Serum antibodies to oxidized low-density lipoprotein and ceroid in chronic periaortitis.

The incidence of serum antibodies to human low-density lipoprotein, to oxidized low-density lipoprotein, and to ceroid extracted from human atheroma was assessed in 100 subjects using an adaptation of the enzyme-linked immunosorbent assay technique. Patients with chronic periaortitis, subclinical chronic periaortitis, and ischemic heart disease, and "elderly control" individuals were compared with young, healthy adults. Provided that precautions were taken to prevent oxidation of the low-density lipoprotein during the assay, antibodies were not found to native human low-density lipoprotein. An

The importance of asking a thorough social history; a case report of aluminum pneumoconiosis.

Aluminum induced lung disease is associated with pulmonary fibrosis, granulomatous pneumonia, and pulmonary alveolar proteinosis. Recognizing aluminum pneumoconiosis remains important as demand for aluminum production continues to rise. The case presents a 53-year-old woman with incidentally noted lung nodules and hilar lymphadenopathy on CT imaging. Despite being asymptomatic, imaging findings prompted referral to a pulmonologist with CT chest demonstrating bulky mediastinal lymphadenopathy with dominant right paratracheal mass as well as several lung nodules in the bilateral lungs. The patie

A 66-year-old man who was on oral medication for type 2 diabetes experienced a rapid decline in glycemic control (increase in glycosylated h … using a 22-G needle, but no malignant findings were observed. There were no signs of sialadenitis, retroperitoneal fibrosis, nephropathy, or other conditions associated with IgG4-related diseases. Distal pancreatectomy was performed; a 23-mm white mass was resected from the pancreatic tail. A histopathological examination showed advanced inflammatory cell infiltration ma …

Histopathology of vasculitis.

The range of pathologies that are related to primitive vasculitis is broad, complex and not as typical as we would expect. Clinicians should be aware that several forms of primitive and systemic vasculitis, regardless of the size of the affected vessel, may exhibit identical histological alterations. This observation has important clinical implications as it means that cases of vasculitis do not correspond clinically and histologically. Thus, while histology remains the diagnostic gold standard, it can be used only as part of the most complete clinical assessment possible. Another point worth

Clinical course after corticosteroid therapy in IgG4-related aortitis/periaortitis and periarteritis: a retrospective multicenter study.

INTRODUCTION: Immunoglobulin G4 (IgG4)-related aortitis/periaortitis and periarteritis are vascular manifestations of IgG4-related disease. In this disease, the affected aneurysmal lesion has been suspected to be at risk of rupture. In this study, we aimed to clarify the clinical course after corticosteroid therapy in IgG4-related aortitis/periaortitis and periarteritis. METHODS: We retrospectively evaluated clinical features, including laboratory data, imaging findings and the course after corticosteroid therapy, in 40 patients diagnosed with IgG4-related aortitis/periaortitis and periarteri

[Immunoglobulin G4 (IgG4)-related disease. A review of head and neck manifestations].

Immunoglobulin G4 (IgG4)-related disease (also known as hyper-IgG4 disease) is a recently defined emerging condition with highly heterogeneous clinicopathological features and variable disease manifestations. This disorder is characterized by unifocal or multifocal (multiorgan) involvement by tumefactive plasma cell-rich inflammatory infiltrates associated with prominent fibrosclerosis. This not uncommonly interferes with organ function resulting in diverse clinical symptoms. The autoimmune pancreatitis represents the prototype of this disease; however, to date almost all organs have been repo

Wegener granulomatosis with back pain, periaortitis, and dural inflammation developing while receiving monthly cyclophosphamide.

A 61-year-old white man was admitted to our department because of severe back and upper abdominal pain of 1 month's duration. The patient was diagnosed with Wegener granulomatosis 10 months before the presentation based on chronic otitis media, hoarseness, and hemoptysis; positive c-ANCA; and laryngeal and lung biopsies showing multinucleated giant cells. The patient was treated with monthly injections of cyclophosphamide (1-1.5 g per month) and 80 mg prednisone daily with rapid improvement. Prednisone dose was tapered off and 1 month before the present admission, the patient developed severe

Conservative treatment of acute Ormond's disease.

Ormond's disease is a chronic inflammatory process of the retroperitoneum, which sometimes takes a very acute course. We report the case of a man with acute bilateral hydronephrosis. Diagnosis was based on typical criteria seen by magnetic resonance imaging, which made biopsy unnecessary.

Crystal-storing histiocytosis and immunocytoma associated with multifocal fibrosclerosis.

AIMS: Crystal-storing histiocytosis is a rare disorder described in patients with lymphoproliferative diseases, mainly in cases of multiple myeloma but also in lymphoplasmacytic lymphoma (immunocytoma). Most cases involve one single organ which, in the majority, is related directly to the presence of tumour. We describe a 44-year-old man with a clinical picture of multifocal fibrosclerosis (with mesenteric panniculitis, peritoneal, mediastinal and orbital fibrosis) in which the autopsy showed a systemic infiltrate of crystal-storing histiocytes and functional alteration of the organs involved,

Localized retroperitoneal fibromatosis causing intestinal obstruction in a cynomolgus monkey (Macaca fascicularis).

Gross examination of a 24-month-old, male cynomolgus monkey (Macaca fascicularis) revealed obstruction of the ileum by a mass that entrapped and compressed the ileocecal junction. The mass was well circumscribed, firm, and white on cut surface. Histologically, the mass consisted of spindle-shaped cells arranged in interweaving bundles or as narrow cords and individual cells widely separated by dense collagen. A diagnosis of localized retroperitoneal fibromatosis was made based on the characteristic gross and microscopic findings and isolation of type D simian retrovirus, serotype-2, from splee

[An accident related to a seat belt. Lymphocoele and extrinsic stenosis of the ureter in a transplant patient. Reparative surgery (author's transl)].

Trauma to a graft by a seat belt in a young adult, 18 months after transplantation. Drainage shortly after of a voluminous perirenal haematoma, abundant discharge of lymphorrhoea taking two months to resolve. Good recovery of the graft from both a radiological as well as biological standpoint. 4 years after the trauma, leucocytes in the urine associated with impairment of function of the graft. Discovery of a large hydronephrosis related to retro-peritoneal fibrosis. Ureterolysis with resection of the affected part of the ureter followed by anastomosis. Return to normal of renal function with

Correlations between IgG4-related disease, autoimmune pancreatitis, and allergic diseases.

IgG4-related disease (IgG4-RD) is a relatively recently described condition whose etiology and pathophysiology remain unclear. Histopathological features include dense lymphoplasmacytic tissue infiltration with numerous IgG4-positive plasma cells, storiform fibrosis, and obliterative phlebitis. Most individuals with IgG4-RD are asymptomatic; however, the disease can affect nearly any organ, leading to a wide range of clinical presentations. The four most common patterns are hepatopancreatobiliary disease, systemic disease, head and neck disease, and retroperitoneal disease. Diagnosis is challe

A role of FDG PET/CT for Response Assessment in Large Vessel Disease?

Currently, a large amount of evidence-based data clearly demonstrates the usefulness of [18F]FDG PET/CT in the diagnosis of several infectious and inflammatory diseases, including those related to the large vessels. The aim of this article is to clarify whether, beyond initial diagnosis, [18F]FDG PET/CT may have a role in treatment response assessment in inflammatory or infectious diseases of the large vessels, including large vessel vasculitis, vascular graft infection, retroperitoneal fibrosis/chronic periaortitis and infective native aortic aneurysms. Rapidly accumulating data suggest that …
