We report a case of acute febrile neutrophilic dermatosis, Sweet's syndrome, associated with chronic myelogenous leukemia (CML) in which we found rearrangement of the bcr gene in DNA obtained from a skin lesion as well as in blood DNA by Southern blot analysis. This indicated the presence of CML cells within the skin lesion. To our knowledge, this is the first report in which the presence of CML cells is shown within skin lesions of Sweet's syndrome. Several chemicals are capable of inducing contact leukoderma. Here we report on a dental nurse who had been investigated elsewhere at a dermatology clinic 2.8 years earlier because of suspected occupational fingertip dermatitis. Persons with atopic dermatitis run a considerable risk of developing hand eczema when exposed to occupational agents that are a burden to the skin. This also pertains to those with a history of skin atopy in childhood. This review presents estimates of the risk of developing hand eczema and examines the evidence for an effect modification by skin atopy on exposure. Skin atopy at least doubles the effects of irritant exposure and, thus, doubles the risk in occupations where hand eczema is a common problem. A patient with an acquired pigmentation related to lichen planus pigmentosus is described. Features of acrokeratosis of Bazex were associated, related to a head and neck carcinoma. Both cutaneous conditions disappeared after treatment of the neoplasia. Diagnostic criteria of lichen planus pigmentosus are reviewed. The paraneoplastic nature of this original observation of lichen planus pigmentosus is discussed.

We evaluated two patients with allergic contact dermatitis that resulted from exposure to color film developers. A lichenoid eruption developed in one patient, whereas an eruption more characteristic of an acute spongiotic dermatitis developed in the second patient. Histologic findings in the first case were those of a "lichenoid dermatitis" but with features distinct from classic lichen planus. Forty-eight patients with progressive systemic sclerosis (PSS) (man-woman ratio, 7:41) were studied, and detailed questionnaires were used to clarify the relationship between occupational stresses and the initial signs of PSS. We recently encountered a palisading granuloma in which an infectious cause was not considered until special stains revealed hyphae and tissue culture revealed organisms of phaeohyphomycosis. A review of more than 2,500 cases of various granulomas revealed 11 cases of palisading-type granuloma caused by infection, including phaeohyphomycosis, nontuberculous mycobacteriosis, papulonecrotic tuberculid, tuberculoid syphiloderm, cat-scratch disease, sporotrichosis, cryptococcosis, and coccidioidomycosis. Infectious disease should be considered in the differential diagnosis of palisading granuloma.

We investigated the clinical applicability of the lymphocyte proliferation test (LPT) in chromium contact sensitivity. 6 out of 8 chromium-sensitive patients were positive in the LPT, whereas none of 8 non-chromium-sensitive controls responded in vitro to tri- or hexavalent chromium compounds. Two cases of pyogenic bacterial infection, which initially resembled lymphocutaneous sporotrichosis are reported. In case 1, Staphylococcus aureus and Streptococcus group A were cultured from the lesion and in case 2, S. aureus alone. Histologically, botryomyotic grains were identified in case 2; this patient had diffuse normolipidemic plane xanthoma. With oral administration of cefaclor, the cutaneous lesions dramatically improved in both cases.

Twenty patients with hand eczema were studied with the use of quantitative bacteriologic cultures before and after successful topical treatment with a potent corticosteroid. 18 cases of hand dermatitis in hairdressers seen over a 5-year period are reviewed. The diagnoses in these patients are discussed with reference to other studies of hand dermatitis in hairdressers. Contact allergy due to paraphenylenediamine and related hair dyes was the presenting complaint in younger hairdressers, while formaldehyde allergy occurred in those who were older.
