Knowledge of filariasis and perceptions of the disease were explored among 413 lymphoedema cases attending two filariasis clinics in the Colombo district of Sri Lanka. The information was collected in interviews based on a pre-tested, interviewer-administered questionnaire. Only 15% of the patients had initially attributed their limb swelling to filariasis. Most knew that filariasis resulted from mosquito bites (81.1%) and that the disease is transmissible (59.8%) and preventable (74.3%). The majority did not know, or were uncertain, whether filariasis causes swelling of the breasts in females

Neglected diseases remain one of the largest causes of disease and mortality. In addition to the difficulties in provision of appropriate drugs for specific diseases, many other factors contribute to the prevalence of such diseases and the difficulties in reducing their burden. We address the role that poor governance and politically motivated oppression have on the epidemiology of neglected diseases. We give case examples including filariasis in eastern Burma and vector-borne diseases (Chagas' disease, leishmaniasis, and yellow fever) in Colombia, we show the links between systematic human ri

In 1997, the World Health Assembly resolved to eliminate lymphatic filariasis as a public health problem by the year 2020. By the end of 2004, almost half of the 83 endemic countries had initiated national programmes, providing mass drug administration to an at risk population of approximately 435 million. This remarkable achievement is the result of an enormous amount of technical, financial and political support from public and private sectors at the community, national, regional and global level. As the global programme to eliminate lymphatic filariasis enters its second quarter of operatio

In bancroftian filariasis, ultrasonography (USG) is a suitable tool to monitor infection by the detection of adult filariae in addition to antigen detection tests (ICT, Og4C3). However, in brugian filariasis, ultrasound examinations in humans have so far failed to detect adult worms and no antigen test is available to verify infections in patients who are carriers of adult worms but amicrofilaraemic. In this study, we describe the feasibility of detection of adult Brugia malayi filariae by USG. Worm nests were detected in 4 of 32 patients in India and Indonesia, located in the breast, the thig

Community compliance is crucial for the success of mass treatment using cooking salt fortified with diethylcarbamazine citrate (DEC) for the elimination of lymphatic filariasis (LF). We describe the role and effectiveness of school-based health education for social mobilization to promote the use of DEC salt, in an endemic district in India. Health education through classroom sessions was the main motivational strategy used. Community members were targeted to receive the message through children. The impact of this approach was assessed among students and community members using an interviewer

OBJECTIVES: An estimated 15 million persons suffer from lymphoedema of the leg in filariasis-endemic areas of the world. A major factor in the progression of lymphoedema severity is the incidence of acute dermatolymphangioadenitis (ADLA), which is triggered by bacteria that gain entry through damaged skin, especially in the toe web spaces ('interdigital skin lesions'). Little is known about the epidemiology of these skin lesions or about patients' awareness of them.
METHODS: We interviewed and examined 119 patients (89% women) with lymphoedema of the leg in Recife, Brazil, an area endemic for

This paper reports the coverage, compliance and other operational issues of mass drug administration (MDA) of diethylcarbamazine and albendazole under a programme to eliminate lymphatic filariasis (LF) in Orissa state of India. Both quantitative and qualitative methods were used to collect data from 90 villages and nine urban areas of four districts of Orissa, India. In Orissa, 67% of people older than 2 years had received the drugs during MDA and 42% had consumed them. About 25% of people had not taken the tablets although they received them. Urban areas recorded lower rates than rural areas.

The tolerability and efficacy of single dose DEC (12mg/kg body weight) or co-administration of DEC (6mg/kg body weight) with Ivermectin (200 or 400 mcg/kg of body weight) was studied in 60 asymptomatic W. bancrofti microfilariae (Mf) carriers following a double blind randomized design. The drugs were tolerated well. The incidence of adverse reactions of DEC (85.0%), DEC + Ivermectin 200mcg (95.0%) and DEC + Ivermectin 400mcg (100%) did not vary significantly (P>0.05). The mean score of adverse reaction intensity due to DEC + Ivermectin 200mcg (1.41) was significantly higher compared to DEC (0.

