Magnitude and Determinants of Placebo Response in Acute Migraine Trials: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: Migraine is a highly prevalent and disabling neurologic disorder. Beyond drug-specific mechanisms, the role of contextual, individual-related, and disease-related factors remains poorly characterized. We quantified placebo response in randomized controlled trials (RCTs) of acute migraine treatments and identified patient-level, drug-level, and study-level determinants. METHODS: This meta-analytic review searched PubMed, Embase, Cochrane Central, ClinicalTrials.gov, and the European and Japanese Trial Registers from inception to November 2025 for published/unpublishe

Diet Versus Exercise in Cardiometabolic Disease: Are Two Always Better Than One?

PURPOSE OF REVIEW: While studies have established the independent benefits of diet and exercise, comparatively few have assessed these modalities side by side, evaluating diet-only, exercise-only, and combined interventions under comparable conditions. This review focuses on randomized trials and meta-analyses that provide such direct comparisons, emphasizing outcome-specific differences and the circumstances in which combining both modalities yields added benefit. RECENT FINDINGS: Evidence indicates that the superiority of combined diet and exercise interventions is outcome-dependent rather

Guidance for Fecal Microbiota Transplantation Trials in Ulcerative Colitis: The Second ROME Consensus Conference.

BACKGROUND: Fecal microbiota transplantation (FMT) is emerging as a potential treatment modality for individuals living with inflammatory bowel disease (IBD). Despite its promise, the effectiveness of FMT for treating IBD, particularly for ulcerative colitis (UC), still requires thorough clinical investigation. Notwithstanding differences in methodologies, current studies demonstrate its potential for inducing remission in UC patients. Therefore, standardized and robust randomized clinical trials (RCTs) are needed to further support its efficacy for managing UC. The aim of the second Rome Cons

Association of candidate surrogate endpoints with overall survival in advanced biliary tract cancer.

BACKGROUND & AIMS: Surrogate endpoints are increasingly used in biliary tract cancer (BTC) trials. While this may expedite drug approval and decrease costs, surrogate endpoints may not always correlate with an overall survival (OS) advantage. We aimed to explore the association of progression-free survival (PFS), objective response rate (ORR) and disease control rate (DCR) with OS at the trial- and patient-level. METHODS: For the trial-level analysis, we performed a systematic review of Pubmed/MEDLINE, Embase, Cochrane, clinicaltrials.gov and conference proceedings for phase II-III trials in
