Rethinking policies for the retention of allied health professionals in rural areas: a social relations approach.

OBJECTIVE: Retaining allied health professionals in rural areas is a recognised problem. Generally the literature has concentrated on three elements: practitioner needs, community needs and organisational needs. There has been little attempt to focus other types of social relations in which health practitioner retention and recruitment takes place. The aim of this paper is to question the present dominant hierarchical approach taken in relation to the retention of allied health professionals in rural localities. METHODS: The data derives from a survey in Southwest Victoria, Australia. The sam

Intermediate care: for better or worse? Process evaluation of an intermediate care model between a university hospital and a residential home.

BACKGROUND: Intermediate care was developed in order to bridge acute, primary and social care, primarily for elderly persons with complex care needs. Such bridging initiatives are intended to reduce hospital stays and improve continuity of care. Although many models assume positive effects, it is often ambiguous what the benefits are and whether they can be transferred to other settings. This is due to the heterogeneity of intermediate care models and the variety of collaborating partners that set up such models. Quantitative evaluation captures only a limited series of generic structure, proc

Get Connected: an HIV prevention case management program for men and women leaving California prisons.

Individuals leaving prison face challenges to establishing healthy lives in the community, including opportunities to engage in behavior that puts them at risk for HIV transmission. HIV prevention case management (PCM) can facilitate linkages to services, which in turn can help remove barriers to healthy behavior. As part of a federally funded demonstration project, the community-based organization Centerforce provided 5 months of PCM to individuals leaving 3 state prisons in California. Program effects were measured by assessing changes in risk behavior, access to services, reincarnation, and

Social support in the neonatal intensive care unit.

That having a baby in hospital undergoing intensive care is a stressful experience is beyond dispute. Every parent, no matter how well-developed their own personal resources, depends to some extent on the support of others within their social system to cope with this stress. Grandparents occupy a special place in support networks and their role was studied in families with a very low birthweight baby. Both professional and lay perspectives were investigated and 93 families participated. Questionnaires were sent to family members 1 month after the baby's birth and 1 month after discharge home.

Nationwide Centralized Manufacturing and Fresh Distribution of TCRαβ/CD19-DepletedGrafts: A Real-World French Experience.

TCRαβ/CD19 depletion enables mismatched and haploidentical hematopoietic stem cell transplantation (HSCT) without the use of a post-transplant alkylating agent and is particularly relevant for pediatric nonmalignant disorders. However, the complexity of graft engineering limits its availability to specialized centers. Indeed, implementing such a process and obtaining regulatory approval are resource-intensive endeavors that require sufficient patient volume. To address these limitations, we established a centralized platform for the real-time nationwide distribution of fresh TCRα/β/CD19-deplet

Evaluation on the efficiencies of centers for disease control and prevention in China: results from a 12-year national survey.

BACKGROUND: The disease prevention and control system represents a fundamental component of public health services. Although the Chinese government has consistently prioritized and increased funding for Centers for Disease Control and Prevention (CDCs), whether these additional resources have been effectively translated into improved public health outcomes remains indeterminate. This study was designed to evaluate the operational efficiency of CDCs in China, objectively present the changes in efficiency values, and provide an evidence base for optimizing their service effectiveness. METHODS:

The importance of contextually specific support relationships in implementing programs to link people to medication for opioid use disorder (MOUD) treatment during reentry from county jails.

BACKGROUND: This study uses the Practical, Robust, Implementation, and Sustainability Model (PRISM) and Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) model to describe how features of jail contexts are associated with the number of people linked to medication for opioid use disorder (MOUD) and sustainment of jail linkage programs implemented in Wave 1 of the HEALing Communities Study in Kentucky (HCS-KY) from 2021 to 22. The HCS-KY is part of a parallel-group, cluster-randomized wait-list controlled trial examining the effects of supporting wide-scale implementation

Measuring residents' perceptions of social-ecological healthy communities: development and initial psychometric evaluation of a multidimensional scale.

INTRODUCTION: Effective community health promotion requires information about residents' health-related characteristics and the interpersonal, environmental, organizational, and policy supports they perceive. The Multidimensional Assessment Scale for Social-Ecological Healthy Communities MASE-HC was designed as a multidimensional resident-reported instrument, not as a unidimensional measure or an objective community-level rating. We report its development and initial psychometric evaluation. METHODS: Items were generated through literature review, a two-round Delphi survey, content-validity a

Artificial intelligence in colonoscopy for colorectal cancer prevention: a comprehensive narrative review of technology, evidence and clinical implementation.

BACKGROUND AND OBJECTIVE: Adenoma miss rates (AMRs) reduce the effectiveness of colonoscopy in preventing colorectal cancer (CRC). Artificial intelligence (AI) systems are being developed to address this limitation. This article reviews current evidence on their performance, prevention potential, real-world implementation, and cost-effectiveness, while offering practical guidance for gastroenterologists. METHODS: A comprehensive literature search was conducted across PubMed/MEDLINE, Embase and Cochrane Library databases from January 2015 to February 2026. All studies focusing on AI on colonos

Implementing Integrated Whole-Person Care for Chronic Pain: Lessons from VA's Multisite wHOPE Study.

BACKGROUND: Guidelines recommend multimodal, biopsychosocial care for chronic pain, but implementing this care on a large scale can be challenging. Recently, the Whole Health Options and Pain Education (wHOPE) study implemented and evaluated a Whole Health Team (WHT) intervention at six VA medical centers across the USA. WHT is an interdisciplinary team composed of a medical provider, a complementary and integrative health (CIH) provider, and a health coach who together provide holistic, non-pharmacological pain care. In a randomized controlled clinical trial of 764 VA patients with chronic pa

Financial Hardship and Unhealthy Lifestyles: Perspectives and Solutions From Breast Cancer Survivors and Their Care Teams.

IntroductionFinancial hardship may undermine healthy lifestyle behaviors that are important for preventing avoidable recurrence and death during survivorship after breast cancer diagnosis. Significant research has characterized these challenges and disparities, but relatively little research has identified which strategies patients and their teams may prefer to overcome these challenges. We employ an assets lens to highlight patient-identified strategies to circumvent barriers across Social-Ecological Model (SEM) levels.MethodsWe conducted a secondary qualitative analysis of semi-structured in

Beyond antigen matching: compatibility intelligence theory for transfusion as an emergent biological system.

BACKGROUND: Despite major advances in serologic testing, extended phenotyping, and blood group genomics, clinically similar transfusion exposures may result in markedly different immune and clinical outcomes. Existing compatibility strategies do not fully explain this biological variability. OBJECTIVES: To examine transfusion compatibility as an emergent donor-recipient biological state and propose a systems-level conceptual framework that integrates established biological determinants into a testable model for future precision transfusion medicine. METHODS: This narrative review critically

Decision fatigue among military primary care physicians: a retrospective study.

BACKGROUND: Decision fatigue, based on Baumeister's "Strength Model of Self-Control", suggests that the quality of decision-making declines with the accumulation of decisions over time. This phenomenon, previously observed in various professional fields, may impact military physicians who make critical and organizational decisions. We aimed to assess how decision-making changes throughout the workday among military physicians in a clinic setting. METHODS: A retrospective observational analysis was conducted using a database of 2 million medical encounters from the military medical record syst
