French Innovations in Lower Limb arthroplasty that have Become "Gold Standards": an overview.

This paper aims at reviewing some of the major French innovations in lower limb arthroplasty. This achievement could happen only through substantial efforts from pioneers that understood they had to adhere to the general principles of research through well designed studies, robust evaluation tools, and long-term follow-up studies. One of the major inventions in total hip arthroplasty (THA) was dual mobility cups (DMC). The concept of DMC has clearly demonstrated clinical efficiency in the prevention and treatment of postoperative dislocation both in primary and revision THAs. Another landmark

Good 5-year results and a low redislocation rate using an à la carte treatment algorithm for patellofemoral instability in patients with severe trochlea dysplasia.

PURPOSE: Trochlear dysplasia is a major risk factor for recurrent patellar instability, reduced quality of life and osteoarthritis of the patellofemoral joint. Patellar instability in patients with trochlear dysplasia can be treated by trochleoplasty, usually in combination with medial patellofemoral ligament reconstruction (MPFL-R). An à la carte treatment algorithm, which also addresses patella alta, lateralisation of the tibial tuberosity and valgus or torsional malalignment when present has been standard in one clinic for treatment of patellar instability patients since 2009, based on the

Modified extramedullary femoral referencing is associated with improved femorotibial component alignment consistency and more stable mobile-bearing positional behavior in Oxford medial unicompartmental knee arthroplasty: a retrospective cohort study.

BACKGROUND: Mobile-bearing stability in Oxford medial unicompartmental knee arthroplasty (UKA) depends on maintaining a consistent femorotibial component relationship throughout knee motion. This study evaluated whether a modified extramedullary femoral referencing technique, designed to account for extension-phase alignment, was associated with reduced motion-related variation in femorotibial component position and more stable mobile-bearing positional behavior than the previously described extramedullary technique. METHODS: This single-center retrospective cohort study included 60 patients

Characteristics of patients with primary acute lateral patellar dislocation and their recovery within the first 6 months of injury.

We prospectively studied the characteristics and early recovery of an unselected population of patients who had acute first-time lateral patellar dislocation. The recovery program used standardized rehabilitation, emphasizing range of motion, muscle strength, and return of function. Patients returned to stressful activities including sports as tolerated when they regained full passive range of motion, had no effusion, and when quadriceps muscle strength was at least 80% compared with the noninjured limb. Seventy-four patients met the enrollment criteria; 37 men and 37 women. The average age wa

Jumper's knee.

The authors present a clinical study on twenty six cases of a painful syndrome affecting the distal quadriceps tendon complex, popularly known as "jumper's knee". They review the diagnostic aspects and discuss the treatment. In particular, they suggest a surgical approach that is not limited to the inflammatory or degenerative component of the syndrome, but also aims to correct imbalance of the extensor apparatus.

RHEUMATOID ARTHRITIS. PHYSICAL MEASURES IN TREATMENT OF CHILDREN.

Prognosis in rheumatic arthritis in children is good, provided total care is given, deformity prevented and function maintained. Bed rest is desirable until active inflammation of the joints has subsided. During convalescence a balance between rest and exercise must be maintained to avoid recurrence of inflammation of the joints. When there is progressive deformity or disabling pain in the wrist, a molded leather wrist-cuff splint can control the deformity, decrease or abolish pain and lessen swelling. If there is valgus deformity of the knee or external rotation of the tibia, with no more tha

Tibial Tuberosity Anteromedialization for Patellofemoral Chondral Disease: Prognostic Factors.

BACKGROUND: Tibial tuberosity anteromedialization (TTA) is a well-established treatment option for patellofemoral chondral disease that is resistant to nonoperative treatment. However, the prognostic factors of this procedure are unknown. PURPOSE: To analyze the prognostic factors correlated with the midterm outcomes of TTA for patellofemoral chondral disease and determine the survivorship. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Indications of TTA for chondral disease included skeletal maturity, age <65 years, <grade 3 Kellgren-Lawrence degeneration, and isolated patellof

Great white sighting: a case of heparin-induced thrombosis with thrombocytosis.

