[How to examine a patient with higher cortical dysfunction at bedside].

In practice, a neuropsychological evaluation may serve one or more purpose. The first systematic applications of neuropsychological assessment dealt with diagnosis. Before the days of sophisticated neuroimaging, the localization of a cerebral lesion was neuropsychology's most important function. Recent developments in neuroradiological techniques have greatly reduced the contributions of neuropsychological assessment to diagnosis and lesion localization. Today, neuropsychological assessment is most usually called upon for the detailed behavioral description necessary for intelligent patient ca

[A case of dementia with motor neuron disease associated with agraphia--the omission of kana letters].

We report a 55-year-old right-handed Japanese man with motor neuron disease and dysgraphia of kana letters. He was admitted to our hospital because of dysarthria and dysphasia. On admission, the results of general physical examination were within normal limits. Neurological examination revealed severe dysarthria, dysphasia, impaired movement of the tongue without fasciculation and slight distal muscle weakness in the bilateral upper limbs. There were no fasciculation of the muscle. Deep tendon reflexes were hyperactive without Babinski's signs. Sensation, coordination, and gait were normal. Ne

The through-and-through oromandibular defect: rationale for aggressive reconstruction.

Through-and-through oral cancer (T4+) involving contiguous mucosa, mandible, and skin is a devastating disease with poor prognosis and represents one of the most difficult reconstructive challenges in head and neck surgery. Thirty-eight patients underwent immediate microvascular reconstruction following surgical tumor ablation. The purpose of the present review was to assess the value of microvascular reconstruction in these essentially palliative reconstructive efforts. The iliac crest osteocutaneous flap was used in the majority of patients and was found to be ideal for the reconstruction of

Effect of neurointensive care upon outcome following severe head injuries in childhood--a preliminary report.

From March 1978 till August 1981 neurointensive care was applied to 24 children with severe head injuries (Glasgow Coma Scale less than or equal to 7, mean 5 +/- 1) corresponding to 7% of a population with head injuries observed during the same period. The neurointensive care included continuous intracranial and arterial pressure monitoring and normalization of intracranial and cerebral perfusion pressure with intubation, hyperventilation, control of body temperature, dexamethason, barbiturates and osmotic agents. The follow-up time is 1.5 to 4.4 years (mean 2.5 years). The results obtained by

Intrafamilial Variability in NRXN1-Associated Neurodevelopmental Disorders: Clinical and Genetic Insights from a Family Case Study with Literature Review.

The neurexin1 gene (NRXN1) encodes a presynaptic adhesion molecule that plays a critical role in synapse formation, maintenance, and function. Copy-number variants (CNVs) affecting the NRXN1 locus, including submicroscopic deletions, represent rare variant acting as a predisposition for neurodevelopmental disorders, such as Pitt-Hopkins-like syndrome type 2 (MIM #614325) and susceptibility to schizophrenia (MIM #621407). Variations in NRXN1 gene are associated with marked clinical heterogeneity. We present a familial case involving two male siblings (aged 6 and 5 years) and their 28-year-old m

Classification of Motor-Imagery Tasks Using a Large EEG Dataset by Fusing Classifiers Learning on Wavelet-Scattering Features.

Brain-computer or brain-machine interface technology allows humans to control machines using their thoughts via brain signals. In particular, these interfaces can assist people with neurological diseases for speech understanding or physical disabilities for operating devices such as wheelchairs. Motor-imagery tasks play a basic role in brain-computer interfaces. This study introduces an approach for classifying motor-imagery tasks in a brain-computer interface environment, which remains a challenge for rehabilitation technology using electroencephalogram sensors. Methods used and developed for

Prenatal and postnatal diagnoses and phenotype of 8p23.3p22 duplication in one family.

BACKGROUND: Distal 8p duplication is rare but clinically significant. Duplication syndrome results in variable phenotypes, such as developmental delay, intellectual disability, and malformation of the heart. We aimed to provide a better understanding of the phenotypes by studying duplication and its effects in a single family. METHODS: In a family with a previously induced labor (second fetus) at 12 weeks gestation due to increased nuchal translucency (3.5 mm), copy number variation sequencing (CNV-seq) revealed a 16.22 Mb deletion of 8p23.3p22. For their subsequent pregnancy, the family requ

A Unifying Algorithm in Microvascular Reconstruction of Oral Cavity Defects Using the Trilaminar Concept.

UNLABELLED: Although many algorithms exist to classify oral cavity defects, they are limited by either considering a single subsite or failing to provide a concise reconstructive algorithm for the breadth of defects. Based upon our experience as a tertiary referral center, a unifying algorithm is presented that guides free flap selection in this heterogenous population. METHODS: All intraoral defects requiring microvascular reconstruction from February 2012 to August 2018 were reviewed. Defects were classified according to their depth as unilaminar (type U = mucosa only), bilaminar (type B =

Cognitive outcome of patients with classic infantile Pompe disease receiving enzyme therapy.

OBJECTIVE: Classic infantile Pompe disease affects many tissues, including the brain. Untreated infants die within their first year. Although enzyme-replacement therapy (ERT) significantly increases survival, its potential limitation is that the drug cannot cross the blood-brain barrier. We therefore investigated long-term cognitive development in patients treated with ERT. METHODS: We prospectively assessed cognitive functioning in 10 children with classic infantile Pompe disease who had been treated with ERT since 1999. Brain imaging was performed in 6 children. RESULTS: During the first 4
