Value of a lateral inferior pedicle flap in Draf IIb for recurrent frontal sinus diseases: a prospective study.

PURPOSE: The Draf IIb procedure allows the widest unilateral access to the frontal sinus in a minimally invasive fashion, with efficiency and safety comparable to the Draf III. However, this technique is still associated with a high postoperative stenosis rate. The exposure of drilled bone induces osteitis predisposing to scarring and neo-osteogenesis causing ostium restenosis. We developed a novel lateral inferior pedicle flap (LIPF) to cover the exposed bone and prevent restenosis during Draf IIb. We aimed to describe our technique. METHODS: Adult patients requiring a Draf IIb for unilatera

Comparative analysis of the outcome of external and endoscopic frontal sinus surgery in children.

Background: Both open and functional endoscopic sinus surgery (FESS) are performed in the case of pediatric frontal rhinosinusitis. However, data from comparative analysis of these surgery types are insufficient. Objective: Prospective randomized trial for comparison of open and endoscopic surgery outcome in pediatric chronic rhinosinusitis.Material and methods: The cohort included 30 pediatric patients (7-17 years) with open frontal sinus surgery and 34 patients who underwent FESS using DrafIIa. Draf IIB with superior septectomy: finding the "middle ground".

BACKGROUND: Previous studies have demonstrated a high failure rate of endoscopic sinus surgery (ESS) with Draf IIa in patients with diffuse polyposis, asthma, and aspirin-exacerbated respiratory disease. A high percentage of these patients progress to endoscopic modified Lothrop procedure (EMLP). We describe a modification of the Draf IIb with a superior septectomy (IIb+SS), which may provide similar therapeutic benefit as demonstrated by the distribution of sinus irrigations in the sinus cavity with ESS with IIb+SS vs ESS with EMLP. METHODS: ESS with IIb+SS was performed on 6 cadaver heads.

Long-term results with nasofrontal duct reconstruction.

The findings and results of 50 consecutive nasofrontal duct reconstructions are analyzed and presented. Postoperative recurrence of frontal sinus disease occurred in 14% of the cases and there were no significant postoperative or intraoperative complications. There was no instance of failure in patients with a history of trauma to the nasofrontal duct and only two recurrences in patients with mucocele or pyocele. Nasofrontal duct reconstruction offers more direct access to the ethmoid cell system than osteoplastic flap obliteration. In addition, it is a lesser procedure with fewer complication

Densitometric studies on the shadows of paranasal sinuses in sagittal projection.

In taking a X-ray photography of the head in sagittal projection, it is most important that the center of the X-ray beam passes exactly through the median-sagittal line. If the subject turns his head either to the right or to the left, the paranasal sinuses become denser on one side, leading to erroneous interpretation of the radiographic findings. In order to eliminate such a misinterpretations, studies were carried out on the following: 1) Application of a standard of what degree of head turning to either side would start to influence judgment of the shadow of the paranasal sinuses. 2. Appli

[Clinical picture of single and multiple rhinogenic brain abscesses].

The authors examined 11 patients with rhinogenous abscesses of the brain aged from 2 to 45, 5 of them with multiple abscesses. According to their data the infection penetrated into the cranial cavity by two principal ways, namely, contactual (in 2 patients) and hematogenous via venous passages (in 4 patients). Contactual cerebral abscesses of rhinogenous genesis more frequently appear during frontitis, have a lingering chronic course and localize at the pole of the frontal lobe. Clinically they manifest themselves by focal and total cerebral symptoms including craniographic changes of hyperten

The outside-in approach to the modified endoscopic Lothrop procedure.

OBJECTIVES/HYPOTHESIS: Drilling in modified endoscopic Lothrop procedure (MELP) is traditionally described as commencing from the frontal recess (FR). This is challenging when the FR is involved by tumor, inflammatory disease, or scarring. The outside-in MELP, where the limits of the sinusotomy are first defined and the FR is addressed last, is described. STUDY DESIGN: Case-control study. METHODS: Patients undergoing MELP, using the standard or outside-in approach, for inflammatory disease or endoscopic skull base surgery were assessed. Data were collected on demographics, disease characteri

Equivalence in outcomes between Draf 2B vs Draf 3 frontal sinusotomy for refractory chronic frontal rhinosinusitis.

