[Arthroscopic stabilisation of post-traumatic ventral instability of the shoulder by use of bioknotless anchors].

PURPOSE OF THE STUDY: Post-traumatic ventral shoulder instability is a frequent disorder resulting from ventral and caudal dislocation of the humeral head. The aim of this retrospective study is to evaluate the long-term results of arthroscopy-assisted reinsertion of the ventral labrum using absorbable bioknotless suture anchors. MATERIAL AND METHODS: A group of 129 patients with unidirectional ventral instability of the shoulder was evaluated for the results of treatment involving labrum reinsertion with use of absorbable bioknotless anchors. This arthroscopy-assisted method is performed fro…

Abnormal translation in SLAP lesions on magnetic resonance imaging abducted externally rotated view.

PURPOSE: The purpose of this study was to measure in vivo axial-plane translation of the glenohumeral joint by use of magnetic resonance ima … comparison group, most of whom had rotator cuff pathology. The glenohumeral contact point (CP) and humeral head center (HHC) were calculated and compared with the glenoid surface as a relative anterior or posterior translation. The relative posterior translation between the ABER and AD views for each patient was calculated as Delta CP and Delta HHC. These val …

Comparison of the ultrasonographic appearance of osteochondrosislesions in the canine shoulder with radiography, arthrography, and arthroscopy.

Osteochondrosis lesions in 29 shoulder joints (from 20 dogs) were evaluated with ultrasound (US) and the results were compared with survey radiography, arthrography, and arthroscopy. US was performed with a 7-12 MHz linear matrix transducer which was placed in cranio-caudal direction just distally to the acromion while the joint was adducted and maximally endorotated to visualize the caudal aspect of the humeral head. With US, the subchondral defect was completely visible in 21 joints and partially visible in 8 joints. The length of the subchondral defect measured on US was comparable with the…

Shoulder arthroplasty for proximal humeral malunions: long-term results.

Between 1976 and 1997, 50 shoulders with proximal humeral malunions in 50 patients were treated with hemiarthroplasty or total shoulder arthroplasty and followed up for a mean of 9 years (range, 2-21 years) or until the time of revision surgery. Of these, 13 had a 4-part malunion, 24 had a 3-part greater tuberosity malunion, 6 had a 2-part greater tuberosity malunion, and 7 had a 2-part head segment malunion. Articular incongruity resulted from an articular surface step-off in 5 shoulders, from osteonecrosis in 19, and from secondary degenerative arthritis in 26. Shoulder arthroplasty resulted

Trabecular microstructure and surface changes in the greater tuberosity in rotator cuff tears.

OBJECTIVE: When planning surgery in patients with rotator cuff tear, strength of bone at the tendon insertion and trabecular bone structure in the greater tuberosity are usually taken into consideration. We investigated radiographic changes in bone structure of the greater tuberosity in rotator cuff tears. DESIGN: Twenty-two human cadaveric shoulders from subjects ranging from 55 to 75 years of age were obtained. The integrity of the rotator cuff was examined by sonography to determine if it is intact without any tear, or torn partially or completely. The humeral head was sectioned in 3 mm th…

Arthroscopic-assisted rotator cuff repair: patient selection and treatment outcome.

Over a 4-year period 24 patients out of 376 who required a rotator cuff repair were selected for arthroscopic-assisted rotator cuff repair. Preoperative selection criteria were refractory pain in the setting of good range of motion and strength (after an impingement test), absence of radiographic superior humeral head translation, and magnetic resonance imaging evidence of minimally retracted tear without rotator cuff muscle atrophy. Intraoperative selection criteria were the findings of an avulsion-type tear configuration with good tendon quality and absence of subscapularis tendon involvemen…

Traumatic Anterior Shoulder Dislocation: Epidemiology, Diagnosis, and Treatment.

BACKGROUND: Traumatic anterior shoulder dislocation is the most common type of joint dislocation, with an incidence of 11 to 29 per 100 000 … nt instability after follow-up periods of up to 12 years. An accompanying bony injury of the dorsal humeral head (Hill-Sachs lesion), depending on its extent, may be an indication for the arthroscopic Remplissage procedure: this can lower the rate of recurrence, but it can also cause a postoperative restriction of external rotation. In cases of chronic instabi …

Biomechanical analysis of the effect of congruence, depth and radius on the stability ratio of a simplistic 'ball-and-socket' joint model.

OBJECTIVES: The bony shoulder stability ratio (BSSR) allows for quantification of the bony stabilisers in vivo. We aimed to biomechanically validate the BSSR, determine whether joint incongruence affects the stability ratio (SR) of a shoulder model, and determine the correct parameters (glenoid concavity versus humeral head radius) for calculation of the BSSR in vivo. METHODS: Four polyethylene balls (radii: 19.1 mm to 38.1 mm) were used to mould four fitting sockets in four different depths (3.2 mm to 19.1mm). The SR was measured in biomechanical congruent and incongruent experimental series…

Arthroscopic partial repair of irreparable large to massive rotator cuff tears.

PURPOSE: The aim of this study was to evaluate the outcome of arthroscopic partial repair and margin convergence of irreparable large to massive rotator cuff tears. METHODS: Between January 2003 and July 2008, 27 patients who met the inclusion criteria underwent arthroscopic partial repair and margin convergence of irreparable large to massive rotator cuff tears. An irreparable tear was defined as a tear with a minimum anterior-to-posterior width of 3 cm or larger, where it was not feasible to completely cover the humeral head with the cuff at the time of surgery. RESULTS: The mean preoperat…

The treatment of upper limb fractures in children and adolescents.

BACKGROUND: The treatment of fractures in children and adolescents must be based on an adequate knowledge of the physiology of the growing skeleton. Treatment failures usually do not result from technical deficiencies, but rather from a misunderstanding of the special considerations applying to the treatment of fractures in this age group. METHODS: We selectively reviewed recent publications on the main types of long bone fracture occurring in the period of skeletal development. RESULTS: Alleviating pain is the first step in fracture management, and due attention must be paid to any evidence

Additive fiber-cerclages in proximal humeral fractures stabilized by locking plates: no effect on fracture stabilization and rotator cuff function in human shoulder specimens.

BACKGROUND AND PURPOSE: The effect of additive fiber-cerclages in proximal humeral fractures stabilized by locking plates on fracture stabilization and rotator cuff function is unclear. Here it was assessed in a human cadaver study. METHODS: 24 paired human shoulder specimens were harvested from median 77-year-old (range 66-85) female donors. An unstable 3-part fracture model with an intact rotator cuff was developed. 1 specimen of each pair received an additive fiber-cerclage of the rotator cuff after plate fixation, and the other one received a plate fixation without an additive fiber-cercl

Recurrent anterior glenohumeral instability with onset after forty years of age: the role of the anterior mechanism.

Recurrent instability in patients over forty years of age is felt to occur primarily as a result of an associated rotator cuff tear. This is … psular redundancy, 1 had a small rotator cuff tear and 1 had an anterior capsular avulsion from the humeral head. At minimum follow-up of 32 months none of the patients reported episodes of instability. The reported incidence of rotator cuff tears in patients over the age of forty following an initial traumatic anterior glenohumeral dislocation ranges from 35% …
