BACKGROUND: Mucinous cystic neoplasm of the liver with extrahepatic growth is a rare benign epithelial neoplasm of the biliary system that presents with a mass effect or is incidentally found on imaging. The tumor affects mostly the common hepatic duct, which is difficult to diagnose preoperatively by radiology, endoscopy, or cystic fluid analysis. Endoscopic ultrasound is a noninvasive tool for the evaluation of features of a cystic lesion and the extent of disease. Optimal treatment is complete tumor resection.

BACKGROUND: Symptoms of choledochal cysts sometimes persist or become exacerbated. As preoperative management for patients with these cysts, we prospectively employed endoscopic drainage, based on the theory that protein plugs cause symptoms by obstructing the pancreatobiliary ducts.
METHODS: Children with choledochal cysts underwent endoscopic retrograde cholangiopancreatography (ERCP). When ERCP showed compaction with filling defects in patients with persistent or worsening symptoms (study patients), the placement of a short biliary stent tube was attempted for drainage. The clinical and ER

BACKGROUND: The management practices of liver abscess (LA) have evolved over time. The precise diagnosis of etiology and complications is pivotal for appropriate management.
METHODS: Descriptive analyses of consecutive patients treated for LA using electronic medical records at a liver unit between years 2010 and 2020 and investigate relationships between clinical, imaging, laboratory and microbiological findings, treatment strategies and mortality.
RESULTS: Of 1630 LA patients, the most common aetiologies were amoebic liver abscess (ALA; 81%) and pyogenic liver abscess (PLA; 10.3%, mainly r

A rare case of a 26-year-old male presenting with abdominal pain and jaundice was initially suspected to be a gallbladder malignancy based on imaging findings of a gallbladder fossa mass and biliary strictures. Serial imaging, including ultrasound, CT, PET-CT, and endoscopic retrograde cholangiopancreatography (ERCP), revealed a hypermetabolic mass with biliary obstruction but no gallstones. Elevated serum IgG4 levels and a marked clinical response to steroids led to the diagnosis of IgG4-related sclerosing cholangitis (IgG4-SC) and cholecystitis, an uncommon presentation that often mimics mal

Chronic hemolysis in sickle cell disease (SCD) predisposes patients to hepatobiliary complications, including pigment gallstones. We report a 13-year-old girl with hemoglobin SS SCD who presented with right upper quadrant pain, progressive jaundice, and fever. Laboratory evaluation revealed severe anemia with reticulocytosis, mixed hyperbilirubinemia, elevated lactate dehydrogenase, normal cholestatic enzymes, and marked C-reactive protein elevation. Imaging confirmed microlithiasis without significant bile duct obstruction. Supportive care included transfusion, hydration, antibiotics, and pai

Portal vein thrombosis with cavernous transformation is a rare cause of biliary obstruction. Portal biliopathy is a term that refers to abnormalities in the intrahepatic and extrahepatic biliary tract, gall bladder, and cystic duct secondary to portal hypertension. Patients may be asymptomatic, but they can also present with abdominal pain, jaundice, and fever. We present the case of a 61-year-old Caucasian female who presented with generalized weakness, dark urine, and yellow skin for three days' duration. Magnetic resonance cholangiopancreatography (MRCP) showed extrahepatic and intrahepatic

Biliary obstruction due to a proximal bile duct stricture is commonly a result of cholangiocarcinoma. We describe a patient who began having intermittent episodes of jaundice 3 years after cholecystectomy. Despite endoscopic placement of a biliary stent and adequate biliary decompression, the serum CA 19-9 level remained elevated at 58 U/ml (normal <37 U/ml). Segmental bile duct resection and Roux-en-Y hepaticojejunostomy were done. The stricture was caused by a traumatic bile duct neuroma. Diagnostic and therapeutic considerations of this entity are discussed, with special emphasis on the val

Percutaneous cholecystocholedochostomy was performed in a patient with acute cholecystitis caused by cystic duct obstruction by gallbladder carcinoma, but removal of the percutaneous cholecystostomy catheter was unsuccessful because of continuing discharge. After creation of a cholecystocholedochostomy through the cholecystostomy tract with use of a transjugular liver access set and a 21-gauge needle, self-expandable metallic stents were placed in the narrowed common bile duct and the newly created tract between the gallbladder and the common hepatic duct. The external cholecystostomy catheter

The most significant complications of hydatid cyst of the liver are rupture and suppuration with an incidence of 6-19%. Rupture into the biliary tract was the most frequent complication and comprised 170 out of 222 complicated hydatid cysts of the liver in the author's series. These jaundiced patients were managed both by obliteration of the residual cavity and restoration of the patency of the biliary tract which was obstructed by daughter cysts of remnants of the laminated membrane. Rupture into the bronchi requires immediate treatment due to bronchopulmonary complications which might ensue.

