PURPOSE: Postpartum pelvic pain beyond the normal level poses a problem to obstetricians. Beyond normal physiologic loosening of the pubic symphysis and sacroiliac joints (SIJs) during pregnancy, symphyseal separation and rupture must be excluded. OBJECTIVE: The purpose of this study was to assess patterns of abnormal pudendal nerve function in women who complain of postpartum fecal incontinence.
STUDY DESIGN: During a 12-month period, a cohort of 83 women underwent neurophysiologic assessment as part of an evaluation of fecal incontinence after vaginal delivery. Pudendal nerve assessment consisted of the measurement of the clitoral-anal reflex and quantitative electromyography of the external anal sphincter. We report for the first time on a puerperal ovarian vein thrombophlebitis associated with heparin-induced thrombocytopenia type II. Two weeks after cesarean section with subsequent prophylactic application of heparin a thrombosis in the left deep femoral and pelvic veins and in the right ovarian vein occurred. Despite only moderately reduced platelet count a heparin-induced thrombocytopenia type II was diagnosed by heparin-induced platelet activation test. OBJECTIVE: During ovulation induction (OI), ovarian stimulation is accomplished by hormonal manipulation, which includes administration of gonadotropins, gonadotropin-releasing hormone agonists, follicle-stimulating hormone, and luteinizing hormone. In in vitro fertilization (IVF), progesterone is often added. Because of the possibility of hormone-associated flare or thrombosis, patients with systemic lupus erythematosus (SLE) and primary antiphospholipid syndrome (primary APS) undergoing OI/IVF are potentially at increased risk. This study identified risk factors associated with readmission for postpartum endometritis. Intradural spinal lipomas that are not associated with spinal dysraphism are rare. We report four cases of adult patients with intradural spinal lipomas. All of the patients were young adults, three women and one man, who ranged in age from 22 to 37 years (mean, 30.3 yr). The clinical presentation of these lesions varied, depending upon the size and location of each lipoma. A series of hysterectomies performed during labor or shortly after delivery, all as emergencies is reported. The study covered the ten years 1982-1991, during which 65,488 deliveries resulted in infants with a birth weight of 500 g or more. We report five cases of OVT and review the literature on this disease. Among our patients OVT was not suspected clinically and was diagnosed by CT scanning or MRI. Ultrasound imaging, utilized in three cases, failed to detect OVT. Although this disease usually occurs after delivery, in two cases it was diagnosed before delivery. Of the two postpartum cases, one had a typical presentation and the other was asymptomatic. The final case was diagnosed incidentally 5 months after removal of an ovarian carcinoma. Five cases of acute fatty liver of pregnancy are described. These are the only recognized cases of this disorder occurring in a 2 year period in Western Australia.