During population-wide cross-sectional surveys for Wuchereria bancrofti microfilaremia, circulating antigenaemia, and clinical disease in a high and a low endemicity community in East Africa in 1998, a portable ultrasound scanner was used simultaneously to examine the scrotal tissue of the male populations (n = 422 and 328, respectively) for signs of adult worms. The overall microfilaria (mf) and circulating filarial antigen (CFA) prevalences in the scanned males were 30.8% and 53.6% in the high and 4.3% and 19.8% in the low endemicity community, respectively. During ultrasound examination, th

Lymphatic filariasis caused by Wuchereria bancrofti is a major public health problem in 73 tropical and subtropical countries including India. Delimitation of endemic areas is essential to plan control operations. The current method of night blood survey (NBS) for delimitation is cumbersome, time-consuming and expensive. Therefore, there is a need to develop assessment procedures which can rapidly delimit endemic areas. For this purpose we evaluated three procedures: direct interviewing of key informants using structured questionnaires, an indirect method of a self-administered questionnaires

The host-parasite interactions of Brugia malayi in mice are complex and multifactorial. In order to study the role of T cells in early B. malayi development, we infected TCRalpha(null) mice, which retain a population of CD4+ TCRbeta+ cells and TCRbeta(null) mice, which lack all TCRalphabeta(+) T cells. TCRalpha(null) mice were permissive to L4 larval and adult worm development but TCRbeta(null) mice were not. Depletion of the CD4(+) T cells in the former abrogated the permissive phenotype. It appears that the CD4(+) TCRbeta(+) T cells that have been described in TCRalpha(null) mice may facilit

Acute attacks of adenolymphangitis (ADL) contribute significantly to the morbidity seen in cases of filarial lymphoedema. Such cases are now being treated with multiple courses of the antifilarial drug diethylcarbamazine (DEC), either alone or in combination with antibiotics or anti-inflammatory drugs, based on anecdotal experience. In this, the first double-blind, placebo-controlled study, 150 patients with lymphoedema caused by brugian filariasis, each of whom recalled two or more ADL attacks in the previous year, were enrolled on a comprehensive foot-care programme. Each was also randomly a

Filarial lymphedema is complicated by frequent episodes of dermatolymphangioadenitis (DLA). It is not certain whether DLA is of filarial or bacterial etiology. The frequency of episodic DLA does not depend on the presence or absence of microfilariae. Antibiotic therapy is effective in prevention and treatment of DLA. These observations point to the bacterial rather than filarial etiology of DLA. Skin and lymph node biopsies, tissue fluid, lymph, and blood from patients with chronic filarial lymphedema, and during acute episodes of DLA, were cultured for detection of bacteria. A high prevalence

OBJECTIVE: To determine the presence of Chlamydia trachomatis in hydrocele fluid.
METHODS: 90 male patients with hydrocele of tunica vaginalis from an endemic area for bancroftian filariasis were investigated for the presence of Chlamydia trachomatis in their hydrocele fluids. C trachomatis antigen detection tests-a direct immunofluorescence assay and an enzyme immunoassay along with polymerase chain reaction assay for amplification of a 517 bp fragment of C trachomatis endogenous plasmid-were used in this study. The patients were also tested for the presence of microfilaria in their hydrocel

In a previous study performed in south Sulawesi (Sulawesi Selatan), Indonesia, we established that the immunoglobulin G4 (IgG4) enzyme-linked immunosorbent assay (ELISA) is a suitable community diagnostic method and that it can distinguish areas of high and low prevalences within short distances. In an attempt to make this diagnostic tool more applicable in the field, a comparative study using serum and blood collected on filter paper was undertaken with 568 individuals living in 2 areas with different endemicity for brugian filariasis in south Sulawesi. In Mamuju district, where the microfila

A sandwich antibody ELISA was employed for the detection of circulating filarial antigen in patients with bancroftian filariasis. Wuchereria bancrofti recombinant antigen-derived polyclonal and monoclonal antibodies were successfully used as the revealing antibodies and their efficiency was compared. All the microfilariae (mf) positive (by finger prick and examination of 20 microliters of blood under the microscope) individuals tested showed the presence of circulating antigen(s). Among the antigen positive endemic normals (mf negative by the finger prick method), 43% showed microfilariae by a

Filaria surveys conducted in some select slum clusters namely Hari Nagar, Yamuna pusht near Vijaya Ghat along the Ring Road and Timarpur in Delhi during 1989, 1991 and 1992 respectively, covering a population of approximately 5000 slum dwellers revealed the presence of bancroftian microfilaria (mf) carriers and disease cases. The mf and disease rates (per cent) in these three slum areas were in the order of 6.3, 2.2, 3.7 and 1.4, 0.5 and 0.1 respectively. The mf density varied from 3.1 to 12.3 per 20 cumm. blood.
