OBJECTIVE: Acute and chronic hydrocephalus are common pathologies after aneurysmal subarachnoid hemorrhage (SAH). Generally, the presence of acute hydrocephalus is associated with elevated intracranial pressure (ICP) treated with a ventricular drain. Subsequently, however, pronounced hydrocephalus without elevated ICP may develop in some patients with SAH in the postacute phase. This is described as acute low-pressure hydrocephalus (aLPH), and there are very limited data in the literature of this pathology. The aim of this study was to evaluate the rate of and factors associated with aLPH and

UNLABELLED: Higher effectiveness of cerebrospinal fluid shunting procedures is still an unresolved issue in the treatment of idiopathic normal pressure hydrocephalus. Thus, over 15% of patients do not experience symptom regression in postoperative period. In recent years, several MRI predictors have been actively investigated to forecast the outcomes of cerebrospinal fluid shunting procedures. We have previously introduced a prognostic model for comprehensive evaluation of MRI data facilitating identification of patients likely to benefit from surgical intervention.
OBJECTIVE: To evaluate dia

BACKGROUND: The pathogenesis of adult chronic hydrocephalus is not fully understood, and the temporal relationship between development of the radiological changes and neurological deterioration is unknown.
OBJECTIVE: To clarify the progression of radiological-histological changes and subsequent clinical manifestations of adult chronic hydrocephalus.
METHODS: Kaolin was injected bilaterally into the subarachnoid space overlying the cranial convexities in 20 adult rats. Magnetic resonance imaging (MRI) was obtained by using an 11.7 T scanner at 14, 60, 90, and 120 days after kaolin injection.

BACKGROUND AND OBJECTIVE: Earlier designs of hydrocephalus valves, focusing on a reduction of the problem of overdrainage, were proved to be insufficiently. Since the middle of the 90's hydrostatic valve, constructions are available, which should solve this problem. The objective of the study was to evaluate, whether these gravitational shunts would indeed reduce the percentage of overdrainage in shunted patients.
METHODS: In 51 patients clinical evaluation and MRI was performed pre- and in regular intervals postoperatively. 21 received a Dual-Switch valve, 14 a programmable Codman Hakim plus

Right atrial thrombi are rare but serious complications of ventriculoatrial (VA) shunts. Two hydrocephalic patients who received a VA shunt developed thromboembolic complications. The initial manifestation was malfunction of the distal end of the shunt and right heart failure. Bidimensional echocardiography demonstrated an intracardiac mass on the distal intra-auricular end of the shunt. Pulmonary scan showed multiple emboli even in the asymptomatic patient. This demonstrates the important role of bidimensional echography as a noninvasive diagnostic technique for this particular complication o

The authors report a case of superficial siderosis of the CNS that developed after ventriculoperitoneal (VP) shunt placement for normal-pressure hydrocephalus. A 65-year-old woman had undergone VP shunt insertion for normal-pressure hydrocephalus. Her gait disturbance, memory disturbance, and urinary incontinence all improved after the procedure. Two years later, however, her gait became ataxic and her appetite became poor. Brain MR imaging revealed a rim of hypointensity on T2-weighted sequences, enveloping the surface of the cortical fissure, cerebellum, and brainstem. Superficial siderosis

CINE phase-contrast MRI (CINE-MRI) was used to measure cerebrospinal fluid (CSF) velocities and flow rates in the brain of six normal subjects and five patients with communicating hydrocephalus. Mathematical brain models were created using the MRI images of normal subjects and hydrocephalic patients. In our model, the effect of pulsatile vascular expansion is responsible for pulsatile CSF flow between the cranial and the spinal subarachnoidal spaces. Simulation results include intracranial pressure gradients, solid stresses and strains, and fluid velocities throughout the cranio-spinal system.

