Transvenous placement of inferior vena cava (IVC) filters has become commonplace in selected patients with deep venous thrombosis (DVT) and pulmonary embolism (PE). IVC filters have been shown to have excellent therapeutic efficacy and low complication rates. Penetration of the IVC by filter hooks or struts has been reported and commonly noted to be inconsequential. We report a laceration of a lumbar artery by a stainless steel Greenfield (SSG) filter strut that resulted in a near fatal hemorrhage, and review the world literature on caval perforation by IVC filters.

OBJECTIVE: To evaluate the clinical effectiveness of retroperitoneal drainage following lymphadenectomy in gynecologic surgery.
METHODS: One hundred thirty-seven consecutive patients undergoing systematic lymphadenectomy for gynecologic malignancies were randomized to receive (Group A, 68) or not (Group B, 69) retroperitoneal drainage. The pelvic peritoneum and the paracolic gutters were not sutured after node dissection. Perioperative data and complications were recorded.

The purpose of the present study was to develop a technique for laparoscopic aortic aneurysm resection and aortobifemoral bypass through an anterior retroperitoneal approach. The surgical morbidity rate of 603 patients who underwent lymphadenectomy after primary chemotherapy for clinical stages II and III testis cancer from 1982 to 1992 was reviewed. There were 144 complications in 125 patients (20.7%). The majority of patients (93%) had a tumor volume of greater than 5 cm. Five patients died 3 to 47 days postoperatively, for an operative mortality rate of 0.8%. The sympathetic skin response (SSR) permits recording of the sympathetic amyelinic fibres activity in relation to the function of sudoriparous glands. We recorded the SSR to check the efficacy of "nerve-sparing" technique in surgery of the sympathetic retroperitoneal postganglionic fibres following lymphadenectomy in nonseminomatous testis cancer. The results showed that SSR can be used to verify the integrity of the lumbar sympathetic chains after retroperitoneal lymphadenectomy.

An unusual case is presented in which a massively dilated common bile duct produced a confusing CT image of multiple cystic areas within the abdominal cavity. Cholangiography and CT-cholangiography were useful in establishing the correct diagnosis. The differential diagnosis of cystic retroperitoneal masses is discussed.

A 29-year-old woman, admitted with a left thigh abscess, was subsequently found to have tuberculosis of the spine. The disease was complicated by fistulous involvement of the sigmoid colon which led to bowel perforation and dissection of the resulting retroperitoneal abscess into the thigh. The clinical presentation was that of a polymicrobial soft tissue abscess, with the tuberculous etiology delineated only on further evaluation.

Eight patients with renal or perirenal abscesses were evaluated by 67gallium scintigraphy. The correlation of localized 67gallium uptake to the inflammatory lesion was proved by tissue diagnoses in 7 patients and by response to treatment and repeated studies in the remaining case. 67Gallium scanning was found to be an accurate, non-invastive technique. It should be applied early in diagnosing inflammatory lesions of the retroperitoneum and for examining fever of unknown origin.

A patient with leukemic reticuloendotheliosis is described who two years following splenectomy developed massive retroperitoneal, abdominal, and mediastinal lymphadenopathy. Therapeutic intervention was necessary because the bulky tumor masses produced disabling pain in the back and chest regions. Low-dose radiation to the involved fields produced prompt regression of the adenopathy along with dramatic pain relief. This case suggests that hairy cells may be quite sensitive to radiation, and this therapeutic modality should be considered, especially when local problems are present.
