Clinical evaluation of plasma abnormal prothrombin (des-gamma-carboxy prothrombin) in hepatobiliary malignancies and other diseases.

Des-gamma-carboxy prothrombin (DCP), a protein induced by vitamin K absence or antagonist-II (PIVKA-II) was measured in the plasma of patients with primary hepatocellular carcinoma and those with various other hepatobiliary and pancreatic diseases. DCP levels were determined by enzyme immunoassay (E-1023), using an anti-DCP monoclonal antibody. Forty-two of the 91 patients (46.2%) with hepatocellular carcinoma had abnormally elevated levels of DCP, whereas only one of the 24 patients with hepatic cirrhosis showed a slight increase. An increase was also observed in some patients with obstructiv

Choledochoduodenostomy for benign biliary tract disease in the elderly.

Endoscopic sphincterotomy is an available nonoperative alternative treatment for recurrent or residual common bile duct stones. Nevertheless, immediate, intraoperative and definitive treatment is needed in patients with common duct stones and other pathologies that are prone to develop into such complications, which is important especially in aged high-risk patients during recurrent operations and nonoperative instrumental interventions. To evaluate the safety of choledochoduodenostomy in elderly patients, we reviewed our experience with 55 patients over the age of 70 years with benign biliary

Carbohydrate metabolism in infants with biliary atresia.

Carbohydrate metabolism in 9 infants with biliary atresia (BA) was investigated preoperatively and during the early postoperative period when these infants were on parenteral nutrition for 7 days. Findings were compared with data on infants without liver dysfunction. Changes of plasma glucose, immunoreactive insulin (IRI) and C-peptide immunoreactivity (CPR) in the biliary atresia group with moderate liver fibrosis were the same as those in the control group. However, in one patient with marked liver fibrosis, the molar IRI/CPR ratio was significantly low, thereby suggesting an increased hepat

The stabilization of serum lipid emulsions by serum phospholipids.

Clarity of high lipid sera is closely correlated with elevated proportions of serum phospholipids, and lipemia (milkiness) with low proportions of phospholipids. Clear high lipid sera occur uniquely in obstructive jaundice, both intra- and extrahepatic in origin. Destruction of the polar nature of serum "lecithin" by enzymatic hydrolysis, using Cl. welchii lecithinase, results in a degree of lipemia which is linearly proportional to total lipid content in clear or lipemic high lipid or normal lipid sera. Even in grossly lipemic sera, a significant proportion of the serum lipids is masked in pa

Case Report: Hepatitis and autoimmune hemolytic anemia induced by EBV-associated infectious mononucleosis.

Epstein-Barr virus (EBV) typically causes infectious mononucleosis, but in rare cases, it may lead to complications such as autoimmune hemolytic anemia (AIHA) and hepatitis. We report the case of an 18-year-old previously healthy female who presented with jaundice, red urine, and arthralgia. Laboratory workup revealed direct hyperbilirubinemia, elevated transaminases, and a direct antiglobulin test positive for IgG (negative for C3d), consistent with warm autoimmune hemolytic anemia in the context of EBV infection. Imaging ruled out biliary obstruction. The patient was managed conservatively,

Hepatic Hydatid Cyst Complicated by Biliary Obstruction: A Case Report.

A 35-year-old male, previously healthy, presented with epigastric and right hypochondrial pain, as well as a change in urine color. The diagnosis of a hepatic hydatid cyst with complications was confirmed based on clinical findings, ultrasound, and serology. The patient was treated with albendazole and underwent an ERCP (endoscopic retrograde cholangiopancreatography) for biliary involvement. Hydatid cysts are caused by Echinococcus granulosus, transmitted through contact with infected animals or contaminated food. The disease can lead to severe complications such as biliary obstruction, jaund

Acute Fatty Liver of Pregnancy Mimicking Puerperal Sepsis: Report of a Case with Literature Review.

Puerperal pyrexia is defined as fever in a woman within 6 weeks of giving birth, which is ≥38°C. We describe the clinical scenario of fever and progressive jaundice in a 32-year-old female in the postpartum period. The laboratory data showed normal leukocytic counts and all her septic screen was noncontributory. While in the hospital, her bilirubin levels progressively increased and showed a mild elevation of liver enzymes. There was no evidence of biliary obstruction. A liver biopsy clinched the diagnosis of acute fatty liver. Acute fatty liver of pregnancy is a rare but serious complication

Liver transplantation for metastases from a bile duct carcinoid.

BACKGROUND: A 29-year-old woman who presented with fatigue and jaundice was found to have an obstructing mass at the bifurcation of the bile duct. The patient underwent a successful left hepatectomy with resection of the bile duct bifurcation and a reconstruction with a right hepaticojejunostomy. Pathology revealed an atypical carcinoid tumour of the left extrahepatic bile duct, with perineural and lymphatic invasion. The patient subsequently developed multiple metastases in the remaining liver. METHODS: In the absence of extrahepatic disease, the patient underwent a successful liver transpla

Hepatolithiasis associated with anomalous biliary anatomy and a vascular compression. Hepatolithiasis and anomalous anatomy.

Biliary tract abnormalities occur in about one of every three people, usually being minor and of no clinical significance. Major abnormalities, however, may present in an unusual manner and provide a major hazard to the unsuspecting surgeon. A patient presenting with cholangitis without jaundice or abnormal liver function tests is reported. Endoscopic retrograde cholangiography failed to demonstrate any bile ducts in the right postero-lateral segments of the liver, the "naked segment sign". A percutaneous transhepatic cholangiogram demonstrated a stricture obstructing the right posterior segme

[5 cases of neonatal duodenal obstruction].