The progress of experimental hydrocephalus-hydromyelia in cats is analyzed in the present study. The animals were rendered hydrocephalic by intracisternal injections of kaolin or by operatively occluding the lateral apertures. Changes in the cerebrospinal fluid (CSF) spaces were monitored by ventriculography or computed tomography. The progress of hydrocephalus in both groups can be divided into two stages: 1) A phase of raised intracranial pressure precedes the dilatation of the central canal, and 2) a progressive phase follows the dilatation of the central canal. The canal acts as a kind of

BACKGROUND: The management of hydrocephalus associated with a posterior fossa tumor is debated. Some authors emphasize the advantages of an immediate tumor removal that may normalize the cerebrospinal fluid (CSF) dynamics. However, in clinical practice, the mere excision of the lesion has been demonstrated to be accompanied by a persisting hydrocephalus in about one third of the cases. Preoperative endoscopic third ventriculostomy (ETV) offers several advantages. It may control the intracranial pressure (ICP), avoid the necessity of an emergency procedure, allow appropriate scheduling of the o

BACKGROUND: Aneurysmal subarachnoid hemorrhage (aSAH) is often associated with acute high-pressure hydrocephalus. Less commonly, an acute low-pressure hydrocephalus (ALPH) variant can develop and contribute to increased morbidity. ALPH is particularly challenging to diagnose and manage, as patients present with symptoms of increased intracranial pressure (ICP) despite the absence of corroborating evidence from ICP measurements. Misdiagnosis or delayed recognition can result in increased morbidity.
OBSERVATIONS: The authors describe a patient with aSAH who developed neurological deterioration

OBJECTIVES: Many diffusion tensor imaging (DTI) studies have described the effects of hydrocephalus on periventricular white matter in patients with normal pressure hydrocephalus. However, little is known about hydrocephalus following stroke. We investigated the effect of hydrocephalus on periventricular white matter in patients with hydrocephalus after an intracerebral hemorrhage (ICH) using DTI.
METHODS: Fourteen patients with ICH and hydrocephalus, and 17 age- and sex-matched normal control subjects were recruited. DTI parameters were estimated in six regions of interest (ROIs) in perivent

The effects of kaolin-induced hydrocephalus on CSF turnover was studied in rats by ventricular perfusion and the results compared to normals. The mean CSF formation rate measured in 20 normal rats was 2.83 microliter/min. Approximately three weeks after intracisternal kaolin, the mean CSF formation rate in 17 hydrocephalic rats was reduced by 40%. CSF absorption in hydrocephalic and normal rats varied linearly with perfusion pressures between -10 and 10 cm H2O. The response of CSF absorption rate to changes in intracranial pressure was decreased in hydrocephalic rats. Ventriculomegaly of hydro

BACKGROUND: The dynamic relationship between pulse waveform of intracranial pressure (ICP) and transcranial Doppler (TCD) cerebral blood flow velocity (CBFV) may contain information about cerebrospinal compliance. This study investigated the possibility by focusing on the phase shift between fundamental harmonics of CBFV and ICP.
METHODS: Thirty-seven normal pressure hydrocephalus patients (20 men, mean age 58) underwent the cerebrospinal fluid (CSF) infusion tests. The infusion was performed via pre-implanted Ommaya reservoir. The TCD FV was recorded in the middle cerebral artery. Resulting

Three cases of intraspinal tumors associated with hydrocephalus were encountered and treated. This review includes 67 cases, including these 3 cases, and discusses the pitfalls associated with intraspinal tumors presenting with hydrocephalus. These tumors are not easily diagnosed, especially in cases where the chief complaint, due to hydrocephalus, is headache or dementia, and does not include back pain. Among the 67 cases reviewed, symptoms suggesting normal pressure hydrocephalus were particularly noticed in patients over 40 years old. In cases presenting with headaches or dementia, back pai

Background: Analysis of cerebrospinal fluid (CSF) is a frequently used diagnostic tool in a variety of neurological diseases. Recent studies suggested that investigating membrane particles enriched with the stem cell marker CD133 may offer new avenues for studying neurological disease. In this study, we evaluated the amount of membrane particle-associated CD133 in human CSF in neuroinflammatory and degenerative diseases. Methods: We compared the amount of membrane particle-associated CD133 in CSF samples collected from 45 patients with normal pressure hydrocephalus, parkinsonism, dementia, and

Communicating hydrocephalus occurs as a result of inappropriate CSF production in the circumstance of diminished absorption. Ventricular enlargement does not progress as rapidly as the measured normal production of CSF would suggest. Thus, compensatory mechanisms must exist to respond in some manner to the altered pathophysiology. In this experiment the metabolic activity (O2 consumption) of the choroid plexus cells in animals with communicating hydrocephalus was compared with that of normals. The modified Cartesian diver technique of Zeuthen13 was employed. These preliminary measurements show
