Biceps-to-triceps transfer in tetraplegia. The medial route.

Eight tetraplegic patients (13 elbows) were treated by biceps-to-triceps transfer. To avoid the risk of radial nerve injury, we chose a medial routing of the biceps. The mean follow-up after surgery was 17.8 months (range, 4-47 months). No complications were encountered. Active extension of the elbow was achieved in each case. The mean postoperative active range of motion of the elbow was 6 degrees extension and 137 degrees flexion. After the biceps-to-triceps transfer mean extension torque of the elbow was 3.7 Nm and mean flexion torque was 10 Nm. In eight elbows in which it was measured, the

[Long-term results following fibulo-tarsal ligament reconstruction. Comparison of the Watson-Jones and the (modified) Chrisman-Snook techniques].

The chronic lateral instability of the ankle joint presents a major problem after an acute tear of the lateral ankle ligaments. In this study we controlled 26 patients clinically and radiologically, who had undergone ankle ligamentoplasty (Watson-Jones and Chrisman-Snook (modified) techniques) between 1987 and 1991 because of chronic lateral ankle instability. The aim of this study was to evaluate if the more difficult Chrisman-Snook technique had better results. The patients had been suffering from a chronic lateral instability on the average of 6-7 years before surgery. The operations were p

Kidner procedure for symptomatic accessory navicular and its relation to pes planus.

The role of relocation of tibialis posterior tendon insertion in the treatment of symptomatic accessory navicular and its relation to the pes planus was studied in 28 patients with symptomatic accessory navicular. Two hundred non-affected individuals were used as control. The calcaneal pitch angle measured radiographically, was used as an indicator of pes planus. All patients had had an excision of the accessory navicular and relocation of the tibialis posterior tendon insertion (Kidner procedure). The average follow-up was 3.2 years. The results were good in 27 patients, and fair in 1 patient

Muscle transfers in brachial plexus lesions.

The concept of reconstruction to regain lost function after brachial plexus lesions has to be as broad and complex as possible. We have been exploring wider and more novel clinical concepts at the Clinic of Plastic, Hand, and Reconstructive Surgery at the Medical School of Hannover. Our ideas are supported by experience in 160 patients. We have attempted to combine the use of a vascularized nerve graft and a microvascularly-transferred autologous muscle. Patients undergoing the procedures have included those with late complete root avulsions and no functional return, as well as previously oper

Rehabilitation of anterior cruciate ligament injuries.

The previously outlined exercise program for the knee is for the instability that results from the loss of the anterior cruciate ligament and/or capsular ligaments laterally. Its success depends on communication between surgeon, therapist, and trainer. Each must understand what the other is doing and must follow the biomechanical and healing restraints of the surgery. The understanding of soft tissue healing is ongoing. Research in this field will greatly enhance the rehabilitation programs of the future.

[Free transplantation of a fibula with epiphysis].

Problems of free bone transplantation, as well as those of bone transplantation by microvascular anastomoses are exemplified by a case of free fibula transplantation--the epiphysis being conserved--by microvascular anastomoses. If revascularisation of the bone transplant is immediately achieved, the chance for survival of bone tissue is improved and there is a chance that the epiphysis will retain the capacity to grow. By multiple surgical interventions the stability of the abnormally mobile forearm was reestablished and considerable lengthening of the shortened skeleton was achieved. By tendo

The adductor transfer in the high-risk hip in myelodysplasia: a preliminary report.

Hip instability occurs in meningomyelocele patients with lumbar level paralysis because of muscle imbalance. Transferring the hip adductor origins to the ischium can be used as the primary procedure to restore muscle balance and prevent hip instability in myelodysplasia patients if performed by the age of one year. Twelve lumbar meningomyelocele patients underwent bilateral adductor transfers. Secondary procedures, such as iliopsoas release or transfer or varus derotation osteotomy, were done in 14 hips. Ten hips needed no further surgery. Twenty of 24 hips were clinically and roentgenographic

Pediatric Microsurgery in the Reconstruction of Complex Posttraumatic Foot and Ankle Defects: A Long-Term Follow-Up with a Comprehensive Review of the Literature.

