Workable criteria are proposed that allow early recognition of mild forms of occult hydrocephalus and correlated disorders. Two groups of patients are analyzed, i.e., a group of twelve mild but classical forms of low-pressure hydrocephalus and another group of 20 patients with atypical clinical forms of disorders of the CSF dynamics. Two control groups are presented for neuropsychological comparison purposes: a normal control group of 50 subjects and a small group of three commissurotomized subjects. Patients with an overt demential syndrome due to a normotensive hydrocephalus syndrome were ex

OBJECTIVE: Serious medical conditions of patients in neurosurgery often require ventriculoperitoneal shunting to normalize the intracranial pressure. Neurosurgeons select a valve opening pressure (VOP), whose exceeding causes the shunt to open to drain cerebrospinal fluid (CSF): In most cases a standard pressure is chosen. Whereas some patients fare well with the primary chosen VOP, others require one to several VOP changes. This study aims to evaluate the prevalence and indication for occurring valve pressure-adjustments.
PATIENTS AND METHODS: We obtained information about 343 ventriculoperi

INTRODUCTION: Headache is usually considered a key symptom of intracranial hypertension (ICHT). However, there are no published experimental data to support the concept that increased intracranial pressure (ICP) is painful in humans.
MATERIALS AND METHODS: This prospective study was performed in 16 patients with suspected normal-pressure hydrocephalus, necessitating a lumbar infusion test with measurement of cerebrospinal fluid (CSF) hydrodynamics. During the test, ICP was increased from baseline to a plateau. Headache was scored on a visual analog scale (VAS) (0 = no pain, 10 = very severe p

As increasingly shown in neuropathological and predementia clinical studies, cognitive decline with altered intracranial dynamics can fulfill current clinical criteria of dementia of the Alzheimer's type (DAT) and there is a marked pathogenic complexity of this epidemic syndrome. Whereas structural studies only suggest the unexpected frequency of cerebrovascular changes in late life DAT, flow quantification MR sequences now offer a great opportunity of in vivo accurate analysis of cerebrovascular function. Their first applications have allowed development of a modern concept of the intracrania

Blake's pouch cyst is a congenital disease in which the ventricle enlargement is a non-communicating hydrocephalus. Some elderly patients have a process of the idiopathic normal pressure hydrocephalus (communicating hydrocephalus). We report an elderly patient with Blake's pouch cyst. A 68-year-old man visited our hospital with a gait disturbance that had begun 2 years previously but had become aggravated. He did not have dementia or urinary disturbance. CT scan and MRI showed the hydrocephalus included an IV ventricle, and the cerebrocerebellar fluid space in the posterior fossa was large. A

This is a report of two cases of optociliary shunt vessels in patients with neonatal hydrocephalus. Complete disappearance of the optociliary shunt vessels occurred after surgical procedures to normalize intracranial pressure. The clinicopathological significance, prognosis, and treatment of patients with acquired optociliary shunt vessels is discussed. This is also the first report of the association of optociliary shunt vessels and neonatal hydrocephalus.

Seven children studied because of clinical macrocephaly and suspected hydrocephalus ultimately proved to have megalencephaly apparently due to an increase in sagittal sinus venous pressure as established from infusion studies. Unexplainably, these patients were all males. All were seen initially between 2 and 8 months of age. Head enlargement exceeded two standard deviations in all seven. Pneumoencephalography, ventriculography, or computerized tomography demonstrated normal or minimally enlarged ventricles that did not progress in size. Isotope cisternography was abnormal. Studies of CSF form

Spontaneous intracranial hypotension (SIH) is characterized by severe postural headache and low cerebrospinal fluid (CSF) pressure. Radionuclide cisternography (RC) is of some value in diagnosing CSF leakage causing SIH. However, the sensitivity of RC is too low to demonstrate the site of leakage. In these cases, the early appearance of the radioactivity in the urinary bladder has also been used as an indirect finding in the diagnosis of SIH. The aim of this study was to evaluate the diagnostic reliability of early urinary bladder activity as an indirect sign of SIH. We investigated early blad

In a prospective series of symptomatic adult hydrocephalus characterized by gait disturbance, cognitive impairment, and/or urinary incontinence, 88 of 118 patients (75%) had additional akinetic, tremulous, hypertonic, or hyperkinetic movement disorders. Their prevalence was highest in patients with idiopathic normal pressure hydrocephalus (NPH) of the elderly (56/65 patients, 86%), and they were less frequent in patients with secondary NPH (10/15, 66%), with nonhydrodynamic atrophic/other hydrocephalus (20/33, 61%), and with obstructive hydrocephalus/aqueductal stenosis (2/5, 40%). Cortical oxygen utilization, oxygen extraction, blood flow, and blood volume, have been measured in patients with hydrocephalus before and after surgical decompression using positron emission tomography (PET). The hydrocephalus subjects fell into two categories: patients with recent-onset obstructive hydrocephalus and symptoms of raised intracranial pressure due to obstruction of cerebrospinal fluid (CSF) drainage by posterior fossa or third ventricular cerebral tumours, and patients with hydrocephalus of more insidious onset that was associated either with congenital abnormalities, or which w

