Severity in phenotypic expression of homozygous sickle cell disease (Hb.SS)--does hypermelanotic or hypomelanotic skin status of affected patients play a role?

Abnormal hemoglobin distribution on global map, of which hemoglobin S (Hb.S) accounted for about 80% of the disorders resulting from them are more prevalent in the tropics and sub-tropics. Homozygous sickle cell disease (Hb.SS) is the most common and most severe form of sickle cell disease (SCD) in phenotypic expression. The prevalence and severity in phenotypic expression of SCD had been noted to decrease farther away from the equatorial region, with prevalence rate of sickle cell trait of about 2% and less than 1% in North African coast and South Africa, respectively, compared to about 10-40…

Evaluation of the expanded newborn screening program in New York City.

Since September 1974, New York State public health law has mandated that all newborn infants be tested for phenylketonuria, maple syrup urin … ected incidence of the conditions of highest prevalence, incidentally found during screening: i.e., sickle cell traits, AS and AC. What is equally apparent in reviewing this first year's experience is the extent to which the New York State law, its structure, and implementation have fallen short of the ultimate objective. The major reason for this failure is lack o …

Plasmodium falciparum.

Plasmodium falciparum is the etiological agent of malaria tropica, the leading cause of death due to a vector-borne infectious disease, claiming 0.5 million lives every year. The single-cell eukaryote undergoes a complex life cycle and is an obligate intracellular parasite of hepatocytes (clinically silent) and erythrocytes (disease causing). An infection can progress to a wide range of pathologies, including severe anemia and cerebral malaria, which can lead to death. P. falciparum repeatedly replicates over the course of 48h inside erythrocytes, resulting in exponential growth and rapid dise

An extreme consequence of splenectomy in dehydrated hereditary stomatocytosis: gradual thrombo-embolic pulmonary hypertension and lung-heart transplantation.

Dehydrated hereditary stomatocytosis (DHS) belongs to the heterogeneous class of hemolytic anemias with leaky red cell membranes. Splenectomy is a highly deleterious treatment, because it favors, with virtually no exception, the occurrence of thromboembolic disease. We describe here the extreme case of a patient with DHS and an associated sickle cell trait. Splenectomy was carried out due to a splenic infarction that occurred during an airplane journey. About 12 years later, the patient noticed an exertional dyspnea, which gradually worsened to such a degree that she became severely incapacita…

Hydroxyurea in the management of sickle cell disease: pharmacogenomics and enzymatic metabolism.

Hydroxyurea (HU) was approved to be used in the treatment of sickle cell disease (SCD) because of its anti-sickling potential. However, there is variability in HU response among SCD patients and this can be due to physiological, socioeconomic, environmental, metabolic and/or genetic factors. The present review focuses on the latter two. Three quantitative trait loci, HBG2, BCL11A and HMIP, have been suggested as important markers for HU response. Other genes (ASS1, KLF10, HAO2, MAP3K5, PDE7B, TOX, NOS1, NOS2A, FLT1, ARG1, ARG2, UGT1A1, OR51B5/6, SIN3A, SALL2, SAR1A, UTB, OCTN1, CYP2C9, AQP9, M

Neonatal screening for sickle cell disease and other hemoglobinopathies in "the changing Europe".

BACKGROUND: Inherited hemoglobin disorders (sickle-cell disorders and thalassaemias) represent an increasing global health problem. The early detection of sickle cell disease allows counselling for family members about disease management and future reproductive decisions. The aim of the present study was to estimate the birth prevalence of hemoglobinopathies in newborns of Italian couples and couples of immigrants from endemic areas living in an urban area of northern Italy in order to assess the opportunity of implementing a neonatal screening programme for hemoglobin disorders. METHODS: Inc

Prevalence of inherited blood disorders and associations with malaria and anemia in Malawian children.

In sub-Saharan Africa, inherited causes of anemia are common, but data are limited regarding the geographical prevalence and coinheritance of these conditions and their overall contributions to childhood anemia. To address these questions in Malawi, we performed a secondary analysis of the 2015-2016 Malawi Micronutrient Survey, a nationally and regionally representative survey that estimated the prevalence of micronutrient deficiencies and evaluated both inherited and noninherited determinants of anemia. Children age 6 to 59 months were sampled from 105 clusters within the 2015-2016 Malawi Dem

Rickettsia felis DNA recovered from a child who lived in southern Africa 2000 years ago.

The Stone Age record of South Africa provides some of the earliest evidence for the biological and cultural origins of Homo sapiens. While there is extensive genomic evidence for the selection of polymorphisms in response to pathogen-pressure in sub-Saharan Africa, e.g., the sickle cell trait which provides protection against malaria, there is inadequate direct human genomic evidence for ancient human-pathogen infection in the region. Here, we analysed shotgun metagenome libraries derived from the sequencing of a Later Stone Age hunter-gatherer child who lived near Ballito Bay, South Africa, c…

The effect of malaria on stunting: an instrumental variables approach.

BACKGROUND: Previous studies have found mixed evidence for an effect of malaria on stunting, but have suffered from concerns about confounding and/or power. Currently, an effect of malaria on stunting is not included in the Lives Saved Tool (LiST) model. METHODS: We used instrumental variables regression with the sickle cell trait and random assignment to bednets as instruments in the analysis of data on children aged 0-2 y from a bednet trial in western Kenya. RESULTS: We estimated that one additional clinical malaria episode per year increases the odds of a child being stunted by 6% (OR es…

Uncommon asymptomatic Plasmodium falciparum infections in Gabonese children.