The authors report five cases of intrinsic duodenal obstruction in neonates, operated upon since 1976 to 1982. In 3 cases, it's a question of atresia and in two cases of annular pancreas. The associated lesions are in one case, a macrocephaly, in a second case, a microcephaly with ocular lesions and in a third case a jejunal atresia. The operating procedure consists four times in a duodeno-jejunostomy and one time in a duodeno-duodenostomy. A complementary jejunal resection with end-to-end anastomosis is necessary in the case of associated jejunal atresia. A discharge gastrostomy is performed

A case of gossypiboma mimicking intrahepatic cholangiocarcinoma.

INTRODUCTION: A gossypiboma refers to a cotton-based foreign body left inadvertently in the human body following a surgical procedure. Although a rare event, they tend to be found in the abdomen but few are known to be intrahepatic. CASE HISTORY: We report the case of a 44 year-old man who presented with recurrent episodes of jaundice and cholangitis, on a background of a right hepatectomy for hydatid cyst excision 20 years previously. This case was discussed at our hepatobiliary multidisciplinary team meetings on several occasions and a presumed diagnosis of intrahepatic cholangiocarcinoma w

Detection of reovirus RNA in hepatobiliary tissues from patients with extrahepatic biliary atresia and choledochal cysts.

Extrahepatic biliary atresia (EHBA) and choledochal cysts (CDC) are important causes of obstructive jaundice in pediatric patients. Viruses in general, and reoviruses in particular, have long been considered as possible etiologic agents responsible for inciting the inflammatory process that leads to these infantile obstructive cholangiopathies. In an effort to determine whether reovirus infection is associated with these disorders, we used a sensitive and specific reverse-transcriptase polymerase chain reaction (RT-PCR) technique designed to amplify a portion of the reovirus L1 gene segment fr

Case Report: A rare case of IgG4-related disease initially manifesting with diarrhea and jaundice.

BACKGROUND: IgG4-related disease (IgG4-RD) is a systemic immune-mediated fibrosing inflammatory disorder with multi-organ involvement. Due to highly variable and nonspecific clinical manifestations, atypical cases are prone to misdiagnosis and missed diagnosis. This study reports an IgG4-RD case presenting with rare initial symptoms of diarrhea and jaundice, aiming to enrich the clinical spectrum of atypical IgG4-RD and improve early differential diagnosis. CASE PRESENTATION: A 62-year-old male with well-controlled type 2 diabetes mellitus presented with 10 days of refractory mucoid diarrhea

High coincidence of Mirizzi syndrome and gallbladder carcinoma.

BACKGROUND: Mirizzi syndrome is a rare complication of long-standing cholelithiasis. It is defined as obstructive jaundice caused by external compression of the common hepatic duct by an impacted stone in the gallbladder neck. Gallstone disease and cholelithiasis-associated chronic biliary inflammation may play a causative role in the pathogenesis of gallbladder carcinoma. The purpose of this study was to investigate the coincidence of gallbladder carcinoma associated with Mirizzi syndrome. Furthermore, the diagnostic value of elevated CA 19-9 levels as indicator for a coincidental gallbladder

North American Indian cirrhosis in children: a review of 30 cases.

BACKGROUND: North American Indian childhood cirrhosis (NAIC) is a distinct, rapidly evolving form of familial cholestasis found in aboriginal children from northwestern Quebec. This is a retrospective review of the 30 patients treated in Quebec since the discovery of NAIC in 1970. METHODS: The clinical records and histologic samples from 30 patients were reviewed. Extensive metabolic, biochemical, viral, genetic, and radiologic studies were performed in most patients. RESULTS: Genetic analysis suggests autosomal recessive inheritance and a carrier frequency of 10% in this population. Gene ma

Extrahepatic biliary cystadenomas and cystadenocarcinoma. Report of seven cases and review of the literature.

OBJECTIVE: The aim of this investigation was to describe the clinical features, diagnosis, pathologic characteristics, and optimal surgical management for patients with extrahepatic biliary cystadenomas. SUMMARY BACKGROUND DATA: Extrahepatic biliary cystadenomas are rare epithelial neoplasms. The clinical features and optimal surgical management for these lesions have not been defined clearly. The usual presenting symptom is jaundice. These lesions should be considered premalignant and necessitate resection. Sporadic case studies have reported instances of recurrence with local excision. [Laparoscopic treatment of Mirizzi's syndrome].

The Mirizzi syndrome is an unusual benign obstructive jaundice due to extrinsic mechanical compression of the common hepatic duct by gallstone impacted within the neck or cystic duct of the gallbladder. This syndrome is described either as an acute form due only to extrinsic compression of the common bile duct (type I) or as a chronic form resulting in an erosive cholecysto-choledochal fistula (type II). Up to date, the syndrome remains a clinically and surgically challenging problem. The anatomic basic of the syndrome (an anomalous relationship between the cystic duct and the common hepatic d

The use of different pathology classification systems in preoperative imaging of Mirizzi syndrome.

INTRODUCTION: The aim was to evaluate the diagnostic value of the Nakagawa and Csendes pathology classification systems in preoperative imaging of Mirizzi syndrome. Mirizzi syndrome is a type of biliary system obstruction caused by stones impacted in a gallbladder neck or cystic duct situated parallel to the common bile duct, causing extrinsic common bile duct stenosis or obstruction, which can lead to recurrent obstructive jaundice, bile duct erosion, and cholangitis. Therefore, the preoperative identification and classification of Mirizzi syndrome is vital for a good surgical result. We expl
