Hypersensitivity of the oral mucosa: clinics and pathology.

Hypersensitivity reactions of the oral mucosa comprise an array of clinical manifestations. Some of the reactions are difficult to differentiate from toxic reactions. Hypersensitivity reactions of type I, type III, and type IV are well known, although, especially for types I and III, they are rarely encountered. Type-I reactions are most frequently seen related to application of polymers in the oral cavity, such as orthodontic bonding and fissure sealant materials. There may also be systemic manifestations such as urticaria. Type-IV reactions may be seen related to most dental materials used,

[Trypanosomiasis presenting with trypanids and complicated by myopericarditis (author's transl)].

In a 26-year-old man who had spent time in a trypanosome endemic zone, lesions of erythema marginatum progressing by acute exacerbations for 2 years despite various forms of treatment led to the discovery of a multiple lymphadenopathy and a greatly accelerated sedimentation rate. These findings suggested a diagnosis which was confirmed by the discovery of Trypanosoma gambiense in the blood and lymph node aspirate. In addition, there was an albumino-lymphocytic reaction in the cerebrospinal fluid. Finally, an atrioventricular block was discovered. All these symptoms and signs responsed to treat

Acute Hemorrhagic Edema in Infants: A Continuing Clinical Challenge.

We report a case of acute hemorrhagic edema in a four-month-old boy, which occurred following a viral respiratory infection seven days prior … dema. The differential diagnosis includes hemorrhagic urticaria, other non-infectious vasculitides, erythema multiforme, Kawasaki disease, infectious eczema, and sepsis (meningococcal or otherwise). Some of these conditions carry a risk of mortality. Management is based primarily on symptomatic treatment, with a favorable prognosis.

Smallpox vaccine complications: the dermatologist's role in diagnosis and management.

In 2002, the United States implemented a new program for smallpox vaccinations among military personnel using a live vaccinia virus product. Approximately 2.4 million US military service members and health care workers have since been inoculated, with considerable numbers experiencing adverse reactions. Military dermatologists are at the forefront of describing and treating these reactions, from relatively benign generalized vaccinia (GV) and erythema multiforme (EM) to more severe progressive vaccinia (PV) and eczema vaccinatum (EV). A wide range of providers, including civilian dermatologist…

[Adverse cutaneous reactions induced by exposure to woods].

Various adverse cutaneous reactions may occur as a result of exposure to wood dust or solid woods. These include allergic contact dermatitis, irritant contact dermatitis and, more rarely, contact urticaria, photoallergic and phototoxic reactions. Also cases of erythema multiforme-like reactions have been reported. Contact dermatitis, both allergic and irritant, is most frequently provoked by exotic woods, e.g. wood of the Dalbergia spp., Machaerium scleroxylon or Tectona grandis. Cutaneous reactions are usually associated with manual or machine woodworking, in occupational setting or as a hobb…

Revisiting thalidomide: fighting with caution against idiopathic pulmonary fibrosis.

Thalidomide is an infamous drug whose use by pregnant women in the middle of last century tragically resulted in serious birth defects. However, as a result of its potent immunomodulatory, anti-inflammatory and antiangiogenic properties, thalidomide may be a potential therapy in many diseases. In recent years, thalidomide has been used effectively to treat various malignancies, including multiple myeloma, myelodysplastic syndromes, renal cell cancer, glioblastoma multiforme and prostate cancer. In addition, thalidomide has also proven effective against other immune-related diseases, including

Clinical study of cutaneous drug eruptions in 200 patients.

Two hundred patients (112 males and 88 females) with cutaneous drug eruption were studied. The aim was to recognize the offending drug, to evaluate mortality and morbidity, educate the patient and avoid self-administration and readministration of drugs. Fixed drug eruption was the commonest reaction, seen in 61 patients; other reactions being urticaria and angioedema, morbilliform rash in 37, pruritus in 25, Stevens Johnson Syndrome (SJS) in 6, purpura in 6, exfoliative dermatitis in 5,photosensitivity in 5, toxic epidermal necrolysis in 2, acneiform eruption in 3, erythema multiforme in 2. Expression of Fas antigen on keratinocytes in vivo and induction of apoptosis in cultured keratinocytes.

Fas antigen, which belongs to a nerve growth factor/tumor necrosis factor receptor superfamily, is a membrane protein that induces apoptosis. In humans, distribution of Fas antigen has been reported on cell lines and lymphocytes. Immunohistochemical studies revealed Fas antigen on the keratinocytes of lesional epidermis in lichenoid drug eruption, erythema multiforme, contact dermatitis, bullous pemphigoid, pemphigus vulgaris, and herpes zoster; it is co-expressed with intercellular adhesion molecule-1. Cultured keratinocytes expressing Fas antigen increased from 8.4% to 34.6% after stimulatio…