We describe a case of intracranial hypotension syndrome due to overdrainage of cerebrospinal fluid presented with hearing loss after ventriculoperitoneal shunting procedure. A 69-year-old man suffering from subarachnoid hemorrhage presented with an angiogram showing two aneurysms, one of the right internal carotid and one of the middle cerebral artery. Neck clipping was performed. One month later, he developed normal pressure hydrocephalus (NPH), which was treated by ventriculoperitoneal (NPH), which was treated by ventriculoperitoneal shunting procedure using low pressure Pudenz system. CSF dynamics were determined as a function of intracranial pressure in patients with pseudotumor cerebri. Servocontrolled variable rate lumbar infusions were used to determine net CSF-absorptive capacities and resting pressures in 10 patients; serial studies were done in 5 of the patients. Nearly all of the patients had abnormally low CSF-absorptive capacities. On the other hand, marked elevations in resting pressure were not a constant feature of the disease. Concurrent changes in the cerebrovascular bed could introduce errors into this manometric determination of CSF dynamics; the significan

A total of 110 unselected demented outpatients aged 60 and over (mean age: 76.2 years), 69 women and 41 men, were prospectively investigated. A potentially reversible cause of dementia was found in 26 patients (normal pressure hydrocephalus 2, cerebral tumor 1, hyperthyroidism 2, hypothyroidism 4, vitamin B12 deficiency 13, pseudodementia 4). Follow-up investigation showed that the only true reversible dementia in which improvement from abnormal to normal mental status occurred and was sustained for up to 2 years was 1 case of normal pressure hydrocephalus and 1 case of pseudodementia. In all

Ventriculopleural shunts are uncommonly used for the treatment of normal pressure hydrocephalus in adults. Pleural effusion has been reported to complicate the course of these ventriculopleural shunts in children. The pleural effusion should typically resemble the cerebrospinal fluid unless frankly infected. There are few good data on the nature of the pleural effusion. We, report a case of recurrent right-sided pleural effusion, 2 years after a ventriculopleural shunt insertion, for normal pressure hydrocephalus with no evidence of an underlying infection. The effusion abated after ligation o

Twenty-nine patients consecutively admitted with a diagnosis of communicating hydrocephalus underwent continuous intracranial pressure (ICP) monitoring; pressure-volume studies; and measurement of resistance to outflow of cerebrospinal fluid (Rout). The two latter calculations were made by the bolus injection and pressure-volume index (PVI) techniques. In 19 patients mean ICP never exceeded 15 mm Hg. In the other 10 patients varying degrees of mildly raised ICP was noted. The frequency of waves at 1/2 to 2/min varied from 3% to 58%. The ICP pulse amplitude ranged from 0.5 to 10 mm Hg, and PVI

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a potentially reversible neurological condition characterized by cerebrospinal fluid (CSF) accumulation contributing to clinical symptoms. Removal of excess CSF can lead to improvement, and among the available approaches, external lumbar drainage (ELD) offers superior diagnostic accuracy. However, the optimal timing for patient evaluation following ELD remains unclear. METHODS: We recruited 54 adult iNPH patients, assessed at baseline (T0), 48 hours (T1), and 10 days (T2) after ELD. Balance performance was measured using clinical s

BACKGROUND: Disproportionately enlarged subarachnoid space hydrocephalus (DESH) findings on MRI were described as a prognostic factor for responsiveness to the treatment of idiopathic normal pressure hydrocephalus (iNPH). Our premise is that DESH could be associated with compression of the cerebral white matter. Microstructural changes can be identified using diffusion tensor imaging (DTI), specifically fractional anisotropy (FA). The aim of this study is to compare FA in iNPH patients with and without DESH and healthy controls.
METHODS: We analysed 1.5-T MRI scans of patients fulfilling the

OBJECTIVE: Diffusion tensor imaging (DTI) parameters were investigated in patients with chronic idiopathic hydrocephalus to evaluate microstructural changes of brain tissue caused by chronic ventricular dilatation.
METHODS: Eleven patients fulfilling the criteria for possible or probable idiopathic normal pressure hydrocephalus and 10 healthy control subjects underwent MRI at 3 Tesla, including DTI with 12 gradient directions. Patients were scanned before lumbar cerebrospinal fluid (CSF) withdrawal tests. Differences in fractional anisotropy (FA) and mean diffusivity (MD) between patients and
