Employment Status of Patients With Colorectal Cancer After Surgery: A Multicenter Prospective Cohort Study in Japan.

BACKGROUND: Because of improved survival rates, patients with colorectal cancer may try to return to work. Many countries, however, have limited knowledge of the employment status of these patients. OBJECTIVE: To explore the employment status of patients with colorectal cancer after surgery in Japan and the risk factors affecting the same. DESIGN: This is a prospective multicenter cohort study that used self-administered questionnaires. Patients were recruited from June 2019 to August 2020 and were followed up for 12 months after surgery. SETTING: Six community hospitals and 1 university ho

Prolapse phenotypes and 12-month postoperative prolapse recurrence after apical native tissue surgery: a combined analysis using multicenter randomized trials and registry data.

BACKGROUND: Recurrence after pelvic organ prolapse surgery is not uncommon, and recurrence rates differ based on baseline prolapse characteristics, such as genital hiatus size and prolapse stage. Better understanding the relationship between anatomic prolapse subgroups and surgical outcomes may allow surgeons to individualize treatment decisions and improve outcomes. Clinically relevant prolapse phenotypes (subgroups) defined using the Pelvic Organ Prolapse Quantification system were previously identified. Therefore, study associations between these phenotypes and surgical outcomes in a large,

Cementless Fixation Is Associated With Increased Risk of Early and All-Time Revision After Hemiarthroplasty But Not After THA for Femoral Neck Fracture: Results From the American Joint Replacement Registry.

BACKGROUND: Despite ample evidence supporting cemented femoral fixation for both hemiarthroplasty and THA for surgical treatment of displaced femoral neck fractures, cementless fixation is the preferred fixation method in the United States. To our knowledge, no nationally representative registry from the United States has compared revision rates by fixation for this surgical treatment. QUESTION/PURPOSE: After controlling for relevant confounding variables, is femoral fixation method (cemented or cementless) in hemiarthroplasty or THA for femoral neck fracture associated with a greater risk of

Outpatient Revision TKA Does Not Increase Incidence of Repeat Revision or Medical and Surgical Complications Compared With Inpatient Revision TKA.

BACKGROUND: The incidence of revision TKA is escalating. The safety and efficacy of performing revision TKA in an outpatient setting is important given this increased demand. QUESTIONS/PURPOSES: (1) Are patients who undergo revision TKA in an outpatient setting more likely to undergo a repeat revision within 1 year compared with patients undergoing revision TKA in an inpatient setting? (2) Are patients who undergo outpatient revision TKA more likely to have increased hospital readmissions, manipulation under anesthesia (MUA), and medical complications compared with patients undergoing revisio

Is intraoperative CT of posterior cervical spine instrumentation cost-effective and does it reduce complications?

BACKGROUND: Symptomatic multilevel cervical myelopathy is often addressed using posterior decompression using two-dimensional fluoroscopy. Intraoperative three-dimensional fluoroscopy provides more accurate information on the position of instrumentation to prevent screw-related complications. QUESTIONS/PURPOSES: We documented the incidence of hardware-related complications and evaluate cost-effectiveness when using intraoperative three-dimensional fluoroscopy (ISO-C CT) in posterior cervical spine surgery. METHODS: Records from 87 patients who underwent posterior cervical decompression and i

Rationale and design of the AlloFIST trial: a phase I/IIa study to evaluate dose escalation of allogeneic adipose-derived stroma/stem cells for the treatment of Crohn's fistula.

INTRODUCTION: Crohn's disease (CD) is a chronic inflammatory disorder of the gastrointestinal tract distinguished by progressive bowel damage with a risk of structuring and penetrating complications. It is characterised by focal or segmental transmural inflammation that disrupts intestinal mucosal integrity and favours the development of abscesses and fistulas. Perianal fistula develops in 13%-39% of patients with CD. Their care is difficult but improves with medical and surgical treatment to preserve anal continence and avoid a maximum proctectomy. Combined treatment with seton placement and

Follow-up non-attendance after long-bone fractures in a low-resource setting: a prospective study of predictors and interventions to increase attendance rates.

BACKGROUND: While the majority of traumatic injuries occur in low- and middle-income countries, the published literature comes chiefly from high-income countries due to poor follow-up. Clinical and radiographic post-surgical trauma follow-up is essential to high-quality research and objective monitoring for healing and/or complications. This study aimed to identify the predictors of follow-up non-attendance in a low-resource setting and investigate the extent to which interventional efforts based on mobile phone technology (MPT) and home visits improved the follow-up rates for fractures treate

Comparison of surgical payer costs and implication on the healthcare expenses between laparoscopic magnetic sphincter augmentation (MSA) and laparoscopic Nissen fundoplication (LNF) in a large healthcare system.

INTRODUCTION: Magnetic sphincter augmentation (MSA) is a promising antireflux surgical treatment. The cost associated with the device may be perceived as a drawback by payers, which may limit the adoption of this technique. There are limited data regarding the cost of MSA in the management of reflux disease. The aims of the study were to report the clinical outcome and quality of life measures in patients after MSA and to compare the pharmaceutical and procedure payer costs and the disease-related and overall expense of MSA compared to laparoscopic Nissen fundoplication (LNF) from a payer pers

Traumatic abdominal wall hernia diagnosed 14 years after a bad fall with lumbar spine fracture.

UNLABELLED: Traumatic abdominal wall hernias have significant implications for patients and insurance companies, especially when not been discovered at the time of trauma. We present the case of a gardener who sustained a bad fall during work in 1994 with immediate admission to the hospital for treatment. A fracture of the second lumbar spine body has been diagnosed and stabilized operatively. Postoperative computer tomography and magnetic resonance examinations demonstrated correct healing of the fracture. Neither the pain in the sacral spine, the left leg and left lower abdominal wall nor th

Considerations for physicians caring for older adults with periodontal disease.

Periodontal disease is a generic term encompassing a variety of inflammatory conditions affecting the supporting tissues of the teeth. Periodontitis is inflammation associated with net resorption of supporting alveolar bone and periodontal ligament. Gingivitis is inflammation limited to the covering gingival tissues and does not directly lead to tooth mobility or loss. Periodontal diseases are very prevalent. Because the bone and ligament resorption are essentially irreversible, accumulated tissue damage of periodontitis is reflected in a prevalence and severity that increase with age. Periodo

Failed back surgery syndrome.

The failed back or postlaminectomy syndrome is obviously multidimensional. Failure of therapy may result from structural abnormalities in the back, psychosocial influences, or a combination of both. The causes of back pain are largely unknown. Correlations with diagnostic studies are uncertain. The lack of precise diagnoses is reflected in a multiplicity of nonspecific treatments, mostly of unproven value. Our current disability-litigation system adds greatly to the problem. Patients are rewarded for nonfunction. Some physicians become advocates for patients, others for insurance carriers and
