore this relations … hat fluctuations in cardiac electrical activity during stress are primarily concentrated around the atrioventricular node and Purkinje fibers. This suggests a potential risk for pre-excitation syndrome, possibly due to the possible presence of a Kent bundle. Overall, we present a practical approach for studying the interplay between emotional states and cardiac health. …

Case Report: Multimodality evaluation and clinical management of a single coronary artery.

A 14-year-old male with no significant medical history presented with intermittent palpitations for 2-3 months that occurred at rest and were associated with light-headedness. Electrocardiogram in clinic showed sinus arrhythmia with early repolarization and no ischemic changes. The echocardiogram showed normal cardiac structure and function, however, there was a concern for possible anomalous origin of the left coronary artery. Contrast-enhanced CT coronary artery angiogram confirmed a single coronary origin from the right coronary sinus. The single main coronary artery gave rise to the right

Retrievable dual-chamber leadless pacemaker implant (Aveir DR) in an adult patient with congenital heart disease.

Leadless pacemakers have demonstrated potential as a transvenous pacing option in Adult Congenital Heart Disease patients. Aveir™ single-chamber (VR) leadless pacemakers have demonstrated safety in patients without congenital heart disease in a dual chamber approach. We present a case of dual-chamber pacing using the Aveir dual-chamber (DR) leadless pacemaker in a patient with repaired dextro-transposition of the great arteries with ventricular septal defect (VSD) surgical closure. A 26-year-old male patient with a history of transposition of the great arteries status post arterial switch and
