The aim for the present study was to test the feasibility of using a front surface aspheric contact lens geometry to correct aphakic patients from a visual standpoint. Two aspheric contact lens designs are tested, using a standard visual acuity test and contrast sensitivity, and their performance is compared to a conventional spherical design. An hyperbolic front surface of eccentricity of 1.2 was chosen, as its performance was similar to the spherical contact lens.

This study evaluates the influence of age, type of aphakic correction and macular disease on outcome of cataract surgery. This is done by multivariate regression analyses on data collected prospectively in 123 consecutive cataract patients. The analyses showed that age and type of correction had only minor influence on outcome. The most influencing factor was macular disease. Seventy-six patients undergoing cataract extraction and intraocular lens implantation were examined prospectively to evaluate the accuracy of a photogrammetric method for estimating postoperative anterior chamber depth and the predictive value of preoperative anterior chamber depth on postoperative anterior chamber depth. The photogrammetric method was less accurate than the use of a fixed figure determined by the average measured postoperative anterior chamber depth. Measuring the anterior chamber depth preoperatively had even less predictive value.

One hundred twenty-eight consecutive aphakic glaucomatous eyes of 91 patients were examined by fluorescein angiography. Macular edema was present in 16 (28%) of 56 eyes currently being treated with epinephrine and 9 (13%) of 72 eyes not currently being treated with epinephrine. CLINICAL CASE: A case is presented of an 85-year-old woman living in a geriatric residence, who was admitted to the emergency department of our hospital with a spontaneous expulsive choroidal hemorrhage in her left eye. There was no a history of ophthalmic disease, and the patient only reported having intracapsular cataract surgery in both eyes 25 years ago, and that she also became blind in her left eye in the past few years.

Angle surgery is often performed for pediatric glaucomas. Here we report visually significant hyphema and "spill-over" vitreous hemorrhage after trabeculotomy performed for aphakic glaucoma. Vitrectomy was necessary to remove the persistent vitreous blood in 2 of the 3 cases. We suspect this risk may be greater in aphakic eyes and propose measures to minimize this potentially amblyogenic complication.

A simple technique for the accurate surgical alignment of toric phakic and aphakic intraocular lenses (IOLs) is described. The mathematical relationship between toric misalignment and cylinder error is reviewed to demonstrate the importance of toric lens alignment in the setting of increased rotational stability with newer generation lenses. A formula for calculating the exact lens misalignment given a manifest cylinder is also presented.