BACKGROUND: Lower limb reconstruction is a well-recognized challenge to the trauma or plastic surgeon. Although techniques and outcomes in the adult population are well documented, they are less so in the pediatric population. Here, we present our experience in the management of posttraumatic foot and ankle defects with free tissue transfer in children. METHODS: We performed a retrospective analysis of 40 pediatric patients between the ages of 3 and 16 from 2008 to 2016 who underwent foot and ankle soft tissue reconstruction. Any patient who underwent reconstruction for any reason other than

Kinetic and kinematic evaluation of the ankle joint after achilles tendon reconstruction with free semitendinosus tendon graft: preliminary results.

Chronic rupture of the Achilles tendon (AT) is a surgical challenge and has effects on the gait. The purpose of this study was to evaluate the kinetic and kinematic parameters of the ankle joint in patients with AT rupture operated using a free semitendinosus tendon graft. Thirteen patients were analyzed 6 and 12 months after surgery in a force platform, while the movements were recorded by six infrared cameras. The kinematic variables analyzed included speed, cadence, step length, percentage of stance phase, and range of movement (ROM) of the ankle joint in the sagittal and frontal planes. Ki

Medial Patellar Retinaculum Cable Transfer With Anchoring Suture Reinforcement and Medial Tightening to Correct Patellar Dislocation in Skeletally Immature Patients.

Surgical treatment of patellar dislocation poses a challenge in skeletally immature patients. The traditional approach of lateral release and medial imbrication carries a high probability of recurrence. Medial patellofemoral ligament reconstruction recently has become a popular procedure for patellofemoral instability, but its broad application in immature skeletal systems is limited by the risk of injury to epiphyseal plate. Herein, we describe an alternative surgical strategy whereby medial patellar retinaculum cable transfer is done in conjunction with anchoring suture reinforcement and med

Flap decisions and options in soft tissue coverage of the upper limb.

Soft tissue deficiency in the upper limb is a common presentation following trauma, burns infection and tumour removal. Soft tissue coverage of the upper limb is a challenging problem for reconstructive surgeons to manage. The ultimate choice of soft tissue coverage will depend on the size and site of the wound, complexity of the injury, status of surrounding tissue, exposure of the vital structures and health status of the patient. There are several local cutaneous flaps that provide adequate soft tissue coverage for small sized defects of the hand, forearm and arm. When these flaps are limit

Type II and IIIA thumb hypoplasia reconstruction.

Thumb hypoplasia treatment requires considering every component of the maldevelopment. Types II and IIIA hypoplasia share common features such as first web space narrowing, hypoplasia or absence of thenar muscles and metacarpophalangeal joint instability. Many surgical techniques to correct the malformation have been described. We report our surgical strategy that includes modifications of the usual technique that we found useful in reducing morbidity while optimizing the results. A diamond-shape kite flap was used to widen the first web space. Its design allowed primary closure of the donor s

Reconstruction of a congenital hypoplastic thumb with use of a free vascularized metatarsophalangeal joint.

Reconstruction of a Blauth type-IIIB hypoplastic thumb with use of a free vascularized metatarsophalangeal joint was performed in four patients (four hands). Several tendon transfers also were performed, either primarily or secondarily, to mobilize the reconstructed thumb. Three patients (three hands) were followed for at least two years after the reconstruction; the results for these three patients were compared with those for four patients (six hands) who had been managed with pollicization of the index finger because of a similar deformity of the thumb. The patients were evaluated with rega…

Craig-Scott orthosis: a biochemical and functional evaluation.

The Craig-Scott double upright knee ankle orthosis was subjected to a biomechanical evaluation which included analysis of the force interaction with the supported limb and a functional evaluation. The results were compared with data previously determined from biomechanical evaluations of other common orthoses. In the Craig-Scott design, the single application of the knee stabilizing force below the knee concentrates this force in a relatively small bony area, sometimes exceeding tolerance. This might be avoided by using a tibial closure with patellar tendon bearing features. The design was fou