BACKGROUND: Endoscopic Draf 2B and Draf 3 frontal sinusotomies are frequently performed for chronic refractory frontal rhinosinusitis. The purpose of this study was to compare outcomes between Draf 2B and Draf 3 procedures. METHODS: A retrospective cohort study was conducted comparing patients undergoing bilateral Draf 2B vs Draf 3 procedures from 2000 to 2016. Patients with neoplasia, dysplasia, mucocele, cystic fibrosis, or ciliary dyskinesia were excluded. Preoperative disease parameters included number of prior surgeries, presence of polyps, preoperative 22-item Sino-Nasal Outcome Test (S

Frontal sinus surgery and sinus distribution of nasal irrigation.

BACKGROUND: Effective mucus lavage and delivery of topical pharmaceuticals are central to successful management of chronic rhinosinusitis (CRS). The frontal sinus remains difficult to penetrate with topical therapies. This study evaluates the benefit of Draf III frontal dissection compared to traditional Draf IIa for distribution of topical therapies. METHODS: Fresh human cadaver heads were dissected sequentially with Draf IIa frontal sinusotomy and then Draf III procedures. Each cavity was irrigated with pediatric (120 mL) and adult (240 mL) irrigation bottles with 1/1000 10% fluorescein-lab

[Evaluation of the curative effect of functional endoscopic sinus surgery by acoustic rhinometry].

OBJECTIVE: To evaluate the curative effect of Functional endoscopic sinus surgery (FESS) who suffered with chronic rhinosinusitis by acoustic rhinometry. METHOD: We collected 60 patients who accepted FESS for chronic rhinosinusitis, and calculated the mean minimal cross-sectional area (MCSA), nasal volume (NV), nasal airway resistance (NAR) by acoustic rhinometry within endoscopy to estimate the curative effect of FESS. RESULT: Four weeks after FESS, the patients' ethmoid sinus, maxillary sinus, frontal sinus and sphenoid sinus were clear by endoscopy. Meanwhile, the mean MCSA and NV by acoust

Indications for external frontal sinus procedures for inflammatory sinus disease.

BACKGROUND: In the modern age of endoscopic sinus surgery (ESS), there is an undefined role for external approaches in the treatment of inflammatory disease. This study examines the frontal sinus surgery practices of three experienced rhinologists with a focus on those who underwent an external approach. Our goal was to characterize these patients and propose indications for the use of an external approach alone or in combination with functional ESS (FESS) for frontal sinus inflammatory disease. METHODS: A retrospective review was performed of frontal sinus procedures performed for inflammato

Intraorbital subperiosteal hematoma due to paranasal mucocele--case report.

A 68-year-old female was admitted with a rare intraorbital subperiosteal hematoma manifesting as sudden orbital pain, progressive blepharoptosis, and diplopia. Computed tomography showed a biconvex high density mass in the superomedial part of the left orbit, which was recognized to be a subperiosteal hematoma intraoperatively. The eroded orbital roof between the frontal sinus and orbit was a result of mechanical compression by the mucocele. Infection extending into the orbit was important in causing the hemorrhage.

Antibacterial treatment of otitis and sinusitis with ciprofloxacin and penicillin V--a comparison.

80 adult outpatients suffering from otitis media, sinusitis (maxillaris or frontalis) or peritonsillitis were treated with 500 mg ciprofloxacin b.i.d. (n = 40) or with 2 g penicillin V t.i.d. (n = 40). The patients were randomly allocated. Three groups of patients resulted, clinically and bacteriologically evaluable, or only clinically evaluable because the isolated strains were resistant to ciprofloxacin or penicillin V or because no strains were isolated. Ciprofloxacin was superior to penicillin V; there were fewer resistant strains (one compared to 11), and the eradication rate (57% compare