To assess the frequency of asymptomatic plasmodial infections in young children living in an area of hyperendemicity, a cohort of 200 children in Gabon was investigated longitudinally. Of 660 cases of Plasmodium falciparum infection, 77% were symptomatic at the time they were identified and only 7% were preceded by an asymptomatic phase of >4 days. Sickle cell trait, glucose-6-phosphate dehydrogenase deficiency, and mutation in the promoter region of tumor necrosis factor (TNF(-376A/-238A)) were significantly associated with asymptomatic P. falciparum infection (P=.03, P=.009, and P<.001, resp…

Sudden, unexpected death of a young marathon runner as a result of bronchial malformation.

Deaths of young athletes are mainly caused by cardiac problems. Noncardiac deaths are infrequent and related to heat stress, drugs, sickle cell trait, and asthma. Herein, we report the case of a 28-year-old man, who collapsed during a marathon race, within sight of the finish line. Despite immediate resuscitation, he died shortly after hospitalization. Autopsy findings revealed neither unambiguous cardiac nor previously published noncardiac causes. Traumatic or drug-related death was excluded as well. We did find, however, focally hyperinflated pulmonary areas adjacent to atelectasis, intersti…

Prevalence of mitral valve prolapse in Saudi sickle cell disease patients in Dammam - A prospective-controlled echocardiographic study.

Mitral valve prolapse (MVP) is the most common valvular heart disease and there are numerous reports of a strong association with many conditions including sickle cell disease (SCD). Since SCD is very common in the Eastern Province of Saudi Arabia, we undertook a prospective controlled study to determine and compare the prevalence of MVP in the SCD patients with other groups of subjects. Three hundred and sixteen subjects (156 males and 160 females) were studied. They were divided into four groups based on their hematologic diagnoses - I SCD, II normal controls, III sickle cell traits, IV othe…

Fibrinolytic activity in adult Kenyan patients with sickle cell disease.

The sickling of erythrocytes increases viscosity and reduces the rate of both local circulation and arterio-venous transit time. This causes occlusion of capillaries by "microthrombin". The occlusion is implicated in the multiplicity of vaso-occlusive complications of both acute and chronic nature. Whether or not anticoagulant therapy is warranted in these states has remained debatable. There is no clear evidence that there is an inherent coagulation disorder. Earlier studies indicate that fibrinolysis is normal in steady state sickle cell disease but decreased during sickle cell crisis. We st

Reproducibility of tricuspid regurgitant jet velocity measurements in children and young adults with sickle cell disease undergoing screening for pulmonary hypertension.

The reproducibility of tricuspid regurgitant jet velocity (TRJV) measurements by Doppler echocardiography has not been subjected to systematic evaluation among individuals with sickle cell disease (SCD) undergoing screening for pulmonary hypertension. We examined sources of disagreement associated with peak TRJV in children and young adults with SCD. Peak TRJV was independently measured and interpreted a week apart by separate sonographers and readers, respectively, in 30 subjects (mean age, 15.8 ± 3.3 years) who provided 120 observations. We assessed intra-/inter-reader, intra-/inter-sonograp

Frequency of foetal haemoglobin and haemoglobin values in various haemoglobin genotypes in Calabar, Nigeria.

The frequency of foetal haemoglobin (HbF) and haemoglobin (Hb) levels were determined among 108 subjects with different haemoglobin genotypes in Calabar, between March and October, 1996. Hb-genotype was determined using the cellulose acetate method, while HbF and Hb were estimated using the modified Betke alkaline denaturation method and cyanmethaemoglobin techniques respectively. Haemoglobin genotype SS individuals had the highest mean HbF concentration of (3.05 +/- 1.61) as compared to AA and AS subjects with means of 0.195 +/- 0.25 and 1.072 +/- 0.98 respectively. Natural protection against severe Plasmodium falciparum malaria due to impaired rosette formation.

Genes for two lethal diseases, thalassemia and sickle cell anemia, are favored by evolution because, in their heterozygous form, they protect against cerebral malaria. Rosette formation, the binding of uninfected red cells (RBCs) to Plasmodium falciparum-infected RBCs (PRBCs), has previously been found to be associated with cerebral malaria, the most important severe manifestation of P falciparum malaria. We show here that thalassemic RBCs and, under certain conditions, even hemoglobin S (HbS)-containing RBCs possess an impaired ability to bind to PRBCs, forming small and weak erythrocyte rose

Adherence to a reproductive health intervention for young adults with sickle cell.

BACKGROUND: The CHOICES intervention is tailored specifically for young adults with sickle cell disease (SCD) or sickle cell trait (SCT). The face-to-face (F2F) delivery format is feasible with efficacy for improving knowledge about reproductive health for those with SCD or SCT. PURPOSE: The purpose of the study was to compare the participant adherence to a remote online CHOICES intervention study ( N = 107) and a F2F CHOICES intervention study ( N = 234). METHODOLOGY: In both studies, participants with SCD or SCT were randomized into experimental or usual care control groups. Descriptive st…