The new 99mTc biliary scintigraphy agents are highly sensitive and specific in detecting biliary tract disease and use of them is the initial procedure of choice in evaluating patients with suspected acute cholecystitis. Other clinically useful indications are evaluation of biliary kinetics; evaluation of patients with suspected traumatic bile leakage, gallbladder perforation, or postsurgical biliary tract complications; and evaluation of patients with suspected biliary obstruction. In 99mTc we have a simple radiopharmaceutical of low radiation for evaluating congenital abnormalities and neona

Four infants with biliary atresia had gross obliteration of the common hepatic duct but residual patency of the gallbladder, cystic duct and common bile duct. The patients were treated by hepatic portocholecystostomy utilizing the extant bile ducts for biliary reconstruction. Bile drainage was achieved in all four infants. There was a conspicuous absence of postoperative cholangitis. Subsequent obstruction of the distal ducts in two patients necessitated reoperation and construction of a standard biliointestinal conduit. The other two children are surviving, jaundice-free, 5 1/2 and 5 years af

Autoimmune pancreatitis (AIP) is typically associated with elevated serum IgG4 levels and IgG4-positive plasma cell infiltration in affected organs. We report a 57-year-old woman who presented with obstructive jaundice. Contrast-enhanced computed tomography showed diffuse pancreatic enlargement, distal bile duct stricture, and retroperitoneal fibrosis. Serum IgG4 was markedly decreased and remained below the reference range on repeated measurement, while immunohistochemical staining of pancreatic tissue showed no detectable IgG4-positive plasma cells. Histology revealed storiform-pattern fibro

Pancreas divisum, a congenital failure of fusion between the dorsal and ventral pancreatic buds, occurs in ~10% of the population. The prevalence of intraductal papillary mucinous neoplasm (IPMN) of the ventral duct in pancreas divisum is not well established. Malignant transformation is exceptionally rare, with this case being the first documented in the USA. A 74-year-old man presented with new-onset obstructive jaundice. A computed tomography of the abdomen and pelvis showed pancreas divisum with a 4-cm cystic mass causing a 1.8 cm ventral pancreatic duct dilatation, concerning for an IPMN.

Ascaris lumbricoides is a globally prevalent helminth that can cause hepatobiliary, pancreatic, and pulmonary complications. In immunocompetent individuals, parasitic migration typically provokes marked eosinophilia. However, underlying hematologic malignancies can blunt expected immune responses and obscure early clinical recognition. We report a case involving a 23-year-old female with a new diagnosis of Burkitt lymphoma who presented with severe epigastric pain, obstructive jaundice, and laboratory findings consistent with cholangitis and pancreatitis. Despite biliary drainage and broad-spe

Lynch syndrome, an autosomal dominant disorder caused by defective DNA mismatch repair genes, is linked to early-onset colorectal, ovarian, gastric, urinary tract and, less commonly, small bowel, biliary and pancreatic cancers. We present a case of a man in his late 50s who presented with obstructive jaundice due to a biliary hilar mass with a history of urothelial malignancy requiring surgical intervention. Family history revealed colon cancer in mother and brother both below the age of 50 years. He underwent curative resection and reconstruction. Histopathology confirmed hilar cholangiocarci

Myeloid sarcoma (MS) is an extramedullary tumor of myeloid precursor cells, frequently associated with acute myeloid leukemia (AML), and rarely occurring in isolation. We present a child with obstructive jaundice secondary to a pancreatic head mass. Initial imaging was consistent with pancreatic hematoma; however, continued symptoms led to endoscopic ultrasound (EUS)-guided biopsies that raised concern for autoimmune pancreatitis. Further histological and cytogenetic analysis confirmed pancreatic MS associated with a RUNX1::RUNX1T1 fusion. Bone marrow evaluation was negative by conventional di

Stenosis of the bile ducts is a relatively common disease arising from benign or more often malignant causes. As a result of stenosis, bile flow is interrupted, obstructive jaundice in some cases leads to inflammation of the bile ducts. The decision on the etiology of stenosis is crucial and is made on the basis of a summary of the clinical picture and the findings of laboratory and imaging examinations. Endoscopy plays a crucial role in the diagnosis of stenosis. Endoscopic retrograde cholangiography and endoscopic ultrasonography allow not only to macroscopically evaluate the stenosis, but a

We present the case of an 82-year-old woman with a history of well-differentiated adenocarcinoma of the cecum, stage pT3N1M0, treated ten years before with right hemicolectomy and adjuvant chemotherapy (Capecitabine and Bevacizumab). She developed painless obstructive jaundice of sudden onset. Computed tomography (CT) showed an ampullary nodule with secondary dilatation of the biliary and the pancreatic ducts. Subsequent duodenoscopy and endoscopic ultrasound identified the presence of multiple 3-10 mm tumor-like nodules from the first to the second duodenal knee, the largest one infiltrating

Situs inversus totalis (SIT) is a rare condition characterised by the reversed positioning of abdominal and thoracic viscera. The anomaly poses a significant anatomical challenge during routine endoscopic procedures, including endoscopic retrograde cholangiopancreatography (ERCP). Here, we present the case of a 51-year-old patient with SIT and obstructive jaundice due to a periampullary mass. Initial ERCP attempts at an external facility for biliary decompression were unsuccessful, prompting referral to our center. Multidisciplinary consensus recommended preoperative ERCP followed by a Whipple

Undifferentiated pleomorphic sarcoma (UDS) is a rare and aggressive form of cancer that poses significant diagnostic and therapeutic challenges. The most common sites of this malignancy include the extremities. It is less common in visceral organs and the liver has rarely been reported as a primary site of disease. This subset of cancer is characterized by anaplastic and highly heterogeneous cells that lack specific markers of differentiation, making it difficult to identify and treat effectively. Due to vague symptoms that mirror other pathologies of the affected organ(s), UDS can present lik