BACKGROUND: Heparin-induced thrombocytopenia (HIT) is an immune-mediated adverse response to heparin therapy, characterized by decreased platelet count and increased risk of thrombosis. HIT, without the tell-tale sign of thrombocytopenia, has rarely been described. KEY CLINICAL QUESTION: Can HIT be diagnosed in the presence of thrombocytosis? What clinical clues and diagnostic tools facilitate accurate diagnosis in such cases? CLINICAL APPROACH: We report a case of HIT with thrombocytosis in a young male who initially presented after traumatic knee dislocation. HIT was diagnosed clinically t…

Soleus nerve transfer to deep peroneal nerve for treatment of foot drop.

Different mechanisms including knee dislocation, replacement surgery, nerve tumor, lumbar disc herniation, sharp injury, and gunshot wound lead to foot drop. Several surgical techniques have been used for treatment of foot drop, however, they have had sub-optimal outcomes. Soleus branch of tibial nerve is a good donor for nerve transfer for treatment of foot drop. In this is retrospective study, we reviewed medical records of 6 consecutive patients with sustained foot drop following injury to lumbar root or peroneal nerve, who underwent transfer of the soleus branch of tibial nerve to deep per…

Medial reefing without lateral release for recurrent patellar instability: midterm and long-term outcomes.

BACKGROUND: There is no consensus on the optimal surgical technique for recurrent patellofemoral instability. Despite many different surgical procedures discussed in the literature, there are few studies with long-term outcomes. HYPOTHESIS/PURPOSE: The purpose of this study was to compare subjective and objective preoperative data from patients with recurrent patellar instability and normal alignment to midterm and long-term outcomes after an arthroscopically assisted medial reefing technique. The hypothesis was that the previously reported successful outcomes at 60 months would be well maint

Progressive subluxation and polyethylene wear in total knee replacements with flat articular surfaces.

One hundred eighty-six Microloc tricompartmental, cruciate-sparing, primary total knee replacements in 136 patients were performed between 1983 and 1987. Femoral components were made of cobalt-chrome-molybdenum alloy. The tibial component baseplates were made of titanium-aluminum-vanadium alloy. The baseplates were made with three small porous-coated pegs for fixation with or without cement, or with a central stem for fixation with cement. The polyethylene was affixed to the baseplate by a mechanical locking mechanism that was nonmodular. The articular surfaces of both components were flat med

Acute patellar dislocation in adolescents: operative versus nonoperative treatment.

PURPOSE: The purpose of this prospective non-randomised study was to compare the efficacy of two opposed methods, operative and conservative. Our hypothesis was that if the method was selected correctly, on an individual basis, the results should be approximately equal. METHODS: The study included 37 adolescents aged between 12 and 16 years, with a mean follow up of 6.1 years. The presence of a significant loose body, confirmed by precise imaging, was the key for selecting operative or arthroscopic treatment. In both groups of patients, we evaluated functional knee scores and the incidence of

[Periprosthetic fractures after total knee arthroplasty].

The increasing implantation rates of knee arthroplasties are associated with a growing prevalence of complications like periprosthetic fractures. Underlying patient, implant and/or operation technique-related risk factors contribute to these fractures which often occur after minor trauma. In the diagnostic process, fracture dislocation, implant stability, and integrity of the extensor mechanism have to be assessed. Valid classification systems are available to guide treatment decisions. Treatment goals are precise reposition, stable fixation, restoration of function, and early mobilization. In

Managing Hip Arthritis in an Above-Knee Amputee: A Case Report of Total Hip Arthroplasty with Surgical Technique and Rehabilitation Considerations.

INTRODUCTION: With improved survival rates and functional outcomes, amputees are increasingly presenting with degenerative hip pathology requiring total hip arthroplasty (THA). However, THA in patients with ipsilateral lower limb amputation poses unique anatomical, biomechanical and technical challenges, and current surgical training often lacks specific technical insights for these complex cases. The literature remains limited, with few reports detailing technical strategies and rehabilitative measures in this patient population. This report aims to contribute to the existing knowledge by hig

Open reduction for hip dislocation in children with arthrogryposis multiplex congenital: Outcomes of a systematic review.

INTRODUCTION: The incidence of hip dislocation (HD) in arthrogryposis multiplex congenital ranges from 15 to 30 %. Besides a stable hip, the ambulation potential of an AMC child is also dependent on severity of associated knee and foot deformations. The primary objective of this review is to determine the proportion of ambulators in AMC children treated by open reduction for HD. METHODS: We searched major electronic bibliographic databases for reports on the treatment of HD among AMC children. Based on the surgical approach for open reduction of HD in AMC children, we divided the included stu
